    Abstract This two-volume work discusses environmental health, the branch of public health concerned with all aspects of the natural and built environment affecting human health, and addresses key issues at the global and local scales. The work offers an overview of the methodologies and paradigms that define this burgeoning field, ranging from ecology to epidemiology, and from pollution to environmental psychology, and addresses a wide variety of global concerns including air quality, water and sanitation, food security, chemical/physical hazards, occupational health, disease control, and injuries. The authors intend to provide up-to-date information for environmental health professionals, and to provide a reference for students and consultants working at the interface between health and environmental sectors. Volume 2 covers the technological, legislative, and logistical solutions for coping with environmental health issues. The principles of environmental legislation are explained in national and international contexts, and assessments are mapped out to craft informed governance plans for health and environmental management. Mitigation measures are introduced to control wastewater and solid waste management and air and noise pollution, and adaptation strategies for emergency preparedness and disaster recovery are discussed.

    Abstract Erratic rainfall, limited surface water, and intensive agriculture have increased dependency on groundwater in semiarid to sub-humid aquifer systems of the north Indian state, Punjab. Rapid declining water levels and water quality deterioration are common issues in Indian states. The present study is a comprehensive study carried out to examine the quality and extent of contamination due to trace elements and nitrate in aquifer systems and the distribution of toxic elements in agricultural and barren soils. Potential toxic elements are analyzed in groundwater samples covering both the shallow (<60 m) and deeper aquifers (>60 m) from rural and urban areas and agricultural and barren soil up to a depth of 35 ft. Elements such as As, B, Cd, Li, Mn, Pb, Sr, U, and Se exceed the prescribed limits. The soil pollution level was determined using various pollution indices like the geo-accumulation index, enrichment factor, pollution index and pollution load index, and potential ecological risk assessment. Shallow aquifers mostly are exceeding nitrate permissible limits (45ppm) more than deep aquifers. Contaminants concentration increases toward groundwater flow direction, from northeast to southwest possibly due to arid climatic conditions. The soil analysis results revealed that pollution in the region is due to anthropogenic activities mostly with few elements of geogenic origin. This study demonstrated an urgent need for agriculture management cropping patterns, use of more surface water, the use of purified water for domestic uses, and some quality control measures that are warranted to protect the health of the exposed community in Punjab.

    Abstract Context: Learn the growth hormone dynamics and discuss the issues of growth hormone therapy in subjects with 45,X/46XY.
    Objective: To study the growth hormone dynamics in children with 45,X/46,XY karyotyping and mixed gonadal dysgenesis (MGD).
    Design: Descriptive clinical study.
    Participants: Five subjects with karyotype 45,X / 46,XY with or without genital ambiguity and somatic features of SHOX haploinsufficiency.
    Interventions: Growth hormone dynamic study and gonadectomy.
    Main outcome: IGF-1, peak GH levels, Turner's stigmata and histology of gonadal tissue.
    Results: Five cases of MGD with both male and female phenotype were studies. IGF-1 levels and GH levels showed both features of growth hormone deficiency and growth hormone insensitivity. One study subject has gonadal germ cell tumour (dysgerminoma). We discuss here the issues regarding the GH therapy in MGD subjects.
    Conclusion: Growth deceleration in MGD subjects is partly due to defective growth hormone secretion and partly due to growth hormone insensitivity. MGD subjects are at high risk for occurrence of gonadal tumours. Gonadectomy or biopsy of underlying dysgenetic gonads is essential prior GH therapy. Close surveillance for second neoplasm is to be considered in subjects with history of gonadal tumors prior starting GH for short stature.

    Abstract Background: Despite concentrated global efforts to bring about reduction in malnutrition among children, it continues to remain a public health concern, especially in developing countries such as India. While substantial reduction in the levels of stunting has taken place over the years, high levels of variation exist in distribution of stunting across the country.
    Objective: The study aimed to identify the determinants of stunting in early childhood and their contribution to change in levels of stunting across India. It also compared the socioeconomic disparity in the levels of stunting and changes therein over the last decade.
    Methods: The study utilizes data from the National Family Health Survey (NFHS-3 and NFHS-4) on children aged under-2 years. Bivariate and multivariate logistic regression identified determinants of early childhood stunting followed by Oaxaca decomposition model to assess the contribution of each of the factors to reduction in levels of stunting over the years. Concentration index was used to study the socioeconomic disparity in early childhood stunting.
    Results: Nearly 19% decrease in early childhood stunting can be attributed to increase in institutional deliveries, 14% to increase in maternal schooling, and 10% to improvement in maternal body mass index. In spite of an overall decrease, very little change is seen in socioeconomic disparity of childhood stunting.
    Conclusions: The study identifies institutional deliveries, maternal schooling, and maternal health as major contributors of decrease in early childhood stunting. It identifies persisting socioeconomic disparity in childhood stunting over the last decade.

    Abstract Post-pandemic world has thrown up several challenges, such as, high inflation, low growth, high debt, collapse of economies, political instability, job losses, lowering of income in addition to damages caused natural disasters, more convincing attributed to climate change, apart from existing inequalities. Efforts are being made to mitigate these challenges at various levels. To the best of the knowledge of the author, most of the prior researches have focussed on specific scenarios, use cases, inter-relationships between couple of sectors and more so on optimal policies, such as, impact of carbon tax on individuals, interaction between taxes and welfare, etc. However, not much effort have been made to understand the actual impact on individual agents due to diverse policy changes and how agents cope with changing economic dynamics. This paper considers progressive deteriorating conditions of increase in expense, degrading environmental utility, increase in taxation, decrease in welfare and lowering of income with recourse to inherited properties, credits and return on investments, and tries to understand how the agents cope with the changing situations using an agent based model with matrices related to savings, credits, assets. Results indicate that collapse of agents' economic conditions can be quite fast, sudden and drastic for all income groups in most cases.

    Abstract Rationality is frequently associated with making the best possible decisions. It's widely acknowledged that humans, as rational beings, have limitations in their decision-making capabilities. Nevertheless, recent advancements in fields, such as, computing, science and technology, combined with the availability of vast amounts of data, have sparked optimism that these developments could potentially expand the boundaries of human bounded rationality through the augmentation of machine intelligence. In this paper, findings from a computational model demonstrated that when an increasing number of agents independently strive to achieve global optimality, facilitated by improved computing power, etc., they indirectly accelerated the occurrence of the "tragedy of the commons" by depleting shared resources at a faster rate. Further, as agents achieve optimality, there is a drop in information entropy among the solutions of the agents. Also, clear economic divide emerges among agents. Considering, two groups, one as producer and the other (the group agents searching for optimality) as consumer of the highest consumed resource, the consumers seem to gain more than the producers. Thus, bounded rationality could be seen as boon to sustainability.
