    Abstract Background and purpose: About 56% of symptomatic COVID-19 survivors have been found with neuropsychological comorbidities, such as depression, anxiety, posttraumatic stress disorders (PTSD), and impaired quality of life (QoL). Alongside, antimicrobial, anti-inflammatory, neuroprotective, regenerative, immunomodulatory, cardio-pulmonary health promotive, and psychological benefits of yogic and Ayurvedic intervention are well documented. Therefore, this study aimed to assess the effect of online Yoga (OYI) and Yoga cum Ayurveda intervention (OYAI) on COVID-19-induced depression, anxiety, PTSD, and poor QoL.
    Method: Seventy-two participants (males/females: 33/26) with at least a 3-month back history of symptomatic COVID-19 infection and age (mean ± SD: 32.33 ± 9.9 and 33.04 ± 12.9 for males and females, respectively) were recruited from Patanjali Ayurveda Hospital, Haridwar, India, and All India Institute of Medical Sciences, Rishikesh, Uttarakhand, India, before random allocation into an equal-sized control group (CG), Yoga group (YG) and Yoga cum concoction (YCG) group.     Abstract Niger offers the possibility of producing green hydrogen due to its high solar energy potential. Due to the still growing domestic oil and coal industry, the use of green hydrogen in the country currently seems unlikely at the higher costs of hydrogen as an energy vector. However, the export of green hydrogen to industrialized countries could be an option. In 2020, a hydrogen partnership has been established between Germany and Niger. The potential import of green hydrogen represents an option for Germany and other European countries to decarbonize domestic energy supply. Currently there are no known projects for the electrolytic production of hydrogen in Niger. In this work, potential hydrogen demand across electricity and transport sectors is forecasted until 2040. The electricity demand in 2040 is expected at 2934 GWh and the gasoline and diesel demand at 964 m³ and 2181 m³ respectively. Accordingly, the total hydrogen needed to supply electricity and the transport sector (e.g. to replace 1% gasoline and diesel demand in 2040) is calculated at 0.0117 Mt. Only a small fraction of 5% of the land area in Niger would be sufficient to generate the required electricity from solar PV to produce hydrogen.

    Abstract The identification of frailty among relatively young survivors of childhood cancer has led to a growing body of work aimed at examining potential risk factors and interventions to decrease its burden in this population. In the accompanying article, Schwartz and colleagues present their findings regarding the association between neighborhood characteristics and frailty in survivors of childhood cancer and show that survivors living in resource poor neighborhoods had increased odds of frailty compared with those in resource-rich neighborhoods. This work further supports the need for focused educational initiatives and nutrition and exercise interventions to improve survivors' quality of health.     Abstract Epiploic appendagitis is a rare cause of acute abdomen and is diagnostically challenging as it mimics common causes of acute abdomen. However, advancements in computerised tomography/Ultrasound imaging have improved the frequency and confidence of diagnosing epiploic appendagitis, preventing unnecessary surgeries. We present a case of an acute abdomen who had to undergo laparoscopy before being diagnosed with epiploic appendagitis, underscoring the difficulty in diagnosis.

    Abstract Introduction: Over-the-counter medication use is commonly practised all over the world. However, in a developing country like Nepal, antibiotics form an essential component of OTC drugs. Fever is one of the most common clinical complaints which makes a patient go to the local pharmacy for over-the-counter medication. This study aimed to find out the prevalence of over-the-counter medication use among patients presenting with fever in the Emergency Department in a tertiary care hospital.
    Methods: This descriptive cross-sectional study was conducted among patients who visited the Emergency Department with the complaint of at least one episode of documented or undocumented fever after obtaining ethical approval from the Institutional Review Committee. Data collection was conducted from 24 June 2022 to 30 September 2022. Convenience sampling method was used. The point estimate was calculated at a 95% Confidence Interval.
    Results: Among 332 patients, 314 (94.58%) (92.14-97.02, 95% Confidence Interval) patients used over-the-counter medication. Antibiotic use was seen in 221 (70.38%) patients.
    Conclusions: The prevalence of over-the-counter medication use among patients with fever was found to be higher than the studies conducted in similar studies.

    Abstract Magnetic tweezers (MTs) have become indispensable tools for gaining mechanistic insights into the behavior of DNA-processing enzymes and acquiring detailed, high-resolution data on the mechanical properties of DNA. Currently, MTs have two distinct designs: vertical and horizontal (or transverse) configurations. While the vertical design and its applications have been extensively documented, there is a noticeable gap in comprehensive information pertaining to the design details, experimental procedures, and types of studies conducted with horizontal MTs. This article aims to address this gap by providing a concise overview of the fundamental principles underlying transverse MTs. It will explore the multifaceted applications of this technique as an exceptional instrument for scrutinizing DNA and its interactions with DNA-binding proteins at the single-molecule level.

    Abstract Background Major complications following pancreaticoduodenectomy have a severely deleterious effect on postoperative course, rather than just occurrence of pancreatic fistula. Surgical risk stratification with Braga and WHipple-ABACUS have been proposed and validated. Objective The study aimed at comparing the Braga and WHipple-ABACUS scores for prediction of major complications following pancreaticoduodenectomies. Method This was a prospective observational study at the Tribhuvan University Teaching Hospital from February 2018 to April 2019. After ethical approval, all consecutive 41 patients who underwent pancreaticoduodenectomies were included. Each patient was graded in Braga and WHipple-ABACUS scores. Perioperative events occurring over 30 days were graded as per Clavien -Dindo complications for pancreatic surgery. The predictive value of the scores were assessed using a receiver operating characteristic curve analysis. The categorical data were compared using the Pearson χ2 test or Fisher's exact test. Result Over period of 14 months, total of 41 patients (M:F=2.15:1) with median age of 58 years (range, 21-86) underwent pancreatoduodenectomy. The mean scores were Braga (4.6±3.1) and WHipple-ABACUS (1.8±1.6). Major complications over 30 days were developed in 11 patients with five mortality. There were significant differences in mean values of Braga score (7.0±3.4 vs 3.7±2.6, p-value=0.02) and WHippleABACUS score (3.2±1.8 vs 1.3±1.3, p-value=0.01) in patients with major complications to those without respectively. The area under curves for Braga and WHipple-ABACUS scores were 0.800 and 0.779 respectively. Conclusion Both WHipple-ABACUS and Braga scores are easy to calculate and predict the development of major complications significantly in patients undergoing pancreatoduodenectomy.

    Abstract Zinc phosphide is commonly used as a rodenticide and often abused for intentional self harm. For years, Scientist believed that, It gets converted to phosphine gas by hydrolysis in acidic circumference of the stomach upon ingestion. It is absorbed into the bloodstream and can affect cardiovascular, respiratory, hepato-biliary hematological and gastrointestinal tract. It also results into electrolyte imbalance and ultimately leads to multi organ failure. We present a case of 46 year-old-male with acute pancreatitis due to zinc phosphide ingestion that was successfully treated conservatively. Acute pancreatitis is a rare complication of zinc phosphide poisoning. It is crucial to identify zinc phosphide as the cause of acute pancreatitis and rule out other factors (like alcohol, drugs, genetics) contributing to acute pancreatitis. Interestingly, patient presents with only mild symptoms of poisoning as metabolic acidosis and abdominal discomfort, acute pancreatitis was incidentally diagnosed through lab investigations and radiographs reports after 3-4 days of ongoing conservative treatment. Management was conservative for acute pancreatitis. This article provides a comprehensive review of management of zinc phosphide poisoning induced acute pancreatitis.
