    Abstract The biomedical research infrastructure of our academic medical centers elucidates disease mechanisms and develops diagnostic tests and treatments with scientific rigor, but the mechanism for reliably moving these findings into clinical settings lacks careful design. Hence, there is a wide gap between medical discoveries and the health benefit realized by the U.S. population. Performance improvement (PI) is a discipline that focuses on intentional redesign of core processes in our health system to close these gaps. The application of PI science is more consequential to health outcomes as is traditional biomedical research. This article describes why application and recognition of PI science as a discipline is imperative and why we should require training with similar intensity in undergraduate medical education (UME) and graduate medical education (GME). It also proposes investment in infrastructure (developing faculty expertise) and rigorous fellowships, which are, perhaps, more likely to thrive in teaching hospitals with a strong clinical mission.

    Abstract Primary cutaneous B-cell lymphomas (CBCL) are a heterogeneous group of B-cell lymphomas without evidence of extracutaneous disease at the time of diagnosis. The 2022 World Health Organization classification of mature lymphoid neoplasms differentiates the indolent primary cutaneous marginal zone lymphoproliferative disorder, primary cutaneous follicle center lymphoma and Epstein-Barr virus-positive mucocutaneous ulcer, from the more aggressive primary cutaneous diffuse large B-cell lymphoma, leg-type and intravascular large B-cell lymphoma. The new updates in the 2022 classification are based on recent scientific advances in the understanding and characterization of these entities. This article aims to review the main clinical, cellular and molecular features of the five CBCL subsets along with their management and treatment. The exponentially growing evidence for new treatment options for systemic B-cell lymphomas raises expectations for the field of CBCL as well. However, specific prospective high quality research on CBCL is still crucial to further define their management and update international guidelines.

    Abstract Many parents have days where they encounter emotional exhaustion, emotional distance from their children, and feeling fed up with being a parent. Some parents experience these characteristics to a severe extent-a clinical phenomenon termed parental burnout. Parental burnout arises when parents chronically endure severe stress without sufficient resources to cope, which may lead to detrimental consequences not only for the parent but also for their partner (e.g., marital conflict) and children (i.e., neglect and violence). However, uncertainty remains regarding how these features interact and trigger one another over time (potentially becoming increasingly severe), nor how the daily variations of the family context influence these features. Therefore, in this study, we recruited 50 parents (with main analyses focusing on 43 mothers with a co-parent, and sensitivity analyses with the full sample) from the general population to rate the core features of parental burnout and the family context daily over 56 days. We used multilevel vector autoregressive models to generate network models. Results suggest that exhaustion contributes to parental burnout: It self-predicts and is closely associated with feeling fed up and finding children difficult to manage. Distance, by contrast, is mainly negatively connected to sharing positive moments with children. Contextual variables also interact with parental burnout features, illustrating the relevance of examining parenting within the family system context. If future research confirms a central role of exhaustion in parental burnout development, prevention efforts can focus on decreasing parental exhaustion.     Abstract Objectives: Migrant Australians with cancer have higher unmet needs and poorer health-related quality of life. Less is known about their palliative care experience. We aimed to assess comparative symptom distress and problem severity for culturally and linguistically diverse Australians with cancer in palliative care.
    Methods: This was a retrospective, consecutive cohort study using data from the Palliative Care Outcomes Collaboration, which routinely collects standardised symptom assessments nationally at point-of-care. Adults with a cancer diagnosis who died 01/01/2016-31/12/2019 were included. The presence/absence of
    Results: A total of 53 964 people with cancer died within the study period, allowing analysis of 104 064 assessments. People preferring non-English languages were less likely to report symptoms (pain: OR=0.89 (0.84 to 0.94); all other symptoms except fatigue OR<1 and CIs did not contain 1). Except for family/carer problems (OR=1.24 (1.12 to 1.31)), linguistically diverse people were less likely to report problems. Variation was seen between non-English language groups.
    Conclusions: We did not find evidence of comparatively worse symptom distress or problem severity for nearly all scores for culturally and linguistically diverse Australians. Better symptom management or differential reporting may explain this. It is important to examine this further, including assessing differences within cultural and linguistic groups to ensure the delivery of high-quality palliative care.

