Abstract Aim: In vitro evaluation of cleanliness of root canal walls of primary molars after preparation with the Self-Adjusting-File and Mtwo-instruments and final irrigation with citric acid and sodium hypochlorite.
Materials and methods: s Study Design: In 23 matched pairs, teeth were prepared either with SAF or with Mtwo NiTi-instruments, and final irrigation was performed with 2 mL citric acid and 4 mL NaOCl. Roots were split longitudinally, SEM-images were taken, and smear layer was evaluated by two blinded observers using a four-grade score. Statistical evaluation was performed with Mann-Whitney-U-Test and Wilcoxon Signed Rank Test (P<0.05).
Results: No significant difference between SAF and Mtwo (P=0.9454) was observed. Overall removal of the smear layer was significantly better in the coronal part of the root canal than in the apical one (P=0.0004393). Mtwo showed no significant difference in cleanliness when comparing the coronal and apical part of the root canal (P=0.1089), whereas SAF cleaned the coronal part of the root canal significantly better than the apical part (P=0.00108).
Conclusion: None of the two instruments was superior concerning cleanliness in root canals of primary molars. Both show good cleaning ability when using an irrigation protocol with citric acid and sodium hypochlorite.

Abstract Background: This study introduces a wild radish population collected from Yelbeni in the Western Australian grainbelt that evolved an early silique abscission (shedding) trait to persist despite long-term harvest weed seed control (HWSC) use. In 2017, field-collected seed (known herein as Yelbeni) was compared to surrounding ruderal and field-collected populations in a fully randomized common garden study.
Results: The Yelbeni population exhibited a higher rate of silique abscission when compared to the ruderal populations collected from the site before wheat (Triticum aestivum L.) harvest (assessed at soft dough stage, Zadoks 83). A similar common garden study was conducted in the subsequent season (2018) using progeny reproduced on a single site without stress. The HWSC-selected progeny (Yelbeni P) shed 1048 (±288) siliques before wheat maturity at the soft dough stage (Zadoks 83) compared to 25 (±7) siliques from the pooled control populations. The Yelbeni P population only flowered 6 days earlier (FT
Conclusion: This study demonstrates that the repeated use of HWSC can lead to the selection of HWSC-avoidance traits including early silique-shedding before harvest and/or locating siliques below the harvest cutting height for interception. © 2024 The Authors. Abstract Background: Proximal phalangeal fractures are one of the most commonly treated hand injuries in children. Conservative management of these fractures is often to splint for 5 weeks post injury, despite children presenting as clinically healed at 3 weeks post injury. Therefore, we investigated the effect of splinting for only 3 weeks in children who present with clinically healed proximal phalangeal fractures at 3 weeks compared with usual care.
Methods: Participants (n=80, aged 10.3 ±2.5 years) presenting to the Hand Clinic of a tertiary Children's Hospital in Sydney, Australia, were randomly allocated into a Current Protocol and a New Protocol group. Following were the inclusion criteria: aged between 5 and 16 years; present with an non-displaced or minimally displaced and stable fracture; no surgical intervention; assessed as clinically healed at 3-week visit. The primary outcome measure was total active motion (TAM) of the injured digit compared with the contralateral digit (deg), at 5 weeks post injury. Secondary outcome measures were grip strength, and a parent-reported questionnaire. Statistical analysis used χ 2 test and the absolute difference described by a 90% CI. The New Protocol was considered noninferior if the 90% CI overlap was > 20% of the Current Protocol. Analysis was by intention to treat.
Results: There was a 10% loss to follow up at 5 weeks (Current Protocol =4, New Protocol =4). All CIs between groups overlapped by >10%. TAM 90% CI for Current Protocol was 17.7 to 5.4 degrees and for the New Protocol was 4.7 to 1.6 degrees.
Conclusions: A change in practice is warranted to cease immobilization for children with conservatively managed proximal phalangeal fractures who present as clinically healed at 3 weeks. Therapist assessment of fracture healing is an appropriate indicator for intervention and can be utilized in a therapist-led model of care.
Level of evidence: Level 1-noninferiority randomized control trial with 2 parallel arms.

Abstract Over the past decade qualitative evidence synthesis (QES), a range of methods for synthesising qualitative research evidence, has become a valued form of evidence for guideline producers who wish to understand more about patient preference and acceptability of treatments. The surge in interest in living systematic reviews and the appearance of living guidelines as a response to the COVID-19 pandemic potentially weaken the value and usability of QES.There are currently no published methods for producing living QES, and if QES are to remain of worth to guideline producers then methods for the rapid, frequent updating of them will need to be developed. We discuss some of the similarities and differences between qualitative and quantitative evidence syntheses and highlight areas where development is needed if reviewers are to progress with living approaches to QES.

Abstract Introduction: The aim of this ex vivo study was to compare 2 techniques for removal of Thermafil obturators from curved root canals in mandibular molars.
Methods: Sixty mesial root canals in extracted mandibular molars were distributed into matched pairs according to degree and radius of curvature and were prepared to size 35/.04 and obturated with Thermafil obturators size 35. Each tooth was embedded in resin, mounted in a mannequin head, and isolated with rubber dam to simulate clinical conditions. Removal of obturators was undertaken under a dental operating microscope by an experienced endodontist using either the FragRemover loop device, or reciprocating NiTi instruments. Pre- and postoperative micro-CT scans were taken to compare the amount of remaining filling material and of removed dentin. Eventual problems during removal attempts were recorded.
Results: Using Reciproc, 24 carriers (80%) were removed, 16 in one piece, 8 in small pieces, and in 6 cases removal failed. The FragRemover removed 12 carriers (40%), all of these completely, but in 18 cases removal failed (P < .05). Reciproc left less remaining filling material than the FragRemover (P < .05) but removed significantly more dentin in the apical part of the root canal (P < .01). There was no significant difference concerning the total amount of removed dentin (P > .05) between both groups. Seven complications occurred in the Reciproc group. In 5 teeth, working length could not be reached; in 2 teeth, a Reciproc file R25 fractured. With the FragRemover, 64 complications occurred, in 36 cases, including second removal attempts, the loop slipped over the carrier, in 24 cases the top of the carrier was sheared off. In 9 cases, separation of the core occurred too far apically, not allowing further use of the FragRemover. In 4 cases, the wire loop tore.
Conclusions: Reciproc was more effective in removal of Thermafil carriers, whereas when using the FragRemover less dentin was removed in the apical part of the root canal and more obturation material was left.

Abstract Introduction: Root discoloration is reported after using Ledermix paste for prevention of external inflammatory root resorption in traumatically injured teeth. Therefore, it seems necessary to remove Ledermix completely from the root canal prior to root canal filling. The aim of this study was to evaluate the efficacy of sodium hypochlorite or alcohol with or without passive ultrasonic irrigation in removing Ledermix from standardized grooves in coronal and apical root canal thirds.
Methods and materials: Root canals of ten extracted single-rooted teeth were prepared to size 35 and split longitudinally. Standardized grooves were cut in the coronal and apical part of the root canal and filled with Ledermix paste. The reassembled specimens were irrigated with 1% NaOCl or 80% alcohol delivered with a size 30 syringe or with an ultrasonic device. The amount of remaining Ledermix paste was evaluated by three calibrated observers under 30× magnification using a four-grade scoring system. The data was analysed with a nonparametric ANOVA-type method for longitudinal data in factorial experiments. Pairwise comparisons were adjusted by using the Bonferroni corrections (
Results: For the coronal groove, no statistical difference between passive ultrasonic irrigation with NaOCl or alcohol was found (
    Conclusion: This
