    Abstract Introduction: This study evaluated the effect of the noise level of the nonstress test (NST) device on stress parameters in primigravid women.
    Methods: A total of 44 pregnant women participated in a randomized clinical trial between February and October, 2021. The participants were divided randomly into 4 equal groups using an automated web-based randomization system and ensuring allocation concealment: the control group (NST device volume turned off), intervention group I (1-35 dB(A)), intervention group II (36-60 dB(A)), and intervention group III (61 dB(A) and above).     Results: NST sound levels above 35 dB(A) increased the average cortisol and ACTH levels in the posttest. NST sound levels were positively correlated with posttest cortisol (r = .448) and posttest glucose (r = .302). There was interaction effect on heart rate, glucose, and cortisol level by time, indicating that the intervention groups experienced a significant acceleration in heart rate, glucose, and cortisol level after a noise intervention compared with the control group.
    Discussion: Midwives should continue applying NST below 36 dB(A), considering the factors that may induce stress during the NST process.

    Abstract Thoracic endometriosis syndrome (TES) is the presence of endometriotic foci within the respiratory system and involves a range of symptoms that coincide with the menses. These symptoms are defined as the presence of pneumothorax, hemothorax, hemoptysis, lung nodules, isolated chest pain, and pneumomediastinum. Catamenial pneumothorax (CP) is described as recurrent abnormal air collection between the lung and chest wall that occurs within the first 72 hours of the menses and is the most common presentation of TES [1].

    Abstract AAbdominal pregnancies are rare and life threatening conditions with increased maternal mortality rate. A 37-year old woman admitted with acute abdominal pain. She had a history of dilatation and curettage due to anembryonic pregnancy five days ago. A pelvic X Ray confirmed an abdominally located IUCD. Her serum β-human chorionic gonadotropin level was 5531 mIU/ml. A laparoscopy was performed and there was a round hemorrhagic mass in the vesicouterine pouch in exploration. The trophoblastic tissue was removed from the vesicouterine pouch and left sacrouterine ligament. The histopathology report revealed chorionic villi both of the excised specimens. The abdominal cavity should be explored carefully in suspicion of abdominal pregnancy and laparoscopic management is a feasible approach for this cases.

    Abstract Amaç: Bu çalışmanın amacı, tek veya bilateral endometriomalarda laparoskopik kistektomi sonrası elektrokoagülasyon veya sütür tekniği uygulaması ile sağlanan hemostazın, endometrioma rekürrensi ve spontan gebelik oranlarına etkisinin değerlendirilmesidir.Gereç ve Yöntem: 18-42 yaş aralığında herhangi bir yardımcı üreme tedavisi desteği almayan 46 primer infertil hasta retrospektif olarak analiz edildi. Laparoskopik kistektomi operasyonu, sıyırma tekniği ile uygulandı. Over doku kanaması sütür (n=23) veya bipolar elektrokoagülasyon (n=22) ile kontrol edildi. Kist rekürrensi postoperatif olarak ultrason eşliğinde değerlendirildi. Spontan gebelik adet gecikmesi sonrası kan Beta- insan koryonik gonadotropin (B-hCG) seviyesi bakılarak saptandı.Bulgular: Ortalama yaş 31.62±6.55 olarak saptandı. Gruplar arasında istatistiksel olarak yaş, vücut kitle indeksi (BMI), preopertaif endometrioma büyüklüğü, pre/postoperatif hemoglobin (hb) ve hemotokrit (Hct) değeri ile anlamlı bir fark saptanmadı. Sütür grubunda rekürrens oranı fazla çıksa da istatistiksel olarak anlamlı bulunmadı. Spontan gebelik oranı her iki grupta benzer saptandı. Çalışma gruplarında endometrioma lateralitesinde istatistiksel olarak bir fark saptanmadı.Sonuç: Bu çalışma, Endometrioma cerrahisi sonrası uygulanan elektrokoagülasyon veya sütür tekniğinin endometrioma rekürrensi ve spontan gebelik oranlarına farklı etkisinin olmadığını göstermiştir.

