    Abstract Background: Female pattern hair loss (FPHL) is one of the hair loss disorders in women characterized by a gradual reduction in hair from the frontal, central, and parietal areas of the scalp. FPHL is more common than previously thought, and data regarding the same are lacking in the Indian literature. Unlike male pattern hair loss, there are not much studies regarding FPHL in South India. Hence, we intend to study the clinical features, associations, and dermoscopic findings of FPHL in our population. Aims and Objectives: The study was conducted to estimate the clinical and dermoscopic features of FPHL. The secondary objective of the study was to estimate the severity of FPHL based on clinical and dermoscopic features. Materials and Methods: The cross-sectional descriptive study was conducted in 300 patients who were clinically diagnosed with FPHL attending the Department of Dermatology, Government Medical College, Kottayam, during a period of 18 months from February 2021 to August 2022. Results: The mean age of patients with FPHL was 46.99±13.8 years. The majority of the patients were asymptomatic. Hair diameter diversity of >20% and miniaturized hair were the most common dermoscopic findings seen in all patients. Honeycombing and yew dots in dermoscopy were seen in 17 and 8.4% of patients, respectively. Conclusion: Dermoscopes are a useful tool to differentiate FPHL from other causes of diffuse hair loss that could mimic FPHL. The presence of yellow dots and honeycombs indicated advanced FPHL. Thus, dermoscopy helps in detecting early FPHL and initiating early treatment of this condition.

    Abstract Introduction: Accommodation is the ability to see over a range of distances by changing the power of the natural lens. With cataract surgery, this ability is partially or completely abolished. Apparent accommodation or pseudo-accommodation is the potential of pseudophakic eyes to sustain a good amount of near vision. Aim: To measure near vision in children with pseudophakia unaided, with distance vision correction alone (mono focal lens) in-situ and to measure the minimum near add (bifocal near segment power) required to read 1M (normal reading print size) at 30 cm and 40 cm. Materials and Methods: A cross-sectional study was conducted among children between 5-15 years of age with pseudophakia. Vision assessment was done using Lea symbol chart, at 3 metres for distance and at 30 cm and 40 cm for near. Near vision assessment was done unaided and with distant vision correction alone in place. Minimum spherical power needed to read 1M and child’s class textbooks were also assessed. Results: Sixty-four children were included in the study. Mean age was 10.5 (±3.12) years. Refractive status showed spherical power ranging from -7.00 DS to +1.00 DS (-0.8 Mean, (±1.33 SD), astigmatism ranging from -4.50 DC to +3.00 DC (-1.29 mean, ±1.37 SD). Unaided near vision assessment showed that 41 (64%) could read <2M at 30 cm. Nine children could read 1M at 30 cm and three children could read 1M at 40 cm with distant vision correction alone in place. Conclusion: Sixteen (25%) children in our study were independent of spectacles for reading 1M at a reading distance of 30 cm. Nine children (14.1%) read 1M at 30 cm with monofocal spectacles. Twenty-three children (35.9%) had good functional near vision and did not require any add for near work. Forty-six children (71.8%) required only 2 D or less near add for their classwork at 30 cm. Thus, monofocal or reduced near add can give these children a better quality of vision.

    Abstract Zygomycetes species contains two orders of organisms that infect humans, namely Mucorales and Entomophthorales. Entomophthorales cause chronic infection in immunocompetent patients, invading subcutaneous tissues but are non-angioinvasive. This includes Basidiobolus ranarum, Conidiobolus incongruus and Conidiobolus coronatus. We report a case of disseminated tuberculosis with Conidiobolus coronatus infection presenting as orbital cellulitis in an adolescent.

    Abstract Rhinosporidiosis is a granulomatous infection of mucocutaneous tissue caused by Rhinosporidium seeberi that most commonly occurs in the nasal cavity. Ocular rhinosporidiosis affects primarily the conjunctiva. Diagnosis of rhinosporidiosis is based on strong clinical suspicion and is confirmed by histopathological examination. We report a rare case of conjunctival rhinosporidiosis in an immunocompromised patient (human immunodeficiency virus) with disseminated cutaneous rhinosporidiosis. A 44-year-old male presented with a swelling in the right upper eyelid for 6 months. Excision biopsy of the ocular lesion showed multiple thick-walled, variable-sized sporangia containing endospores within the subepithelium suggestive of rhinosporidiosis. A multidrug regimen of systemic cycloserine, ketoconazole, and dapsone was administered to treat disseminated rhinosporidiosis, in addition to antiretroviral therapy. There was good response with reduction in the swellings.

    Abstract Introduction: World Health Organisation’s (WHO) “Vision 2020” right to sight programme gives high priority towards control of ChildHood Blindness (CHB). Blind school screening provides data on the causes of CHB to focus on health care programmes towards prevention of CHB. Aim: To determine the causes of CHB seen in local schools for the blind, to compare these with reports from India and focus on changes in the causes of CHB over the years. Materials and Methods: A cross-sectional study was conducted in two schools for the blind in Tamil Nadu, India. Children less than 18 years were included. Best Corrected Visual Acuity (BCVA), anterior segment and dilated fundus examination was done for all children. Information was gathered as per WHO prevention of blindness (WHO/PBL) form. Results: BCVA in the better eye was 6/24 to 3/60 in 31 children (16.8%), <3/60 to no perception of light in 154 children (83.2%). The causes of blindness based on anatomical site were retina 77(41.5%), whole globe 59(32%), cornea 14(7.5%), optic nerve 14(7.5%), lens 10(5.7%), refractive error in 7(3.8%) and uvea in 4(2%). Hence, 14(7.5%) of the causes were preventable and 21(11.4%) were treatable; thus 35(18.9%) were avoidable causes of blindness. Conclusion: Retinal dystrophies were the major cause of CHB in our study. Posterior segment anomalies contributed to 50% of the causes for CHB. Our study showed that one-fifth of children had avoidable blindness. Regular screening in well baby clinics, schools and in the community is needed for early identification of avoidable blindness in children.

