    Abstract Objective. To describe the relation between corruption indicators and statistics on noncommunicable diseases and their risk factors by continent. Methods. An ecological study was conducted to examine the relation of the GINI coefficient, the Country Policy and Institutional Assessment (CPIA), and the Corruption Perception Index (CPI) with noncommunicable diseases, using the Spearmans rank correlation test. Results. There is a moderate and positive correlation between Corruption Perception Index and cause of death due to noncommunicable diseases and risk factors for these diseases (r = 0.532), prevalence of schizophrenia (r = 0.526), bipolar disorder (r = 0.520), and eating disorders (r = 0.677). There is a moderate negative association between the GINI index and cause of death due to noncommunicable diseases (r = 0.571) and smoking prevalence (r = 0.502), and between the Corruption Perception Index and mortality caused by cardiovascular diseases, cancer, diabetes, or chronic respiratory diseases between the exact ages of 30 and 70 years (r = 0.577) and malnutrition prevalence (r = 0.602). Conclusions. This study indicates a correlation between corruption and noncommunicable diseases and their risk factors. This suggests that the high prevalence of noncommunicable diseases and risk factors could be related with political practices that negatively impact the population. Further research should study the weight of these associations, to take action on the way corruption is impacting on the health of societies.

    Abstract He leído con atención el artículo titulado “Perfil de la neurocirugía en Colombia: 2015”, aparecido en su edición 111. En esta publicación, el autor hace un amplio recorrido por campos de la demografía, la historia, la dinámica social y las ciencias administrativas (lamentablemente sin citar sus bases documentales). Cuando aterriza su tema en la neurocirugía en Colombia, afirma que en 2002 había 324 neurocirujanos en el país. Ese número de neurocirujanos fue justamente el que se encontró en el estudio de oferta y demanda de médicos especialistas (proyecto MEDEA) que yo dirigí en 2000 y que publicamos en 2001 (1). Desde ese momento era clara la problemática de la especialidad. Ya decía entonces nuestro estudio: “La tasa de neurocirujanos en Colombia es similar a la de Canadá, Suiza y Australia, y el triple de la del Reino Unido. En el contexto de un país en desarrollo, con una red hospitalaria deficiente, esta tasa es claramente excesiva” (2). Alertábamos también sobre el número creciente de nuevos especialistas, muchos de ellos formados fuera del país. La preocupación por el futuro de la neurocirugía ocurre también en otras latitudes. Una encuesta a una muestra de 783 miembros de la Asociación Americana de Neurocirujanos (AANS) mostró que 52 % creían que su ejercicio profesional estaba empeorando, mientras que 57 % cumplían criterios de agotamiento (“burnout”). Cabe agregar que la principal de las preocupaciones de este grupo de neurocirujanos era la búsqueda de un equilibrio entre su trabajo y la vida por fuera del hospital (3).

    Abstract Introduction. Breast reconstruction, either immediate or delayed, is part of the treatment of breast cancer. Each country and health system pays for and evaluates these procedures in different ways. Thus, it is important to determine which strategy is most cost-effective in Colombia. Objective: To evaluate the cost-utility of breast cancer treatment with immediate reconstruction compared with delayed reconstruction. Materials and methods: We used a decision tree model and a one-year time horizon from the perspective of the third-party payer; the cost data were taken from the Colombian Instituto de Seguros Sociales 2001 rate manual plus a 30% adjustment according to the methodology of the Instituto de Evaluación Tecnológica en Salud, IETS, and the billing model of the Centro Javeriano de Oncología at the Hospital Universitario San Ignacio. The transition probabilities and profits were obtained from medical specialists, patients, and the medical literature. We also conducted univariate and probabilistic sensitivity analyses. Results: The expected costs per capita were COP$ 26,710,605 (USD$ 11,165) for the immediate reconstruction and COP$ 26,459,557 (USD$ 11,060) for the deferred reconstruction. Immediate reconstruction generated an incremental cost of COP$ 251,049 (USD$ 105) and 0.75 quality-adjusted life years (QALY), while deferred reconstruction generated 0.63 QALYs, with an incremental cost-utility ratio of COP$ 2,154,675 per QALY (USD$ 901). Conclusions: The cost per QALY did not exceed the acceptability threshold of the Gross Domestic Product (GDP) per capita. The costs for the first year were similar. Both techniques are favorable for the Colombian health system, but the utility reported by patients and the literature is greater with the immediate reconstruction.

With continued drug development and approval of hepatitis C antivirals, research is expected to increase. Our findings will assist scholars and policy makers in their efforts to improve scientific research policies, with the goal of maximizing the access to treatments, especially in low and middle-income countries.

