Abstract Introdutcion: Idiopathic palsy of facial nerve is an acute disease which causes lower motor neuron palsy characterised by facial asymmetry leads to psychological effects and limits one’s social life. Steroids are very effective in the management of Bell’s palsy. The usage of antiviral is still in debate due to the idiopathic aetiology of Bell’s palsy. Aim: To evaluate the efficacy of acyclovir with steroid and steroid alone in the management of Bell’s palsy with respect to the recovery time. Materials and Methods: The randomised double-blinded study was conducted in the Department of Otorhinolaryngology, Chettinad Hospital and Research Institute, Chennai, Tamil Nadu, India, from September 2021 to February 2022 among 100 patients with idiopathic facial nerve palsy. Patients were divided into two groups, 50 patients in each. One group treated with acyclovir (400 mg oral five times daily) and methylprednisolone (1 mg/kg/day), and other group treated with steroid alone. Both the groups received physiotherapy in the Department of Physiotherapy and taught about home facial exercises. The patients were on regular follow-up for 6 months to monitor the time of the recovery. House-Brackmann grading system was used for initial assessment and monitor the recovery of the patient. The improvement of House-Brackmann grade to I or II was considered a satisfactory recovery. The association between two groups were assessed by Chi-square test (χ2), repeated measures and paired t-test. The statistical significance was considered when p-value<0.05. Results: Out of 100 patients, 52 patients were male and 48 were female, while the age range was 19-58 years. At 2 months, 43 (86%) patients recovered in the combination group and 41 (82%) in the steroid alone group (p-value=0.0038). At 6 months, 48 (96%) patients recovered in the combination group and 42 (84%) in the steroid alone group (p-value=0.178). The overall recovery rate was higher in the combination group (96%) compared to steroid alone group (84%). Conclusion: The combination ...

Abstract Foreign bodies in throat is a very common entity encountered in otolaryngology practice. Out of these most common foreign bodies in adults include fish bone, mutton bone, piece of meat. Present case was the first in literature to present with duck bone stuck in throat. A 30-year-old female presented to the Ear, Nose and Throat (ENT) Outpatient Department (OPD) with history of duck meat ingestion and complaints of foreign body sensation in throat and odynophagia for a period of three days. Video laryngoscopy revealed the presence of a foreign body at the level of laryngeal inlet, just above the right vocal cord, adherent to the anterior commissure. After obtaining anaesthesia fitness on the same day, direct laryngoscopy under general anaesthesia was performed. The foreign body was visualised, found to be more adherent near the anterior commissure and was removed using microlaryngeal forceps. The patient significantly improved and was discharged. All patients who present to the Otorhinolaryngology OPD with complaints of swallowing a foreign body/bone or experiencing a foreign body sensation in the throat should be taken seriously. A lateral radiograph of the neck soft tissue may sometimes not reveal the foreign body. A video laryngoscopy examination should be done to confirm the presence and assess the site of impaction of foreign body.

Abstract Many 3-aminochroman-2-ones and β,β-diarylalanines exhibit significant biological activities. A new method was thus developed for the syntheses of these compounds with high efficiency and diastereoselectivity. First, treatment of various phenols with Erlenmeyer–Plochl (Z)-azlactones and AlCl₃ in toluene produced the desired cis-3-aminochroman-2-ones in 65–90% yields under kinetic control. This coupling reaction involved a domino process of Friedel–Crafts alkylation, 1,4-AlCl₃ shift, transesterification, and protodealumination in a “single-flask.” The corresponding products, however, were not generated by replacement of AlCl₃ with a protonic acid. Second, hydrolysis of the resultant 3-amino-4-arylchroman-2-ones by NaHCO₃ in a mixture of THF and water gave α-(N-benzoyl)amino acids. Further deprotection of these isolated compounds by use of hydrochloric acid (12 N) in methanol afforded the desired free amino acids in 80–88% yields. Under these optimized conditions, epimerization did not occur at the α carbons of α-(N-benzoyl)- and free α-amino acids. These new findings provide a convenient way to generate 3,4-disubtituted chroman-2-ones and β,β-diarylalanine derivatives with very high stereoselectivity.

Abstract Dengue fever is an infectious tropical disease caused by the dengue virus. Dengue is transmitted by several species of mosquito within the genus Aedes, principally A. aegypti. The Study area of Madurai is located at 90 30’ and 100 30’ longitudes and between 770 00’and 780 30’ latitude. The purpose of selection of the study area is to investigate the spatial distribution of Dengue at the micro level in an urban area of Madurai city and to analyze the causal relationships between the distribution of Dengue and its association with socio – economic, environmental, behavioral and medical variables. The data collected were taken by using the method of stratified Random sampling and a total of 300 samples were collected from the study area with reference to different age groups and sex ratio. It includes mapping of the disease using the Arc GIS. The data were analyzed with the help of Multivariate statistical technique known as ‘Factor Analysis’ The study has identified that environment parameters played a major role in the health status of Dengue.

Abstract Element geochemistry and Sr–Nd isotopes have been used to characterise the palaeoflood sediments at a tributary junction in lower Narmada Valley archiving the Little Ice Age (LIA) and to determine the present day sources for the mainstem and the tributary sediments. The major and trace element concentrations show minor variations in values post 360CalyrBP. The 87Sr/86Sr and εNd values of palaeoflood sediments range from 0.7122 to 0.7191 and −2.89 to +6.28 respectively. The bed sediments from the mainstem river (Narmada) and its tributary (Karjan) yield 87Sr/86Sr values of 0.72394 and 0.7113 and εNd values of −7.18 and +6.09 respectively. Potential sources of palaeoflood sediments have been investigated from Sr and Nd isotope compositions and mixing of the mainstem as well as the tributary sediments is suggested. Pronounced contribution from the Deccan Basalt during the early part of the Little Ice Age is inferred. The more radiogenic Nd values indicate a contribution from the weathering of younger Deccan basalts, especially the Poladpur formations. The control of weathering intensity and mineral sorting is observed in ‘higher than the source rock’ 87Sr/86Sr ratios of the sediments. The major and trace element variations during the last 700years are not conclusive although, isotopic signatures of the flood sediments help in constraining the flux from the tributaries. Sr and Nd isotopic composition of the palaeoflood sediments can be used to track the monsoon intensity and pattern but their concentrations in a fluvial system remain primarily controlled by mineral sorting processes and mixing of sources.

Abstract The aluminium alloy AA2219 (Al–Cu–Mg alloy) is widely used in the fabrication of lightweight structures with high strength-to-weight ratio and good corrosion resistance. Welding is main fabrication method of AA2219 alloy for manufacturing various engineering components. Friction stir welding (FSW) is a recently developed solid state welding process to overcome the problems encountered in fusion welding. This process uses a non-consumable tool to generate frictional heat on the abutting surfaces. The welding parameters, such as tool pin profile, rotational speed, welding speed and axial force, play major role in determining the microstructure and corrosion resistance of welded joint. The main objective of this work is to develop a mathematical model to predict the corrosion resistance of friction stir welded AA2219 aluminium alloy by incorporating FSW process parameters. In this work a central composite design with four factors and five levels has been used to minimize the experimental conditions. Dynamic polarization testing was carried out to determine critical pitting potential in millivolt, which is a criteria for measuring corrosion resistance and the data was used in model. Further the response surface method (RSM) was used to develop the model. The developed mathematical model was optimized using the simulated annealing algorithm optimizing technique to maximize the corrosion resistance of the friction stir welded AA2219 aluminium alloy joints.
