    Abstract Objective: To investigate the mechanism of self-efficacy between self-management ability and self-management behavior and its differences among patients with different disease courses through mediation tests.
    Methods: In the study, 489 patients with type 2 diabetes who attended the endocrinology departments of four hospitals in Shanxi Province and Inner Mongolia Autonomous Region from July to September 2022 were enrolled as the study population. They were investigated by General Information Questionnaire, Diabetes Self-Management Scale, Chinese version of Diabetes Empowerment Simplified Scale, and Diabetes Self-Efficacy Scale. Mediation analyses were performed using the linear regression model, Sobel test, and Bootstrap test in the software Stata version 15.0 and divided the patients into different disease course groups for subgroup analysis according to whether the disease course was > 5 years.
    Results: In this study, the score of self-management behavior in the patients with type 2 diabetes was 6.16±1.41, the score of self-management ability was 3.99±0.74, and the score of self-efficacy was 7.05±1.90. The results of the study showed that self-efficacy was positively correlated with self-management ability (
    Conclusion: Self-efficacy enhanced the effect of self-management ability on the behavior of the patients with type 2 diabetes, and this positive effect was more significant for the patients with shorter disease course. Targeted health education should be carried out to enhance patients' self-efficacy and self-management ability according to their disease characteristics, to stimulate their inner action, to promote the development of their self-management behaviors, and to form a more stable and long-term mechanism for disease management.

    Abstract Objective: Gelanxinning capsule (GXSC) is a Chinese medicine to cure coronary artery disease (CAD) and a compound of Pueraria lobata, hawthorn extract, and gypenosides. However, whether GXSC could improve coronary microvascular dysfunction (CMD) is unknown. We aimed to demonstrate the therapeutic effect of GXSC on CMD and its underlying mechanisms in CAD patients.
    Patients and methods: This was a single-center, randomized control trial. A total of 78 patients diagnosed by selective coronary angiography (CAG) participated in this study. Patients' demographics, medical history, medications, and results of laboratory testing were collected. The index of microcirculatory resistance (IMR) and coronary flow reserve (CFR) were obtained by CAG and single-photon emission computed tomography (SPECT) separately. Fasting blood samples were obtained on the morning following the admission day. Concentrations of several molecules of inflammation, endothelial function, and coronary microvascular function were measured by ELISA. Patients were followed-up two months after discharge and fasting blood samples were also acquired.
    Results: All patients were randomly divided into 2 groups: GXSC, 38 (48.7%), and control, 40 (51.3%). The intergroup comparison revealed no significant differences with respect to all baseline variables. As for inflammation biomarkers, proinflammatory NOD-like receptor thermal protein domain associated protein 3 (NLRP3) and interleukin (IL)-1 were significantly decreased in GXSC compared with the control group (0.71±0.08 vs. 1.04±0.07, p<0.01 and 7.16±0.59 vs. 10.93±1.04, p<0.01). Anti-inflammatory adropin was increased in the GXSC group (7.75±0.59 vs. 5.71±0.68, p=0.03). As for indexes of endothelial function, the concentrations of syndecan (SDC) 1, SDC4 and heparan sulphates (HS) were significantly downregulated in 2 months GXSC treatment (3.31±0.28 vs. 4.85±0.43, p<0.01, 3.79±0.56 vs. 5.69±0.68, p=0.03 and 21.31±2.79 vs. 35.18±4.11 p<0.01). In addition, the level of SIRTUIN 1 (SIRT1), which is a vascular protective protein, was upregulated in GXSC group (5.63±0.30 vs. 4.22±0.37, p<0.01). As for molecules of coronary microvascular function, endocan, soluble urokinase plasminogen activator receptor (suPAR), and growth differentiation factor (GDF)-15 were significantly decreased consistently in GXSC compared with the control group (0.09±0.01 vs. 0.19±0.03, p<0.01, 4.44±0.40 vs. 5.73±0.40, p=0.03 and 2.08±0.17 vs. 2.69±0.18, p=0.02).
    Conclusions: In conclusion, GXSC could improve CMD by inhibiting inflammation and restoring endothelial function. GXSC might be an effective drug in CAD patients without obstructive epicardial coronary arteries but suffering from angina.