    Abstract Granulocytic sarcoma is an extramedullary tumour consisting of malignant granulocytic precursor cells that is common among children with acute myeloid leukaemia (AML). We report a case of orbital granulocytic sarcoma in an adult with relapsed undifferentiated AML–M0. It presented as bilateral medial canthal swellings. An incisional biopsy confirmed the diagnosis of granulocytic sarcoma. The swelling resolved with re-induction chemotherapy.

    Abstract Introduction: Headache is among the first three most prevalent disorders with a wide treatment gap due to barriers in help seeking. Headache has been associated with disability. However, the relationship of barriers to help-seeking and disability are unexplored. Aim: To find out the barriers to help seeking and its relationship with headache related disability in patients with headache. Materials and Methods: In this hospital based cross-sectional study, 200 consecutive subjects with headache attending a tertiary care centre were recruited as per selection criteria and assessed with Sociodemographic & Clinical Proforma, Mini International Neuropsychiatric Interview (MINI), Barriers to Help Seeking Scale (BHSS), The Henry Ford Hospital Headache Disability Inventory (HDI). Results: High mean score was observed on BHSS subscale need for control and self reliance (19.45; SD ±9.66) and minimizing problem and resignation (10.02; SD ±6.98). Mean score on the HDI was 25.65 (SD ± 14.09). Socioeconomic status of the patient was statistically significant and positively associated with need for control and self reliance (p=0.035), concrete barriers and distrust of care givers (p=0.039), emotional control (p=0.005), and privacy (p=0.002). Occupational status had significant association with need for control and self-reliance (p=0.01), minimizing problem and resignation (p=0.033), and emotional control (p=0.006). Score on hospital headache disability inventory significantly predicted the value of score on concrete barriers and distrust of caregivers domain of HDI (p=0.001). Conclusion: Autonomy and under estimation of seriousness of headache are common barriers to help seeking. Pattern of help seeking barriers may vary with socio-economic status and occupational status, while disability varies with gender and severity of headache. Headache associated disability is positively associated with concrete barriers.

    Abstract Introduction: Studies have found that headache is associated with suicidality. Some demographic and clinical features have been reported to be associated with suicidality. Pain catastrophizing and disability may also have bearing on suicidality in patients with headache. Aim: To evaluate the relationship between pain disability and catastrophizing in headache and suicidal behaviour. Materials and Methods: Patients diagnosed with headache were recruited from neurology Out-Patient Department (OPD). With ethical approval and informed consent, patients were subjected to a structured interview. Headache disability and pain catastrophizing was assessed with the Henry Ford Hospital Headache Disability Inventory (HDI) and Pain Catastrophy Scale (PCS) respectively. For evaluation of suicidal ideation and behaviour, MINI International Neuropsychiatric Interview (MINI Plus) scale and The Columbia-Suicide Severity Rating Scale (C-SSRS) were applied. Data was analysed with Mann-Whitney U test and Kruskal-Wallis test using appropriate statistical programs. Results: In 200 patients of headache, male: female ratio was 0.48: 1. Headache disability was significantly higher in females (p=0.060) and unemployed (p=0.019) patients whereas, pain catastrophizing was significant in patients belonging to low socioeconomic class (p=0.045). Headache disability and pain catastrophizing had significant association with suicidal ideation. Disability score was significantly associated with the score of deterrents (p=0.067) and controllability (p=0.039) subscale of intensity of suicidal ideation. There were four patients who actually attempted suicide. Pain catastrophizing was significantly associated with non-suicidal self injurious behaviour (p=0.041). Conclusion: Disability due to headache and pain catastrophizing is associated with increased suicidal tendencies, behaviour and suicidal attempts.     Abstract Moya Moya Disease (MMD) is characterised by idiopathic vasculopathy affecting the terminal internal carotid arteries resulting in the formation of extensive collaterals at the base of the brain, leptomeninges and parenchymal regions with resultant infarcts and bleeds. Four children presented with clinico-radiological features suggestive of Moyamoya disease/syndrome. This includes global developmental delay, recurrent seizures, transient ischaemic attacks and impaired vision. The first patient had vision of 6/15 in both eyes with bilateral optic disc pallor. Second case also had bilateral optic disc pallor with arteriolar attenuation, but had vision of perception of light only in both eyes. The third child had vision of 6/60 with alternate divergent squint and clinical features suggestive of Neurofibromatosis 1 (NF 1). Fourth patient presented with poor fixation in both eyes with bilateral total cataract. He underwent bilateral cataract surgery with intraocular lens implantation and vision improved to 2/60 with good fixation. We also describe their medical and neurosurgical interventions in this report.
