    Abstract Management of chronic conditions and optimization of overall health has become a primary global health concern in the care of people living with HIV in the era of highly active antiretroviral therapy (ART), particularly in lower-and-middle income countries where infrastructure for chronic disease management may be fragmented. Alterations in body composition can reflect important changes in musculoskeletal health, particularly among populations at risk for developing fat and muscle redistribution syndromes, such as women with HIV on ART. Given the lack of data on this topic in Latin America and the Caribbean, we designed an exploratory study to measure these outcomes in a population of women aging with HIV in Peru. We conducted a cross-sectional study among Peruvian women with and without HIV aged ≥40 years. Dual X-ray absorptiometry was used to measure trunk and limb lean mass (LM) and fat mass (FM). Physical performance was assessed with the Short Physical Performance Battery (SPPB) and physical strength with a dynamometer. Sarcopenia was assessed based upon EWGSOP criteria. We used linear regression to model associations between body composition, sarcopenia and physical performance scores. 104 women with HIV and 212 women without HIV were enrolled (mean age 52.4±8.2 vs. 56.4±8.8 years, p≤0.001). Among women with HIV, mean years since diagnosis was 11.8±6 and all were on ART. Mean SPPB score was 9.9 vs 10.8 (p<0.001) between both groups. Sarcopenia spectrum was found in 25.9% and 23.1%, respectively. In the multivariable regression analysis, trunk FM and older age were negatively correlated with physical performance among women with HIV. Severe sarcopenia was found among a greater proportion of those with HIV (3.8% vs. 0.9%, p = 0.84), however this finding was not statistically significant. Women with HIV had significantly lower SPPB scores compared to women without HIV, and trunk FM and upper limb LM were independent predictors for the SPPB and Grip Strength tests, respectively.     Abstract Central line catheter-related bloodstream infections (CLABSI) burdens great morbidity, mortality and unnecessary hospital expenses. Studies related to its incidence and epidemiological and clinical profile among neonates in Peru are scarce, not being clear it's actual impact. A prospective cohort study was conducted in a neonatal intensive care unit (ICU) of a public hospital in Lima, Peru between 2017- 2018. 167 patients were included (52,7% male) with gestational age between 24-41 weeks, obtaining 1999 catheter-days and 16 cases of CLABSI. The incidence rate was 8/1000 catheter-days. Use of umbilical catheter (p=0,005) and multiple catheters (p0,001) both showed a statistically significant correlation regarding the development of CLABSI. It’s necessary to extend the study to other ICUs and stablish solid, efficient and long-lasting system of CLABSI surveillance that allows the evaluation of possible interventions to reduce the incidence of CLABSI.

