    Abstract Introduction: Laryngomalacia is the most common congenital laryngeal alteration, with spontaneous resolution in most cases. However, in the face of more severe presentations of the disease, it is necessary to perform supraglottoplasty surgery. Studies have been dedicated to researching changes in swallowing in children with laryngomalacia before and after surgical intervention.
    Objectives: To identify the prevalence of oropharyngeal dysphagia in children with pre and postsupraglottoplasty laryngomalacia.
    Data Synthesis: A search strategy was developed with terms and entreterms to designate a population pediatric with laryngomalacia, exposure supraglottoplasty, and outcome frequency of dysphagia, adapted to the requirements of the main databases in the health area. The analysis of the records found was performed by two independent examiners and, in the end, 6 articles were included in the study. The articles found enabled a sample of 330 children with laryngomalacia, 311 of whom underwent supraglottoplasty. Among the included studies, 5 were grouped and meta-analyzed. After supraglottoplasty surgery, a 59% reduction in the prevalence of oropharyngeal dysphagia was identified, with high heterogeneity I 2  = 93%.
    Conclusion: Despite the heterogeneity of the sample, the supraglottoplasty procedure significantly reduces the prevalence of dysphagia in children with laryngomalacia.

    Abstract Objetivo: Desenvolver um instrumento de rastreio para a identificação de crianças com risco para disfagia, em ambiente hospitalar.
    Método: O Instrumento de Rastreio para o Risco de Disfagia Pediátrica (IRRD-Ped), constituído por 23 questões, foi desenvolvido por fonoaudiólogos, após revisão da literatura. Ele foi proposto para ser aplicado aos responsáveis por crianças em internação hospitalar. O IRRD-Ped foi enviado a juízes para análise, tendo sido também realizado um estudo piloto. Para verificar a validade de critério, aplicou-se o IRRD-Ped aos responsáveis por crianças internadas e, posteriormente, realizou-se, com estas crianças, avaliação clínica da deglutição, através do Protocolo de Avaliação da Disfagia Pediátrica (PAD-PED). Os responsáveis assinaram o Termo de Consentimento Livre e Esclarecido. Os sujeitos foram separados em dois grupos (com disfagia e com deglutição normal), sendo verificada a associação entre as questões do IRRD-Ped e o resultado do PAD-PED, através do teste qui-quadrado de Person ou exato de Fisher. Definiu-se o ponto de corte para presença de risco para disfagia através da Curva ROC. A confiabilidade do IRRD-Ped foi verificada pelo coeficiente α de Cronbach.
    Resultados: A amostra foi constituída por 40 crianças com mediana de idade de 3,7 meses. Verificou-se associação estatisticamente significativa em oito itens do instrumento. A consistência interna do IRRD-Ped foi de 0,828. O ponto de corte para o risco de disfagia foi de cinco pontos (sensibilidade = 100% e especificidade = 80%).
    Conclusão: Devido aos satisfatórios resultados encontrados, deve-se dar prosseguimento ao processo de validação do IRRD-Ped.

    Abstract Purpose: To evaluate the variation of cardiorespiratory parameters in the feeding of cardiopathy infants.
    Methods: A cross-sectional, controlled study was performed on infants from 0 to 6 months of age, both genders, at a referral hospital for pediatric cardiac surgery. They were divided into a group study of infants with a medical diagnosis of congenital heart disease, post-cardiac surgery and in a control group composed of infants with no co-morbidities pre-established as risk factors for dysphagia. Vital signs monitoring and pulse oximetry were performed before, during and after clinical evaluation of swallowing in both groups.
    Results: There was a significant difference in heart rate between the two groups prior to the evaluation, however, a marked increase was observed in the study group during the oral route. The respiratory rate variation was significant during the maternal breast offer in the study group, which also showed a greater decrease in saturation when compared to the control group.
    Conclusion: There is variation of cardiorespiratory parameters during feeding of cardiopathy infants.

    Title translation Clinical signs of dysphagia in infants with acute viral bronchiolitis.
    Abstract Objective: To determine the occurrence of clinical signs of dysphagia in infants with acute viral bronchiolitis, to compare the respiratory parameters during deglutition, and to ensure the intra- and inter- examiners agreement, as well as to accomplish intra and interexaminators concordance of the clinical evaluation of the deglutition.
    Methods: This was a cross-sectional study of 42 infants aged 0-12 months. The clinical evaluation was accompanied by measurements of respiratory rate and pulse oximetry. A score of swallowing disorders was designed to establish associations with other studied variables and to ensure the intra- and interrater agreement of clinical feeding assessments. Caregivers also completed a questionnaire about feeding difficulties. Significance was set at p<0.05.
    Results: Changes in the oral phase (prolonged pauses) and pharyngeal phase (wheezing, coughing and gagging) of swallowing were found. A significant increase in respiratory rate between pre- and post-feeding times was found, and it was determined that almost half of the infants had tachypnea. An association was observed between the swallowing disorder scores and a decrease in oxygen saturation. Infants whose caregivers reported feeding difficulties during hospitalization stated a significantly greater number of changes in the swallowing evaluation. The intra-rater agreement was considered to be very good.
    Conclusions: Infants with acute viral bronchiolitis displayed swallowing disorders in addition to changes in respiratory rate and measures of oxygen saturation. It is suggested, therefore, that infants displaying these risk factors have a higher probability of dysphagia.

