    Abstract As a promising alternative to bone marrow aspiration (BMA), mutational profiling on blood-derived circulating cell-free tumor DNA (cfDNA) is a harmless and simple technique to monitor molecular response and treatment resistance of patients with refractory/relapsed multiple myeloma (R/R MM). We evaluated the sensitivity and specificity of cfDNA compared to BMA CD138 positive myeloma plasma cells (PCs) in a series of 45 R/R MM patients using the 29-gene targeted panel (AmpliSeq) NGS.

    Abstract Purpose: Follicular lymphoma (FL) is one of the most common lymphoma. Occasionally, FL is associated with tumoral epidural compression and management of these patients remain poorly codified. This study aims to report incidence, clinical characteristics, management and outcomes of patients with FL and tumoral epidural compression.
    Material and methods: Observational, retrospective cohort study of adult patients with FL and epidural tumor compression, treated in a French Institute over the last 20 years (2000-2021).
    Results: Between 2000 and 2021, 1382 patients with FL were followed by the haematological department. Of them, 22 (1.6%) patients (16 men and 6 women) had follicular lymphoma with epidural tumor compression. At epidural tumor compression occurrence, 8/22 (36%) patients had a neurological clinical deficit (motor, sensory or sphincter function) and 14/22 (64%) had tumor pain. All patients were treated with immuno-chemotherapy; the main regimen being used was R-CHOP plus high dose IV methotrexate in 16/22 (73%) patients. Radiotherapy for tumor epidural compression was performed in 19/22 (86%) patients. With a median follow-up of 60 months (range=[1-216]), 5 year local tumor relapse free survival was achieved in 65% (95% CI 47-90%) of patients. The median PFS was of 36 months (95% CI 24-NA) and 5 years OS estimate was 79% (95% CI 62-100%). Two patients developed a relapse at a second epidural site.
    Conclusion: FL with tumoral epidural compression reached 1.6% of all FL patients. Management based on immuno-chemotherapy with radiotherapy appeared to produce comparable outcomes with the general FL population.

    Abstract RESUMEN El trabajo analiza cómo se construye socialmente un espacio público usado por jóvenes, el botellódromo de Granada. Para el análisis se usa: bibliografía, noticias de prensa y entrevistas a jóvenes que frecuentan este espacio. Los resultados señalan, por un lado, que los medios de comunicación (españoles) inciden en los aspectos más negativos y a veces “criminalizante” de la actividad; y, por otro, que para los jóvenes significa un ritual de paso en el inicio de una etapa de vida adulta y universitaria a través del ocio. Palabras clave: espacio público; jóvenes; botellódromo. ABSTRACT This article analyzes the social construction of a public space, located in the city of Granada and frequented mainly by young people: the “botellódromo”. The main sources and tools of the research were the bibliographic and hemerographic survey and interviews with the regulars. The results indicate that the Spanish media highlight the most negative and sometimes “criminalizing” aspects of the activity. On the other hand, for the young people, attendance at this place means a ritual of passage to adulthood and university life, through leisure. Keywords: public space; youth; “bottellódromo”.

    Abstract The paper presents the activity and the objectives of “The Big Four” Group of Financial Advisory Firms. The “Big Four” are the four largest international professional services networks in accountancy and professional services, offering audit, assurance, tax, consulting, advisory, actuarial, corporate finance and legal services. They handle the vast majority of audits for publicly traded companies as well as many private companies, creating an oligopoly in auditing large companies. It is reported that the Big Four audit all but one of the companies that constitute the FTSE 100, and 240 of the companies in the FTSE 250, an index of the leading mid-cap listing companies.

    Abstract Background: Despite that cytogenetic and molecular analysis of tumor cells can rapidly identify recurring molecular abnormalities, no personalized therapy is currently available in the setting of relapsed/refractory multiple myeloma (r/r MM).
    Methods: MM-EP1 is a retrospective study aimed at comparing a personalized molecular-oriented (MO) versus a non-molecular-oriented (no-MO) approach in r/r MM. Actionable molecular targets and their associated therapies were the BRAF V600E mutation and BRAF inhibitors; t(11;14)(q13;q32) and BCL2 inhibitors; and t(4;14)(p16;q32) with FGFR3 fusion/rearrangements and FGFR3 inhibitors.
    Results: One hundred three highly pretreated r/r MM patients with a median age of 67 years (range 44-85) were included. Seventeen (17%) patients were treated using an MO approach with BRAF inhibitors (vemurafenib or dabrafenib,
    Conclusion: Despite the low number of patients treated with an MO approach, this study highlights the strengths and weakness of a molecular-targeted approach for the treatment of multiple myeloma. Widespread biomolecular techniques and improvement of precision medicine treatment algorithms could improve selection for precision medicine in myeloma.

    Abstract La intención del presente artículo es mostrar procesos sociales locales en coexistencia con las realidades posmodernas globales, donde parece persistir una lógica egocéntrica e hiperespecializada irremediable. Esta idea parte de observar la pervivencia y la renovación de solidaridades tradicionales en espacios que se asumen globalizados, pero en los que persisten o se recuperan tejidos sociales históricos donde la memoria colectiva actúa como resistencia cultural. En este contexto, se analizan, a través de la observación participante, dos tianguis localizados en el área purhepecha y protagonizados por la comunidad purhe: el tianguis del Santuario, en la ciudad de Páztcuaro, y el mojtakuntani o tianguis regional purhepecha, desarrollado por las poblaciones cercanas al lago de Pátzcuaro (Estado de Michoacán, México). Los tianguis son mercados públicos que se asumen aquí como espacios de intercambio y tienen la particularidad de que la actividad económica de adquisición de bienes se realiza a través del trueque, sin intervención de la moneda como valor de cambio.

    Abstract Introduction: Peripheral T-cell lymphomas (PTCL) represent a heterogeneous group of malignancies frequently associated with a poor outcome. The frontline treatment strategy for PTCL relies mostly on CHOP or CHOP-like regimens, which are associated with a high failure rate and frequent relapses.
    Areas covered: In this review, the authors present recently registered drugs and their positioning in the therapeutic armamentarium against PTCL and new drugs currently in development. The successful results in CD30-positive anaplastic large cell lymphomas suggest that a better characterization of these lymphomas could open new areas of efficient drug development.
    Expert opinion: Advances in the field of molecular biology have started to unravel the anomalies associated with T-cell malignancies. Recent knowledge on potential epigenetic modifiers like IDH2, which is frequently mutated in angioimmunoblastic T-cell lymphoma, opens new areas of research and confirms that epigenetic drugs could represent an attractive area of clinical research. The recently developed immune checkpoints regulators might represent another area of potential interest.