    Abstract Background Studies have shown that women aging with HIV have significantly lower health-related quality of life (HRQoL) compared to women without HIV. However, no studies have examined this issue in Latin America and the Caribbean. We aimed to explore HRQoL measured by the 36-Item Short Form Health Survey (SF-36) among women aging with and without HIV in Peru. Materials and methods We conducted a cross-sectional study at a large HIV-clinic in Peru. Outcomes of the SF-36 were evaluated, exploring the relationship between physical activity (International Physical Activity Questionnaire), sociodemographic factors (ethnicity, alcohol/tobacco use, age, BMI) and clinical data (AIDS progression, treatment duration, CD4+ cell count and viral load, years since HIV diagnosis) with HRQoL using regression analysis. Statistical significance was set with a two-tailed p-value <0.05. Results We enrolled 427 women (175 HIV-infected) with mean age of 54±8 years. From the SF-36 individual domains: physical functioning, role limitations due to physical and emotional health, and emotional wellbeing were significantly lower for HIV-infected women. Summary component scores were lower for the HIV-subset for both physical (45.8 vs 47.3) and mental (45.1 vs 45.8) components, although they did not achieve statistical significance. Regression analysis of the HIV-infected women revealed that the physical component score was significantly associated with physical activity, ethnicity, and chronic comorbidities while the mental component was significantly associated with physical activity, employment, and CD4+ cell count. Conclusion In our study, HIV-infected women scored lower in both physical and mental component scores. Important determinants for each component included CD4+ cell count as an assessment of HIV severity for the mental component, and ethnicity, reflecting socio-cultural factors, for the physical component.     Abstract En Colombia, el género Polylepis está representado por P. quadrijuga, P. sericea y P. incana. La modelización de la distribución geográfica potencial permite identificar áreas relevantes para conservar especies focales. Los objetivos de este trabajo fueron establecer las áreas de distribución potencial de las tres especies e identificar oportunidades de conservación en áreas protegidas a escala nacional, regional y local. Para esto, se recopilaron 223 registros biológicos (GBIF y SIB Colombia) que, junto con 23 variables ambientales, fueron los insumos para modelizar la distribución potencial en MaxEnt 3.3.3K. Como oportunidades de conservación, se cuantificó la presencia de reservas del SINAP-Colombia en las áreas de distribución potencial de las especies. Las variables que más contribuyen en los modelos son altitud, temperatura estacional y rango de temperatura media mensual, entre otras. P. sericea presentó el área de distribución potencial más amplia, con 23243.5 km2 en las tres cordilleras; le siguieron P. quadrijuga, con 6264.2 km2 en la Cordillera Oriental y P. incana, con 1772.8 km2 en la Cordillera Central y la frontera con Ecuador. Una parte de dichas distribuciones potenciales se encuentra en, al menos, un tipo de área protegida. P. incana tiene 59.5% del área de distribución potencial dentro de alguna figura de protección, P. quadrijuga presenta 44.8% de área potencial de distribución ubicada dentro de cualquier figura de protección y P. sericea, al tener una mayor superficie potencial a partir de la modelización, posee un porcentaje más bajo de representatividad (33.7%) en áreas protegidas del SINAP. Finalmente, a partir del análisis de amenazas y oportunidades se proponen áreas clave para la conservación de Polylepis y zonas para la búsqueda de posibles poblaciones no reportadas. https://doi.org/10.25260/EA.18.28.1.1.585

    Abstract The COVID-19 pandemic and societal response implemented may interact with the ongoing HIV epidemic in multiple ways. There are approximately 87000 people living with HIV (PLWH) who are at risk of developing COVID-19 in Peru and 67,000 of them are on antiretroviral therapy (ART) and at risk of limitations in their access to ART, compromising their adherence and their health during the pandemic. Finally, the potential effect of the pandemic on the mental health of PLWH is not documented. This opinion aims to: describe the clinical implications of the HIV/SARS-CoV-2 coinfection; discuss the challenges to the continuity of care of PLWH in Peru during the COVID-19 crisis; and comment possible implications that the COVID-19 crisis may pose on the mental health of PLWH.

    Abstract Los bosques de Polylepis, endémicos de las montañas de Sudamérica, suelen estar asociados con sitios relativamente inaccesibles como quebradas, roquedales o laderas empinadas. Las hipótesis más discutidas que explican esta asociación son: 1) factores abióticos como la humedad, la temperatura o el viento, ó 2) factores antropogénicos como la protección del ramoneo por ganado doméstico, la tala o los fuegos iniciados por el ser humano. Para resumir el estado de avance en el conocimiento, realizamos una revisión cuantitativa sobre los estudios de ecología y conservación de bosques de Polylepis a lo largo de toda su distribución. Identificamos 139 publicaciones relevantes, de las que el 36% estuvo concentrado en la Argentina. Las publicaciones incluyen 20 de las ~27 especies existentes, de las cuales las más estudiadas son P. australis y P. tarapacana, con 45 y 21 publicaciones, respectivamente. En el otro extremo, hay 7 especies no representadas en publicaciones. Las disciplinas más populares fueron la ecología de poblaciones (35%) y los estudios sobre biodiversidad y comunidades (18%). Muy pocos estudios se repiten de una región a la otra. Los estudios que intentan comprender la contribución relativa de factores abióticos vs. antropogénicos para explicar la asociación de los bosques con lugares relativamente inaccesibles señalan una contribución mayor de los factores antrópicos, aunque todos están concentrados en el extremo sur de la distribución de Polylepis en la Argentina. Sugerimos áreas de vacancia en estudios y posibles mejoras en la producción de nuevas publicaciones y en la red de interacciones entre investigadores interesados en los ecosistemas de Polylepis. https://doi.org/10.25260/EA.18.28.1.1.522