    Abstract Purpose: To perform a systematic review of screening instruments for dysphagia available in the literature.
    Methods: For the selection of studies, the following descriptors were used: "questionários", "questionnaires", "transtornos de deglutição", "deglutition disorders", "programas de rastreamento", and "mass screening". The online databases used for the research were Virtual Health Library (LILACS, IBECS, MEDLINE, Cochrane Library, SciELO) and PubMed. The research was performed from April to June 2013. Selection criteria articles in English, Portuguese, and Spanish, regardless of the year of publication, whose title, abstract, or text had any relation to the purpose of the research. After reading the articles in their entirety, identification data and method of the articles were extracted for later analysis.
    Results: The research carried out from the proposed descriptors produced 1,012 articles. After reviewing the titles, summaries, and fully reading the articles, 20 studies were chosen. The publications on instruments for the identification of dysphagic patients started in 1999, and 50% of the analyzed studies were carried out in the USA. The methods used on the instruments were questionnaires, observation of patient's clinical signals and symptoms, the request for execution of some orofacial movements, and swallowing test with water or food.
    Conclusion: Screening instruments in dysphagia are fairly heterogeneous and have been developed for different audiences with the main objective of identifying patients with swallowing disorders.

    Abstract Arthrogryposis is a rare, multiple, congenital syndrome of non-progressive nature characterized by a series of genetic malformations, as well as stiffness and joint contractures. This is a clinical case study whose objective is to describe speech-language pathology disorders through the evaluation process in a case of arthrogryposis in Pediatrics. The medical records of a patient were analyzed from birth. A complete clinical evaluation of pediatric dysphagia was performed, establishing a diagnosis of severe oropharyngeal dysphagia evidenced by functional and structural impairments. Hearing loss was detected in association with this condition.

    Abstract Purpose: To describe the breastfeeding initial conditions for premature infants.
    Methods: The sample consisted on 26 mother/baby dyads. The babies had an average of 36.1 weeks corrected gestational age and were hospitalized in a Neonatal Intensive Care Unit in Southern Brazil. Data was collected from medical records, and the observation of the dyads was conducted during feeding, using the Observation and Evaluation of the Breastfeeding Protocol. Favorable and unfavorable behaviors were registered regarding position, responses, suction, affection and anatomy of the breast. After the observation, a questionnaire was applied to the mothers.
    Results: The best results refer to the position of mother/child and affection and the poorer ones to the infant responses. Statistically significant correlations by the Spearman Correlations Coefficient were observed between some items of the protocol and other study variables.
    Conclusion: Most premature infants from this sample presented a satisfactory initiation on analyzed aspects referring to breastfeeding. Still, we emphasize the need of practices for breastfeeding effectiveness and its encouragement in this population.

    Abstract Introduction: Evidences suggesting that a short lingual frenulum is a genetic abnormality strictly related to cleft palates. This aspect shows the need for a thorough assessment of these subjects with cleft, verifying the alteration found, as well as possible therapeutic and/or surgical indication.
    Objectives: To relate a speech therapy assessment case of a child with cleft lip and palate and short lingual frenulum.
    Resumed report: A male child, eight years old, with cleft lip and palate was referred by the plastic surgery department for speech-language therapy. After the speech therapy assessment, incisive transforaminal cleft bilateral, with fistulae in the anterior palate, and with sensitivity and tone-appropriate oral facial structures were identified. It is an appropriate clinical exam of structural mobility, except for the tongue, which showed difficulty in performing lateralization, elevation, and vibration movements, noticing the need for lingual frenulum assessment. Therefore, the Lingual Frenulum Evaluation Protocol was applied, resulting on score 6 (best result: 0; worst result: 8) on general evaluation, considering the frenulum as altered. On the functional evaluations, obtained score 25, on which the best is 0 and the worst is 40, suggesting a possible interference on the functionality because of the lingual frenulum.
    Conclusion: Besides presenting incisive transforamen cleft lip and palate, the child also has short lingual frenulum, which interferes on the patient’s tongue functionality. It is noticed, in this case, the importance of a speech therapy assessment, which can adapt therapy to the cleft and frenulum deficits, as well as return to the plastic surgery department the alteration found, suggesting possible correction.
