Abstract Background and objective: Type 1 diabetes mellitus one of the biggest health concerns around the world, is difficult to manage during adolescence. Among the non-medical methods of controlling this disease is empowerment through self-efficacy. Poor self-efficacy leads to social anxiety and ultimately deficiencies in diabetes. There is also a correlation among health literacy, self-efficacy, and social anxiety. Thus, the present study aimed to evaluate the impact of a literacy promotion training program based on social learning theory on the self-efficacy and social anxiety of adolescents with T1DM.
Methods: The current research is a semi-experimental type that was carried out with the cooperation of 66 adolescents aged 15-18 years with type 1 diabetes in Iran (Tehran, 2022). It has control and intervention groups. The endocrinology and diabetes clinics of the intervention and control groups were randomly selected in a multi-stage manner (endocrine and diabetes clinic of children's medical center hospital for the control group and endocrine and diabetes clinic of Mofid hospital for the intervention group) and the participants were selected by Simple Random Sampling method (draw). The training program designed based on Bandura's social learning theory was used to teach adolescents during seven consecutive sessions of 30-45 min during one week. Questionnaires were completed before and one month after the intervention. Data were analysed in SPSS-25 software.
Findings: The intervention for adolescents with T1DM in intervention group compared to the control group had a significant effect on improve health literacy (P<0.001), self-efficacy (P<0.001), and social anxiety (P<0.05).
Conclusions: The results can also be used to improve the capabilities of adolescents with T1DM, reduce and prevent disease complications, and develop operational-educational programs in the centers from which these adolescents receive various services.

Abstract Aim: 'Helplessness' is one of the psychological concepts that exploring nurses' helplessness during the COVID-19 pandemic can lead to timely intervention and empowerment of nurses.
    Design: Concept development.
Methods: It was carried out using Schwartz-Barcott and Kim's hybrid model.
Results: In the literature review, helplessness is characterized by anxiety symptoms (muscle tension, headache, anorexia and insomnia) and, to some extent, depression (loneliness, guilt, apathy and insensitivity). In the fieldwork, five categories were obtained, including antecedents of helplessness (the nature of the disease, professional responsibility, personal lifestyle disruption and social behaviours), attributes of helplessness (inability to do more for the patient, inability to control the situation, feeling of uselessness, frustration, giving up and uncertainty to continue), consequences of helplessness (increase clinical error, physical manifestations and psychological manifestations), strategies to reduce feelings of helplessness and the difference among helplessness, powerlessness and hopelessness.
Conclusion: Nurses' helplessness occurs when the nurse has no control over the situation and cannot change it.

Abstract Objectives: Moral distress (MD), which is affected by several factors such as futile care provision and is considered the cause of adverse effects such as job dissatisfaction and decreased care quality, is a new concept attracting increasing academic interest. This study aims to assess the correlation between nurses' perception of futile care and MD in neonatal care units.
Material and methods: This descriptive-correlational study was carried out among 115 nurses working in the neonatal intensive care units and neonatal special care units of two hospitals in West Azerbaijan Province during 2020. A demographic information form, the 21-item MD-Pediatric version scale, and the 17-item perception of futile care questionnaire were used to collect data and analysed using SPSS 16 software.
Results: The results confirmed the direct correlation between MD and the perception of futile care. In addition, MD and the nurses' perceptions of futile care were estimated to be moderate.
Conclusion: The results of this study provide evidence to emphasise the need for further studies to investigate other causes of MD in neonatal units and find the solutions to make the work environment more ethical. Furthermore, the results provide the platform needed for hospital and university managers to make the necessary decisions and create the required changes in the educational curriculum of nursing students and provide the appropriate courses for neonatal unit nurses to improve their ability to cope with the MD caused by providing futile care.

Abstract Background: Parenting approach and early childhood experiences are thought to be two important factors in the initiation of substance use.
Aims: We aimed to explore the nurse's role in the prevention of childhood addiction.
Methods: In this qualitative study we conducted in-depth interviews with young men and women who were either in treatment for their addiction or were active drug users as well as with family members and nurses. The data analysis followed Strauss and Corbin's constant comparison method.
Results: The data analysis revealed six categories: traumatic events during childhood, inappropriate parenting approach, Lack of knowledge and a tolerant attitude toward drug use, turning a blind eye on the threat of drug use, nurses' poor experience of drug use prevention, and the lack of a clear definition of the nurse's role in prevention of drug use.
Conclusions: Nurses who work with young people and their families have a special opportunity for prevention, early detection and timely intervention for drug dependency.

Abstract Discharge against medical advice (DAMA) refers to the phenomenon that patient or the patient’s surrogate decides to leave the hospital before the attending physician confirms the patient is discharged. Children are much more vulnerable to such discharges. This process occurs with different mechanisms that identifying them can be helpful in reducing this phenomenon. We aimed to explore the process of parents' decision-making to discharge their child against medical advice. In-depth, semi-structured interviews were conducted with 10 fathers, 10 mothers, 6 nurses and 3 physician assistants and the data were collected to the point of saturation. Grounded theory methodology was adopted for data collection and analysis. The results of qualitative analysis in the field of the parents' decisionmaking on the DAMA revealed 4 main themes: "lack of family-centered care", "disruption of the parenting process", "distrust to the medical team and center" and "psychological strategy of shirk responsibility for child care and treatment ". By providing family-centered care, adopting measures to empowering the families, developing the trust of parents to the health care team and developing a discharge plan from the beginning of children hospitalization with the cooperation of health care team and parents and considering all factors such as child's special health condition and parent's health related perceptions and beliefs, children will not be discharged against medical advice and will experience better outcomes.