    Abstract Polylepis is the dominant genus of the high Andean forest vegetation and they have a crucial ecological role. However, these forests are highly endangered and many of their biological aspects are still unknown. In this context, a series of questions were formulated with the aim that their resolution would contribute to a better understanding of Polylepis’dynamics and biological diversity, as well as improve their conservation efforts. Twenty one speakers from the V International Congress of Conservation and Ecology of Polylepis forests participated in the formulation of 67 questions related to these forests. Comments and additional information were received via email and google doc spreadsheet, open to the participants. Afterwards, two workshops were organized with the researchers to formulate, discuss, analyze and define the final list, narrowing down to a total of 40 questions. The questions were categorized into 6 different topics: Taxonomy, Ecology and Biogeography, Genetics, Conservation, Ethnobiology and Climate Change. We hope these questions will be considered as a priority for new research agendas and to define joint efforts for Polylepis forest conservation.

    Abstract Background Pandi’s mushroom-tongue salamander (Bolitoglossa pandi) is one of the threatened amphibians in South America, as well as a flagship species for the Colombian conservation agenda. This species is endemic to the Andean cloud forests of the western slope of the Cordillera Oriental of Colombia, occurring only in the department of Cundinamarca within a narrow elevational range. At night, B. pandi can be seen perching on the upper side of leaves at heights ranging from ground level to 2.5 m. During the day, it can be found under leaf litter or cover objects. Few studies have provided relevant information that can help the Colombian government to formulate lines of action for the conservation of this species; consequently, its threat assessments so far have been based on very limited information. Methods We conducted surveys for salamanders in four municipalities of Cundinamarca, Colombia, using two approaches: visual encounter surveys (Guaduas and Villeta) and the basic sampling protocol for single-species occupancy modeling (Supatá and Venecia). Multivariate analyses were employed to explore the correlation between habitat structure and natural history traits, abundance, and detection/non-detection of B. pandi. We evaluated the B. pandi activity pattern through kernel density curves for each sampling occasion and explored the variability of salamander abundance during their activity period by performing a nested ANOVA. Results We report the discovery of two new populations of B. pandi, which represent the most northwestern records known. A significant correlation between body length, body mass, and habitat structure was observed. Multivariate analyses indicated that leaf litter depth, mean temperature, percent vegetation cover, and altitude were the habitat variables that together explained 60.3% of the B. pandi abundance variability, as well as the main determinants of its optimal habitat.     Abstract Background: In the COVID-19 pandemic, older adults from vulnerable ethnoracial groups are at high risk of infection, hospitalization, and death. We aimed to explore the pandemic's impact on the well-being and cognition of older adults living in the United States (US), Argentina, Chile, Mexico, and Peru. Methods: 1,608 (646 White, 852 Latino, 77 Black, 33 Asian; 72% female) individuals from the US and four Latin American countries aged ≥ 55 years completed an online survey regarding well-being and cognition during the pandemic between May and September 2020. Outcome variables (pandemic impact, discrimination, loneliness, purpose of life, subjective cognitive concerns) were compared across four US ethnoracial groups and older adults living in Argentina, Chile, Mexico, and Peru. Findings: Mean age for all participants was 66.7 (SD = 7.7) years and mean education was 15.4 (SD = 2.7) years. Compared to Whites, Latinos living in the US reported greater economic impact (p < .001, ηp2 = 0.031); while Blacks reported experiencing discrimination more often (p < .001, ηp2 = 0.050). Blacks and Latinos reported more positive coping (p < .001, ηp2 = 0.040). Compared to Latinos living in the US, Latinos in Chile, Mexico, and Peru reported greater pandemic impact, Latinos in Mexico and Peru reported more positive coping, Latinos in Argentina, Mexico, and Peru had greater economic impact, and Latinos in Argentina, Chile, and Peru reported less discrimination. Interpretation: The COVID-19 pandemic has differentially impacted the well-being of older ethnically diverse individuals in the US and Latin America. Future studies should examine how mediators like income and coping skills modify the pandemic's impact. Funding: Massachusetts General Hospital Department of Psychiatry.