    Abstract Abstract Background In Colombia, the best strategy to establish indication for adjuvant chemotherapy in early breast cancer (EBC) remains unknown. This study aimed to identify the cost-utility of Oncotype DX™ (ODX) or Mammaprint™ (MMP) tests to establish the necessity of adjuvant chemotherapy. Methods This study used an adapted decision-analytic model to compare cost and outcomes of care between ODX or MMP tests and routine care without ODX or MMP tests (adjuvant chemotherapy for all patients) over a 5-year time horizon from the perspective of the Colombian National Health System (NHS; payer). Inputs were obtained from national unit cost tariffs, published literature, and clinical trial database. The study population comprised women with hormone-receptor-positive (HR +), HER2-negative, lymph-node-negative (LN0) EBC with high-risk clinical criteria for recurrence. The outcome measures were discounted incremental cost-utility ratio (ICUR; 2021 United States dollar per quality-adjusted life-year [QALY] gained) and net monetary benefit (NMB). Probabilistic (PSA) and deterministic sensitivity analysis (DSA) were performed. Results ODX increases QALYs by 0.05 and MMP by 0.03 with savings of $2374 and $554 compared with the standard strategy, respectively, and were cost-saving in cost-utility plane. NMB for ODX was $2203 and for MMP was $416. Both tests dominate the standard strategy. Sensitivity analysis revealed that with a threshold of 1 gross domestic product per capita, ODX will be cost-effective in 95.5% of the cases compared with 70.2% cases involving MMP.DSA showed that the variable with significant influence was the monthly cost of adjuvant chemotherapy. PSA revealed that ODX was a consistently superior strategy. Conclusions Genomic profiling using ODX or MMP tests to define the need of adjuvant chemotherapy treatment in patients with HR + and HER2 −EBC is a cost-effective strategy that allows Colombian NHS to maintain budget.

    Abstract RESUMEN Por la menor incidencia de efectos secundarios extrapiramidales y por una mayor adherencia de los pacientes al tratamiento, los antipsicóticos atípicos de liberación prolongada constituyen una alternativa para el tratamiento de la esquizofrenia. En este análisis de costo-beneficio se compara un antipsicótico atípico de liberación prolongada (risperidona) con dos medicamentos orales de nueva generación (olanzapina y clozapina) y uno típico de depósito (pipotiazina). Se empleó un modelo de árbol decisión que incorporó exclusivamente costos directos, desde la perspectiva de una EPS (“tercero pagador”) y empleando las ivariables adherencia, recaídas con y sinhospitalización, y efectos adversos. Según el modelo, el empleo de risperidona inyectable de liberación prolongada durante un año de tratamiento, y de manera similar a la clozapina, ahorraría un millón de pesos al sistema al compararse con la olanzapina, y dos millones al compararse con la pipotiazina. A esta ganancia en dinero con la risperidona inyectable se agregaría el beneficio directo e Indirecto para el paciente y su comunidad al reducir las recaídas. Summary Due to the lower incidence of extrapyramidal side effects and greater adherence of patients to treatment, long-action atypical antipsychotics are an alternative for the treatment of schizophrenia. In this cost-benefit analysis we compare a long-action atypical antipsychotic (risperidone) with two second-generation oral antipsychotics (olanzapine and clozapine) and a “typical” prolonged action drug (pipotiazine). We designed a decision tree that included only direct costs, from the third party payer perspective, using as main variables adherence, relapses with and without hospitalization, and side effects.     Abstract ABSTRACT Rabies encephalitis is a fatal zoonotic viral disease transmitted to humans either by domestic animals like dogs and cats or by wild animals like bats, skunks and raccoons. We present the case of a 25-year-old woman admitted due to behavioral disorders, generalized paresthesiasand acute respiratory deterioration compatible with a respiratory distress syndrome (ARDS) requiring orotracheal intubation, mechanical ventilation and empirical initiation of antibiotic and antiviral therapy. Chest tomography showed pneumomediastinum and changes suggestive of pulmonary infection. In the presence of neurological symptoms, a central nervous system (CNS) infection was suspected and the cerebrospinal fluid showed no pleocytosis, hiperproteinorrachy without glucose consumption; cranial CT scan was normal. During hospitalization, the family reported that the patient was bitten by a cat 30-day prior to the onset of rabies-like symptoms, and the animal was sacrificed. The patient had an unfavorable clinical evolution, with electroencephalographic activity dissociation evidenced by video telemetry. The zoonotic exposure led to the suspected diagnosis of rabies infection. The patient died and the suspected diagnosis was confirmed by histopathology, with presence of Negri bodies on cerebellum Purkinje cells and a positive immunofluorescence test for rabies virus. Both, initial extra-neural manifestations and late reporting of rabies exposure led to delayed diagnosis.