    Abstract Endocan is a novel marker for inflammation but its significance in preeclampsia remains unknown.
    Objective: The aims of this study were to delineate the changes of maternal endocan in normal pregnancy and preeclampsia and to explore its possible role in the pathogenesis of preeclampsia.
    Materials and methods: Blood samples were taken from 42 non-pregnant women, from 43 normal pregnant women at third trimester, and from 41 preeclamptic women. Serum endocan levels were determined with enzyme-linked immunosorbent assay (ELISA) and compared among the groups.
    Results: There were no significant differences in serum endocan among non-pregnant women, normal pregnant women, and women with preeclampsia (X²= 2.207, p = 0.137) and there was no significant difference in serum endocan between women with mild and severe preeclampsia (Z= 0.368, p = 0. 713). The authors concluded that serum endocan did not change in pregnancy and preeclampsia, indicating endocan may not be involved in the pathogenesis of preeclampsia.

    Abstract Aim: The study was to determine whether preeclampsia with superimposed chronic hypertension results in worse maternal and perinatal outcomes than preeclampsia alone.
    Materials and methods: A retrospective study involving 850 pregnant women was conducted and divided into two groups: preeclampsia superimposed on chronic hypertension (group A, n = 84) and preeclampsia alone (group B, n = 766). The maternal and fetal outcomes of all subjects were collected and analyzed.
    Results: There were no significant differences between the two groups in baseline information. However, the systolic and diastolic blood pressures in group A were significantly higher than those in group B (p < 0.05). The average interval between the onset of preeclampsia and the termination of pregnancy was significantly longer in group A as compared to group B. The incidence of serious maternal complications showed no differences between the two groups (p > 0.05). It showed a higher rate of neonatal respiratory distress syndrome and intracranial hemorrhage in group A than in group B (p < 0.05).
    Conclusions: Women in group A had higher risks of maternal and perinatal outcomes as compared to women in group B.

    Abstract Serum ferritin concentrations of 50 normal women and 90 patients with neoplasms of female genital tract were determined by radioimmunoassay. The mean value of serum ferritin in 23 cases of ovarian carcinoma was 402.04 micrograms/L, significantly higher than that of normal subjects and patients with benign genital neoplasms. Serum ferritin levels in patients with endometrial carcinoma, endometrial stromal sarcoma, and benign genital neoplasms were significantly higher than that of the normal subjects. There was a positive correlation between the serum ferritin level and the clinical stage of ovarian carcinoma. The serum ferritin determination is useful in the diagnosis, differential diagnosis and prognosis of ovarian cancers.

    Abstract The therapeutic effect of bismuth subsalicylate (BSS, Pepto-Bismol) in Clostridium difficile colitis was studied in golden Syrian hamsters. C. difficile was fed to the hamsters by orogastric intubation 2-3 days after their arrival. Clindamycin (1.5 mg per animal) was given intraperitoneally 4 days later. Twenty-four hours after challenge with clindamycin, animals were given BSS at dosages of 5, 10, and 15 mg twice daily for 5 days by orogastric intubation. Controls included untreated animals and those given 5 mg of vancomycin once daily by intubation. Delay in the time of death was observed in all BSS-treated animals and was statistically significant on days 4-6 in those receiving 15 mg twice daily. Vancomycin produced a greater delay in death than did BSS. Our study suggests that BSS at a dosage of 15 mg twice daily has some therapeutic effect on C. difficile colitis in hamsters.

    Abstract LY146032, an acidic lipopeptide antibiotic which inhibits the biosynthesis of cell wall peptidoglycan, was found to be effective in delaying death in a hamster model of pseudomembranous colitis. A dose of 0.05 mg/day was effective. The equivalent protection with vancomycin required a dose 100-fold higher, i.e., 5 mg/day.

    Abstract Mice exposed to 1400 rads of total body irradiation experienced 80%-100% mortality in 2 wk. Bacteremia was demonstrated in all dead animals. Feeding Lactobacillus GG strain reduced Pseudomonas bacteremia and prolonged survival time in animals colonized with this organism. In animals not colonized with Pseudomonas, feeding Lactobacillus GG also produced some reduction in early deaths, and there was less Gram-negative bacteremia in these animals compared with controls.
