    Abstract The Global Health Consortium at Florida International University developed an end-to-end solution framework based on the input of a diverse panel of experts from middle-income country (MIC) government agencies, public health think tanks, academia, and nonprofit organizations to identify mechanisms to help MIC governments and stakeholders increase access to novel vaccines for infectious diseases. The resultant layout can be deployed to improve vaccine discovery and development, strengthen regulatory processes, and boost vaccine production, access, and implementation. Mechanisms include policies and incentives MIC governments can use to stimulate vaccine investment and activity, as well as actions government agencies can take together with other stakeholders to coordinate efforts or build capabilities. Through a series of individual virtual interviews, we engaged diverse experts from MIC government agencies, public health think tanks, academia, and nonprofit organizations who understand the vaccine ecosystem, immunization policies, and population health financing at global, regional, and country levels. Responses were mapped, and in-depth questions were prepared for a group virtual discussion. This paper is the result of such a group discussion. The panel identified clear opportunities for MICs to improve locally-driven innovations and future access to novel vaccines. It proposes a solution framework for countries considering investing in vaccine research and development and innovation to use as a guide to evaluate the steps they could take to improve such an environment and incentivize innovation in vaccine development. It is hoped that this end-to-end solution framework will become a key resource to help MICs strengthen policies and take more actions to make such improvements.

    Abstract Objectives: To review and describe alternative strategies for the supply of vaccines in Latin America.
    Methods: We conducted a narrative review to explore and describe alternatives for equitable vaccine access in Latin America. We searched and considered the main access strategies reported in the literature through PubMed, Science Direct, and Google Scholar. Additionally, we reviewed the web sites of key stakeholders. The search was conducted using the following keywords: ("access" or "availability" or "acquisition" or "affordability" or "tiered pricing") and ("vaccine"). Subsequently, documents that met the inclusion criteria were selected. Finally, findings were grouped by means of a thematic analysis and an interpretative synthesis.
    Results: Twenty-four publications were included. We identified 5 main topics: current supply strategies, challenges for the acquisition of vaccines, vaccine prices equity, alternative supply strategies, and the advantages and impact of a tiered pricing strategy.
    Conclusions: Our review suggests that tiered pricing can be an tool for accelerating the process of introducing vaccines in low-income countries at affordable prices and for countries that do not adhere to the current procurement mechanisms or are not eligible for Vaccine Alliance because giving countries prices for vaccines that reflect their ability to pay can result in better programmatic and financial planning for the purchase of these vaccines, and in return, vaccine manufacturers can gain access to wider markets However, this model has not been z improve access to vaccines that are aimed only at developing countries, mainly because the market in these countries is not profitable for producers.

    Abstract Objective: To provide an overview of the status of the childhood vaccination schedule in the Americas, outline program structures, and identify updated implementation strategies to improve vaccination coverage following the COVID-19 pandemic.
    Methods: A group of experts in pediatrics, epidemiology, vaccines, and global and public health discussed the current status of the childhood vaccination schedule in the Americas, describing the program structure and identifying new implementation strategies that have the potential to improve vaccination coverage in the post-pandemic context, after the challenges COVID-19 presented for more than two years.
    Results: The Americas currently face a high risk of resurgence of diseases that were previously controlled or eliminated. Therefore, it is important to find new strategies to educate citizens on the risks associated with lower vaccination rates, especially in children.
    Conclusions: New strategies along with strong mobilization of the population and advocacy by citizens are necessary to prevent antivaccination groups from gaining a stronger presence in the region and jeopardizing the credibility of the Expanded Program on Immunization.

    Abstract Objectives: Define a legal framework for electronic medical records (EMRs) and determine the degree to which the countries of the Region of the Americas are prepared in this regard.
    Methods: The methodology consisted of two phases. In the first, a Delphi methodology was used to develop a questionnaire and define the framework and legal domains applicable to EMRs. In the second, the questionnaire was completed in each country. This included identifying national experts and the data collection process.
    Results: Information was collected from a total of 21 countries regarding their situation with respect to legislation on EMRs in the following domains: 1) specific legislation on EMRs; 2) protection of patients' data and secondary use of the data; 3) legislation related to the actions of health professionals; 4) regulation on EMRs and the role of patients; and 5) health standards and EMR promotion programs.
    Conclusions: There has been progress in the Region with respect to new legislation covering the main areas related to EMRs (for example, protection of sensitive data and use of digital signatures). However, it is necessary to continue to address issues such as access to and updating of information in EMRs. This study contributes information on the essential components of legislation on EMRs, and reports on the situation in the Region of the Americas.

    Abstract The Region of the Americas, together with the rest of the world, is preparing to face the major challenges presented by the post-2015 sustainable development agenda (SDA), whose 17 objectives include health and well-being, the reduction of inequity, and action on a wide range of social and environmental determinants of health (1). In these times of change, the first of many lessons learned on the road toward achieving—or in some cases, not achieving—the Millennium Development Goals (MDGs) is that the principle of equity is an ethical imperative essential to guiding, informing, and defining political priorities for action on health and well-being. Rooted in the Health for All movement and in the legacy of Alma-Ata, the principle of equity continues to inspire the spirit of universality and social inclusion required to implement the global and Regional post-2015 agenda, eliminate social inequities in health, and achieve the sustainable development goals.

    Abstract La Región de las Américas, junto con el resto del mundo, se viene preparando para enfrentar los grandes retos que supone la atención de la agenda de desarrollo sostenible posterior al 2015, entre cuyos 17 objetivos se destacan la salud y el bienestar, la reducción de la inequidad y la acción sobre una amplia gama de determinantes sociales y ambientales de la salud (1). Entre las muchas lecciones aprendidas en el camino hacia el logro —esquivo, en algunos casos— de los Objetivos de Desarrollo del Milenio se destaca una primicia fundamental en estos tiempos de cambio: la primacía del principio de equidad como imperative ético para orientar, informar y definir las prioridades de la acción política en materia de salud y bienestar. Enraizado en el movimiento de Salud para Todos y el legado de Alma Ata, el principio de equidad debe continuar inspirando la esencia de universalidad e inclusión social requerida para implementar la agenda mundial y regional posterior al 2015, eliminar las inequidades sociales en la salud y alcanzar los objetivos de desarrollo sostenible.