    Abstract Objective: Endometriosis is a common disease that affects 5%-15% of women of reproductive age. This study aimed to evaluate the effect of dienogest (DNG) on improving pain scores of patients with endometriosis.Methods:Data of 77 women who were admitted to the endometriosis clinic from March 1, 2015, to March 1, 2017, were evaluated. Patients were divided into 2 groups: DNG group (n=46) or group that denied use of any medication (expectant management group, n=31). The main presenting symptoms were graded using the visual analog scale (VAS). Age, parity, gravidity, body mass index (BMI), VAS scores, and further surgery requirement of the study groups were compared.Results:The most common presenting symptom was dysmenorrhea (79.2%), and the least common was dysuria (7.8%). No statistically significant difference was found between the study groups in terms of age, parity, gravidity, BMI, and further surgery requirement. A significant difference was found in the reduction of dysmenorrhea, dyspareunia, dyschezia, and pelvic pain VAS scores between the study groups, and more reduction was observed in the DNG group after 6 months of follow-up (p<0.001, p=0.04, p=0.009, and p=0.01, respectively).Conclusion:DNG might be an alternative treatment for reducing pain symptoms related to endometriosis.

    Abstract Objective:To declare our initial experience on hysterectomy cases performed using vaginally-assisted natural orifice transluminal endoscopic surgery.Material and Methods:The study was conducted with data from 12 patients in our department who were recommended for hysterectomies for various indications between January 2017 and May 2017. The following data were collected retrospectively: age, body mass index (BMI), parity, previous abdominal or pelvic surgery, total operating time, preoperative hemoglobin (Hb), postoperative Hb, peri-operative complications and Visual Analogue Scale scores for evaluating postoperative pain. All patients were laid in the dorsal lithotomy position under general anesthesia. A cervical circumcision, as well as anterior and posterior colpotomy were performed. A self-constructed glove port was then inserted through the anterior and posterior colpotomy openings into the abdominal cavity. After pneumoperitoneum was achieved, a 10-mm rigid zero-degree telescope, disposable conventional laparoscopic grasping forceps, and a tissue sealer were used as standard equipment. After exploration of the abdominal cavity, all uterine vessels and ligaments were sealed and transected using the tissue sealer. After removing the uterus, the vaginal opening was closed using a Vicryl 1-0 suture.Results:The following are the mean values for each variable: patients’ age: 55.75±9.8 years (range, 43-72 years), BMI: 29.4±5.4 kg/m2 (range, 21-42 kg/m2), operation duration: 66.8±25.3 min (range, 42-120 min), decrease in Hb: 1.5±1 (0-4) gr/dL, and hospital stay: 2.1±0.3 (2-3) days. There were no vaginal wound infections or dehiscence, and no patients reported pain during postoperative pelvic examinations. Conclusion:Although these findings are from our initial experience, we can affirm the feasibility of this technique.

    Abstract Heterotopic pregnancy occurs 1 in 30000 pregnancies. We present case of an acute abdomen caused by a ruptured ectopic component. Our patient had no known risk factors, which made the diagnosis even more challenging. Intrauterine pregnancy was desired by patient and her husband. A natural orifice transluminal endoscopic surgery (NOTES) procedure was performed, which is a next-generation minimally invasive procedure in this area. After the procedure, our patient was discharged one day after surgery with a viable intrauterine pregnancy.

    Abstract Ectopic pregnancy and the rupture of corpus hemorrhagicum are fatal gynecological emergencies that comprise an important part of the causes of referring to the emergency services. 26 year old-gravida 1, para 1 woman was referred to our clinic with lower quadrant abdominal pain. In her medical history there were no delayed menstruation and the pregnancy test which practiced in an outside clinic, was reported as negative. According to the acute abdomen signs in the medical examination, emergency laparatomy has been performed. There was a mass that pretends to be ruptured hemorrhagic cyst prediagnostically was observed and excised. Pathology results of the patient, whose bhCG value was measured post operatively as 2090mIU/mL, was compatible with ovarian ectopic pregnancy. The case of ovarian ectopic pregnancy, which is a rare situation and can be interfering with the rupture of corpus hemorrhagicum cyst, is presented with a brief literature review.

    Abstract Round ligament fibroids are rare tumors and can present as inguinal, adnexal, or vulvar masses. Preoperative diagnosis can be made by ultrasonography or magnetic resonance imaging (MRI). A 28-year-old virgin female presented with pelvic pain that persisted for a few days. Her sonographic evaluation detected a well-defined hyperechoic 45 × 40 mm right adnexal mass interfering endometrioma. A contrast-enhanced pelvic MRI showed a 43 × 39 × 32 mm solid mass located in the right adnexa. Laparoscopic exploration revealed a well-defined double torsion around the peduncle pinky solid mass arising from the right round ligament. The mass was resected with the final histopathological diagnosis being leiomyoma. The diagnosis of round ligament fibroids can be challenging, and laparoscopy is one of the first options where surgical facilities are available.
