    Abstract Since its inception in 2009, femtosecond laser-assisted cataract surgery (FLACS) has become an alternative to conventional phacoemulsification cataract surgery (CPCS). Clinical studies were unable to demonstrate superior visual outcomes, but revealed reduced endothelial cell loss. More recently, the cost-effectiveness of FLACS over CPCS in routine cataract surgeries has been challenged. However, the unique abilities of FLACS to customize anterior capsulotomies precisely, soften and fragment the nucleus without capsular bag stress, and create corneal incisions may have special utility in complex cataract and less-common scenarios. In this article, we review the unique role of FLACS in complex cataract surgeries and how it could play a role to improve the safety and predictability of nonroutine cataract surgery.

    Abstract The advent of open data in health care has increased healthcare innovation, with the publication of complete datasets aggregated by private and public entities that lead to efficiency through crowdsourcing working code, facilitating research into personalized medicine, and publishing reproducible data pipelines for experimental validation. However, there lacks an internationally recognized definition for health data governance at the scope of individual health data and open source big data, which bring about a discussion about the implications of open data on data privacy. First, healthcare data sourced directly from public healthcare systems: by whom and for what purpose is these data used for within the context of healthcare research. Second, health data from private research: the regulations needed for mutual disclosure. Third, personal user-generated health data: safeguards in a digital era needed to prevent misappropriation and abuse. This paper addresses the opportunities of open data in healthcare research in a digital age without transparent regulation. The consequence of open data on privacy leads to a framework of four safeguards for stakeholders: public education, operational transparency, regulation for accountability, and validation of research ethics. It also pioneers public policy direction for a balanced agenda between privacy and healthcare research.

    Abstract The majority of inguinal hernia repairs worldwide are performed on an outpatient basis. However, incarceration and concern for strangulation of abdominal contents necessitates emergent repair in order to address visceral ischemia. In the setting of salvageable ischemia, this necessitates release of strangulation of blood supply by the hernia defect and reduction of visceral contents into the abdominal cavity. In certain cases, this cannot be achieved with simple manual reduction, and requires enlargement of the aperture of the hernia defect with releasing incisions in order to allow reduction. We aim to describe strategies for releasing incisions via open, laparoscopic, and robotic approaches in emergency inguinal hernia repair.

A collection of the most celebrated molecules in total synthesis are revisited and the author’s solutions to these historical challenges are presented.