    Abstract In Benin, adaptation to climate change in the livestock sector has led cattle farmers to develop different livestock practices. Most research has focused on evaluating the effects of these practices on livestock productivity. However, information on the effect of these practices on carbon (C) sequestration in farmland soils is lacking. Soil C sequestration has been identified as a potential strategy to offset greenhouse gas emissions. Thus, the present study aimed at filling this gap. The calculation was one hand based on inventory data obtained from literature sources (excrement production of each cattle category, moisture content of each crop, ratio of crop residue to main product, and C content of the main product and excrement) and on the other hand on activity data (cattle herd size, manure applied, land use area, crop yield, and crop residues management) obtained from surveys carried out among 360 cattle farmers belonging to 3 cattle farming types. The results revealed that whatever the cattle farming type, annual C input from manure was higher (p˂0.05) than C input from crop residues.     Abstract Background: Persistent fibromyalgia-like symptoms have been increasingly reported following viral infections, including SARS-CoV-2. About 30% of patients with post-COVID-19 syndrome fulfill the fibromyalgia criteria. This complex condition presents significant challenges in terms of self-management. Digital health interventions offer a viable means to assist patients in managing their health conditions. However, the challenge of ensuring their widespread adoption and adherence persists. This study responds to this need by developing a patient-centered digital health management app, incorporating patient preferences to enhance usability and effectiveness, ultimately aiming to improve patient outcomes and quality of life.
    Objective: This research aims to develop a digital health self-management app specifically for patients experiencing postviral fibromyalgia-like symptoms. By prioritizing patient preferences and engagement through the app's design and functionality, the study intends to facilitate better self-management practices and improve adherence.
    Methods: Using an exploratory study design, the research used patient preference surveys and usability testing as primary tools to inform the development process of the digital health solution. We gathered and analyzed patients' expectations regarding design features, content, and usability to steer the iterative app development.
    Results: The study uncovered crucial insights from patient surveys and usability testing, which influenced the app's design and functionality. Key findings included a preference for a symptom list over an automated chatbot, a desire to report on a moderate range of symptoms and activities, and the importance of an intuitive onboarding process. While usability testing identified some challenges in the onboarding process, it also confirmed the importance of aligning the app with patient needs to enhance engagement and satisfaction.
    Conclusions: Incorporating patient feedback has been a significant factor in the development of the digital health app. Challenges encountered with user onboarding during usability testing have highlighted the importance of this process for user adoption. The study acknowledges the role of patient input in developing digital health technologies and suggests further research to improve onboarding procedures, aiming to enhance patient engagement and their ability to manage digital health resources effectively.

    Abstract Layer 5 (L5) pyramidal neurons receive predictive and sensory inputs in a compartmentalized manner at their apical and basal dendrites, respectively. To uncover how integration of sensory inputs is affected in autism spectrum disorders (ASD), we used two-photon glutamate uncaging to activate spines in the basal dendrites of L5 pyramidal neurons from a mouse model of Fragile X syndrome (FXS), the most common genetic cause of ASD. While subthreshold excitatory inputs integrate linearly in wild-type animals, surprisingly those with FXS summate sublinearly, contradicting what would be expected of sensory hypersensitivity classically associated with ASD. We next investigated the mechanism underlying this sublinearity by performing knockdown of the regulatory β4 subunit of BK channels, which rescued the synaptic integration, a result that was corroborated with numerical simulations. Taken together, these findings suggest that there is a differential impairment in the integration of feedforward sensory and feedback predictive inputs in L5 pyramidal neurons in FXS and potentially other forms of ASD, as a result of specifically localized subcellular channelopathies. These results challenge the traditional view that FXS and other ASD are characterized by sensory hypersensitivity, proposing instead a hyposensitivity of sensory inputs and hypersensitivity of predictive inputs onto cortical neurons.
