Abstract Background: Demise of mother wreaks havoc in family, society and ultimately nation. Amongst so many countries, India alone contributes one quarter of total world maternal deaths. Millennium Development Goal targeted to cut down maternal mortality up to or less than 109 per lakh of total live births is far away from present level prevailing especially in our institution of Madhya Pradesh in centre of India. Methods: A retrospective analysis of maternal deaths from hospital records and death summaries of one year between Jan 2014 to Dec 2014 at NSCB Medical College and Hospital, Jabalpur in Madhya Pradesh. Results: During the review period, there were 75 maternal deaths and 5509 live births. (Institutional maternal mortality ratio, 1361.1 per lakh live births). All cases were analysed in detail: 70.6% (n = 53) were direct maternal deaths and PIH (n = 34, 45.33%) and Haemorrhage and sepsis (n = 7, 9.33%) were the leading causes of mortality. In indirect causes 29.33% (n = 22), severe anaemia topped (n=14, 18.66%). Women frequently did not attend prenatal clinics (n = 73, 97.33%),were resident of rural areas (n = 67, 89.33%) , referred (n = 68 , 90.66% ) most of them were of age of 20-30 years ( n=64, 82%) primigravida ( n = 45 , 60%),70% postnatal and experienced delays in care (n = 41 ,44.66%) and lost battle of life within 24 hrs. of admission. Conclusions: The burden of maternal mortality was found to be unacceptably high. Most women died of direct causes and experienced delays in care. Improvement in the quality of skilled maternity care, utmost need to avail good, proper and effective antenatal care, timely referral, prompt transportation, provision of family planning services, among other factors, can drastically curtail the maternal deaths. Abstract Background: Uterine Fibroid Embolization (UFE) is a new modality which has appeared in the horizon for non-surgical management of many conditions including fibroids. Moderate to severe anemia with fibroids is one of the reasons for delay in definitive treatment. Need for blood transfusion and its consequent risk of adverse reactions and diseases became a necessity to consider alternative treatment. Objective: To evaluate the efficacy and safety of UFE as an alternative to major surgery. Methods: Seventeen women with symptomatic uterine fibroids who declined surgery or were at poor risk for surgery were treated by Uterine Artery Embolization (UAE). The uterine arteries were catheterised and embolized with polyvinyl alcohol particles. The patients were followed up at regular interval clinically and with transabdominal ultrasonography. Results: UAE performed on all 17 patients had no major complications. Clinical follow-up showed a significant reduction in symptoms within six months of the procedure, with 79% improvement in menorrhagia and 86% improvement in pain. Follow-up imaging showed reduction in fibroid sizes ranging from 9-60%. The mean procedural time was 150 minutes. The mean hospital stay was 5 days (range, 3 to 10). Conclusions: Mid-term results of UAE for the treatment of symptomatic fibroids in our hospital indicate this to be a safe and effective therapeutic option. Abstract Background: In the recent past there had been a steady rise in caesarean section rate as against the vaginal delivery. Information is still scarce on the benefits of physiotherapy and birthing experience of women who exercises in antenatal duration. Benefits of exercise during pregnancy include; prevention of excessive gestational weight gain, gestational diabetes, pre-eclampsia, decrease need for caesarean section and reduced low back pain. The objective of this study was to evaluation of antenatal physiotherapy on maternal and perinatal outcome and subjective evaluation of degree of labour pain perception. Methods: In the study 150 primigravida subjects in second to third trimester without any associated high risk factor and were willing to exercise during pregnancy were chosen. Results: In the study group, the average duration of latent phase [7.56 (+/-2.02) hours] and active phase of first stage of labour [3.26 (+/-1.06) hours] was significantly lesser as compared to the control group, 89.3% (n=134) delivered vaginally and 10.7% (n=16) subjects had to be taken for caesarean section. In the study group no perineal tear or need for any instrumentation (outlet forceps/ventouse) to facilitate delivery was seen. Higher proportion of the study group subjects i.e. 70.9% experienced moderate to mild pain during labour. and majority i.e. 72% subjects delivered newborns with birth weight >2.5 kg,with higher one minute and five minutes APGAR scores. Conclusions: This highlights the very need for physiotherapy to be inculcated into the curriculum during antenatal period with conducive maternal and foetal outcome as it inflates the chances of normal deliveries and abate the rates of cesarean section and subsequent, which is the dire need of the day. Abstract Multiple myeloma (MM) is a proliferative disorder of plasma cells which produce abnormal immunoglobulin proteins. Skin involvement is rarely found in this disorder. They are either specific or nonspecific lesions. We report four such interesting patients who presented to us initially with common dermatoses such as leukocytoclastic vasculitis, pyoderma gangrenosum, and vesiculobullous disorders and were subsequently diagnosed to have MM. There were no skeletal involvements or renal function abnormality at the time of presentation. Unusual presentation, nonresponsiveness to conventional therapy, and abnormal blood parameters prompted us to suspect some underlying systemic conditions which were later confirmed to be MM after serum immunoelectrophoresis for M-band and bone marrow biopsy.

Abstract Inflammatory linear verrucous epidermal nevus (ILVEN) is a pruritic, erythematous scaly epidermal nevus which follows a Blaschko′s lines. Lichen striatus, linear Darier disease, linear porokeratosis, linear lichen planus, linear psoriasis, and the verrucous stage of incontinentia pigmenti may all have similar clinical presentations as the linear verrucous epidermal nevus. ILVEN can be distinguished from true nevoid psoriasis by pruritus and lack of response to antipsoriatic treatments. Various therapeutic modalities have been described, but no one therapy has been successful consistently. Though giant ILVEN is a relative contraindication to surgical excision, here we report a case showing effectiveness of full thickness excision and skin grafting for this condition.
