    Abstract "I have been a midwife for 35 years. Throughout this time, I have witnessed the impact and implications of childbirth. However, more recent events have encouraged me to examine childbirth experiences which have involved close family members. Heartfelt comments have ranged from desperate pleas such as 'nobody tells you the reality of becoming a mother ... I feel overwhelmed' to 'I feel abandoned'. Such observations have urged me to explore the concept of what it means to women and midwives to be 'with woman'. In addition, new standards for the profession were published in November 2019, one of which requires midwives to be able to provide and promote continuity of care and carer. This is not a new phenomenon but seen through the concept of being 'with woman' would provide a safer and more effective delivery of maternity care. 'With woman' is an old term in the English language from which the word 'midwife' is derived. The meaning and concept of being 'with woman' may be interpreted as providing care and support in a physical, psychological, emotional and spiritual sense. Hunter (2002, p. 650), explored this concept to focus on the 'presence and support by a caregiver as desired by the labouring woman'. Early on, nurse-midwife core competencies in America emphasised the importance of human presence with the childbearing woman as a therapeutic and professional philosophy (ACNM 1979). More recently, Bradfield et al.'s (2018a) integrative review of the literature explored the 'with woman' phenomena to present an understanding and perspective of this concept. The authors suggest that being 'with woman' is an evolutionary construct, which is dynamic and continues to develop, and is fundamental to midwifery practices and professional philosophy"--

    Abstract Immunotherapy treatments for cancer are known to cause adverse thyroid events which present a diagnostic challenge to clinicians and radiologists. This case report highlights the importance of a high clinical index of suspicion and careful assessment of the thyroid on serial imaging studies to make the diagnosis. The case involves a 65-year-old male with malignant melanoma who was started on immunotherapy as part of a clinical trial. He developed thyroid dysfunction followed by an attack of acute neck pain. Ultrasound of his thyroid was performed which showed significant atrophy. A review of previous imaging was undertaken which confirmed the patient had suffered from thyroiditis and subsequent atrophy. Following this, the diagnosis of immunotherapy-induced thyroid dysfunction was made. Thyroxine supplementation and steroid dose were then adjusted causing his thyroid function and symptoms to improve. Immunotherapy agents for cancers are becoming more and more common. As the case report shows, physicians and radiologists will need to be vigilant to diagnose and treat any adverse events.

    Abstract Hereditary platelet disorders (HPDs) are a group of blood disorders with variable severity and clinical impact. Although phenotypically there is much overlap, known genetic causes are many, prompting the curation of multigene panels for clinical use, which are being deployed in increasingly large-scale populations to uncover missing heritability more efficiently. For some of these disorders, in particular RUNX1, ETV6, and ANKRD26, pathogenic germ line variants in these genes also come with a risk of developing hematological malignancy (HM). Although they may initially present as similarly mild-moderate thrombocytopenia, each of these 3 disorders have distinct penetrance of HM and a different range of somatic alterations associated with malignancy development. As our ability to diagnose HPDs has improved, we are now faced with the challenges of integrating these advances into routine clinical practice for patients and how to optimize management and surveillance of patients and carriers who have not developed malignancy. The volume of genetic information now being generated has created new challenges in how to accurately assess and report identified variants. The answers to all these questions involve international initiatives on rare diseases to better understand the biology of these disorders and design appropriate models and therapies for preclinical testing and clinical trials. Partnered with this are continued technological developments, including the rapid sharing of genetic variant information and automated integration with variant classification relevant data, such as high-throughput functional data. Collective progress in this area will drive timely diagnosis and, in time, leukemia preventive therapeutic interventions.

    Abstract Introduction: We conducted a statewide assessment of the availability of stroke treatment, services, and programs in North Carolina (NC) hospitals. We also examined differences in stroke care capabilities between urban, suburban, and rural hospitals and trends over the past 2 decades.
    Methods: An electronic survey was distributed to all 111 licensed hospitals in NC. Survey questions asked about stroke center certification status (i.e., standardized levels of stroke care capabilities), diagnostic testing, acute treatments and protocols, and post-acute management. Responses were collected from October 2020-April 2021. Select characteristics were compared to those from prior NC surveys in 1998, 2003, and 2008.
    Results: All 111 hospitals responded to the survey (100% response rate). Among 108 hospitals providing acute stroke care, 12 (11%) were Comprehensive Stroke Centers or Thrombectomy-Capable Stroke Centers, which were all located in urban or suburban areas. While 38% of urban/suburban hospitals were non-certified, 48% of rural hospitals were non-certified. Non-contrast computed tomography (CT), CT angiography, and alteplase treatment were widely available (100%, 95%, and 99%, respectively). Endovascular thrombectomy was solely available in urban/suburban hospitals (29%). Of non-tertiary hospitals, 81% were using telestroke for treatment and transfer decisions. Compared to prior survey results, the availability of CT angiography (76% in 2008 to 95% in 2020-2021), alteplase treatment (69% in 2008 to 99% in 2020-2021), and acute stroke clinical pathways (47% in 2008 to 90% in 2020-2021) increased. However, having an in-house neurologist on staff dropped from approximately 55% in prior surveys to 21% in the current survey.
    Conclusions: Rural NC hospitals were less likely to have advanced diagnostic imaging and treatment capabilities for acute stroke. Temporal trends in staffing with an in-house neurologist and use of telestroke services should be further examined.

    Abstract Germline RUNX1 variants have been identified in relation to myeloid malignancy predisposition, with lymphoid hematological malignancies present at a lower frequency in families. In this issue of the JCI, Li and Yang et al. examined the frequency and type of germline RUNX1 variants in pediatric patients with acute lymphoblastic leukemia (ALL). Patients with T cell ALL (T-ALL) harbored rare, damaging RUNX1 mutations that were not seen in patients with B cell ALL (B-ALL). Further, several of the T-ALL-associated RUNX1 variants had potential dominant-negative activity. RUNX1-mutated T-ALL cases were also associated with somatic JAK3 mutations and enriched for the early T cell precursor (ETP) leukemia subtype, a finding that was validated when RUNX1 and JAK3 mutations were combined in mice. This study confirms germline RUNX1 predisposition beyond myeloid malignancy, demonstrates the importance of examining both germline and somatic mutations in malignancy cohorts, and demarcates the ETP ALL subtype as a flag for germline predisposition in patients.

    Abstract To prevent response bias, personality questionnaires may use comparative response formats. These include forced choice, where respondents choose among a number of items, and quantitative comparisons, where respondents indicate the extent to which items are preferred to each other. The present article extends Thurstonian modeling of binary choice data to "proportion-of-total" (compositional) formats. Following the seminal work of Aitchison, compositional item data are transformed into log ratios, conceptualized as differences of latent item utilities. The mean and covariance structure of the log ratios is modeled using confirmatory factor analysis (CFA), where the item utilities are first-order factors, and personal attributes measured by a questionnaire are second-order factors. A simulation study with two sample sizes, N = 300 and N = 1,000, shows that the method provides very good recovery of true parameters and near-nominal rejection rates. The approach is illustrated with empirical data from N = 317 students, comparing model parameters obtained with compositional and Likert-scale versions of a Big Five measure. The results show that the proposed model successfully captures the latent structures and person scores on the measured traits.
