    Abstract Polymyalgia rheumatica (PMR) is the most common inflammatory rheumatological condition affecting individuals aged >50 years. There have been rare reports of PMR and other vasculitides developing within 3 months of influenza vaccination. Influenza is a major public health issue associated with seasonal increased mortality and intensified health care service use. Annual vaccination is the most effective intervention to prevent influenza, especially in elderly individuals. We report a severe "flare" of PMR in a 70-year-old patient after receiving the adjuvanted trivalent influenza vaccine, as recommended by the Joint Committee on Vaccination and Immunisations for this age group in the UK National Health Service in 2018-2019. The adverse event (AE) could be interpreted as the newly described autoimmune/inflammatory syndrome induced by adjuvants (ASIA syndrome) as both PMR and ASIA display hyperactive immune responses. Caution is warranted in the use of vaccine adjuvants in patients with PMR with pre-existing imbalance of B and T cell homeostasis. Rare AEs are important to individuals, and personalized medicine means we should move away from "one size fits all" for vaccines, as well as for therapeutics.

    Abstract Background: Taking a medical history is an effective way of finding out important information about the patient and their presenting problem/s but this approach may inadvertently be disempowering for the patient. An increased sense of agency can increase the likelihood of behavioural change, so facilitating empowerment of the patient in the context of a medical consultation is an important challenge for general practitioners.
    Objective: This article describes an alternative or additional model of history taking - the 'competency history'. It describes its theoretical base, components and appropriate use, and gives examples of how a competency history can be used in general practice.
    Discussion: A competency history uses strength based empowerment approaches and is especially appropriate with patients who have a poor sense of agency. The core elements of a competency history include an understanding of the patient's past and current context, respect for the patient's expertise and strength based interventions, including alternative narratives and solution focused conversations. Another important aspect of this approach is to allow the patient to take the initiative in making a change in their behaviour, and to take responsibility for their health. Taking a competency history is a useful additional skill for GPs, and may be a valuable addition to undergraduate medical training.

    Abstract Human milk oligosaccharides, proteins, such as lactoferrin, and bacteria represent just some of the bioactive components of mother's breast milk (BM). Bacteriophages (viruses that infect bacteria) are an often-overlooked component of BM that can cause major changes in microbial composition and metabolism. BM bacteriophage composition has been explored in term and healthy infants, suggesting vertical transmission of bacteriophages occurs between mothers and their infants. Several important differences between term and very preterm infants (<30 weeks gestational age) may limit this phenomenon in the latter. To better understand the link between BM bacteriophages and gut microbiomes of very preterm infants in health and disease, standardised protocols are required for isolation and characterisation from BM. In this study, we use isolated nucleic acid content, bacteriophage richness and Shannon diversity to validate several parameters applicable during bacteriophage isolation from precious BM samples. Parameters validated include sample volume required; centrifugal sedimentation of microbes; hydrolysis of milk samples with digestive enzymes; induction of temperate bacteriophages and concentration/purification of isolated bacteriophage particles in donor milk (DM). Our optimised method enables characterisation of bacteriophages from as little as 0.1 mL BM.     Abstract Background: Suicide is a major cause of death from adolescence upward. While there has been a concentrated effort to educate families and health workers to be aware of warning signs of pending suicide, there has been limited information or practical strategies available for people at risk of suicide or for those who have suicidal thoughts. Recent acknowledgment of the role of impulsivity in suicide highlights the need for such information.
    Objective: This article identifies those at risk of suicide and outlines strategies for suicide prevention.
    Discussion: Patients with suicidal thoughts are not an infrequent component of presentations in general practice and present a considerable challenge to the general practitioner because it is very difficult to predict who will make a suicide attempt. There has been much published to assist the GP to identify those at risk, but little to suggest how to assist the patient in such a crisis. Research into the role of impulsivity in suicide would suggest that unless the patient has strategies to confront the suicidal thoughts themselves, they remain at significant risk.
