    Abstract Background: There is little debate regarding the workhorse flaps for soft-tissue defects in head and neck reconstruction. However, in certain circumstances, the workhorse flaps are not available or suitable, and an alternate flap is needed.
    Methods: The author performed a retrospective review of a single-surgeon experience with alternate soft-tissue flaps performed over a 10-year period.
    Results: Overall, 156 alternate soft-tissue free flaps were performed in 155 patients. The distribution of free flaps was 60 ulnar artery perforator (UAP), 28 lateral arm perforator (LAP), 33 medial sural artery perforator (MSAP), and 35 profunda artery perforator (PAP) flaps, with one patient receiving two PAP flaps. Defects included glossectomy, facial defects following parotidectomy or resection of cutaneous malignancies, and other intraoral defects. Eleven patients had donor-site complications: two patients with UAP flap with partial skin graft loss, two patients with delayed wound healing in the MSAP donor-site group and one who developed compartment syndrome with primary closure of the MSAP donor site, one patient with a radial nerve palsy following an LAP flap, and six patients with delayed wound healing in the PAP donor site, which healed secondarily with conservative management. There was one total flap loss each of an LAP and MSAP flap.
    Conclusion: Reconstructive microsurgeons performing high-volume head and neck reconstruction may need to incorporate alternate donor sites into their armamentarium when the primary workhorse flaps are not suitable or available.

    Abstract Patients undergoing treatment for breast cancer who undergo an axillary dissection and require adjuvant therapies such as radiation and chemotherapy are at high risk of developing lymphedema of the associated extremity. Historically, patients with lymphedema were treated with ablative procedures aimed simply to remove excess fluid and adiposity; however, the field of lymphatic surgery employing super-microsurgery techniques has witnessed tremendous advances in a relatively short period of time. Advancements in surgical instruments, microscope magnification and optics, imaging technology, and surgeon experience have ushered in a new era of hope to treat patients suffering from breast cancer-related lymphedema (BCRL). Here we aim to present the available options for patients suffering from BCRL, and the pinnacle in reconstruction and restoration for these patients.

    Abstract Siponimod is a sphingosine 1-phosphate receptor (S1P) modulator used to treat secondary progressive multiple sclerosis (SPMS). We report 3 SPMS patients treated with siponimod who developed new or worsening peripheral oedema soon after commencing treatment. In one case, peripheral oedema resulted in immobility. Siponimod-related peripheral oedema deserves wider recognition due to the potential for morbidity and over-investigation. Clinicians should assess for pre-existing oedema and coexisting conditions that may predispose to developing peripheral oedema prior to commencing siponimod.

    Abstract This paper presents a systematic approach to using the Socratic method in developing prompt templates that effectively interact with large language models, including GPT-3. Various methods are examined, and those that yield precise answers and justifications while fostering creativity and imagination to enhance creative writing are identified. Techniques such as {\em definition}, {\em elenchus}, {\em dialectic}, {\em maieutics}, {\em generalization}, and {\em counterfactual reasoning} are discussed for their application in engineering prompt templates and their connections to inductive, deductive, and abductive reasoning. Through examples, the effectiveness of these dialogue and reasoning methods is demonstrated. An interesting observation is made that when the task's goal and user intent are conveyed to GPT-3 via ChatGPT before the start of a dialogue, the large language model seems to connect to the external context expressed in the intent and perform more effectively.

    Abstract The CoCoMo model proposes a computational solution to the challenge of incorporating ethical and emotional intelligence considerations into AI systems, with the aim of creating AI agents that combine knowledge with compassion. To achieve this goal, CoCoMo prioritizes fairness, beneficence, non-maleficence, empathy, adaptability, transparency, and critical and exploratory thinking abilities. The model employs consciousness modeling, reinforcement learning, and prompt template formulation to support these desired traits. By incorporating ethical and emotional intelligence considerations, a generative AI model can potentially lead to improved fairness, reduced toxicity, and increased reliability.
