    Abstract Purpose: The purpose of this study was to explore and describe the meaning and essence of a mother’s experience of hospitalization of her newborn in the neonatal intensive care unit (NICU). Methods: This study employed a qualitative research design. An interview was conducted with a mother whose newborn was hospitalized in the NICU, and the data were analyzed using Giorgi’s phenomenological method. Results: Five main themes and 19 formulated meanings were indentified. The 5 themes were ‘drowning in pain’, ‘just look outside the glass door’, ‘being a pillar’, ‘a deepening attachment’, and ‘prepare for nurturing with hope’. Conclusion: The results of this study provided an in-depth understanding of the experience of a mother with a newborn in the NICU. These results can be used in the development of a nursing intervention program that provides psychological and emotional support to the mother and family.

    Abstract The simultaneous regulation of cancer cells and inflammatory immune cells in the tumor microenvironment (TME) can be an effective strategy in treating aggressive breast cancer types, such as triple-negative breast cancer (TNBC). Apurinic/apyrimidinic endonuclease 1/redox effector factor 1 (APE1/Ref-1) is a multi-functional nuclear protein that can be stimulated and then secreted. The extracellular APE1/Ref-1 causes a reduction in disulfide bonds in cytokine receptors, resulting in their conformational changes, thereby inhibiting inflammatory signaling. Furthermore, the secreted APE1/Ref-1 in response to acetylation has been shown to bind to a receptor for the advanced glycation end product (RAGE), initiating the apoptotic cell death of TNBC in vitro and in vivo. This study used PPTLS-APE1/Ref-1 in an adenovirus vector (Ad-PPTLS-APE1/Ref-1) for the constant expression of extracellular APE1/Ref-1, and our results demonstrated its dual function as an apoptotic initiator and inflammation regulator. Injecting MDA-MB 231 orthotopic xenografts with the Ad-PPTLS-APE1/Ref-1 inhibited tumor growth and development in response to acetylation. Moreover, Ad-PPTLS-APE1/Ref-1 generated reactive oxygen species (ROS), and tumor tissues derived from these xenografts exhibited apoptotic bodies. Compared to normal mice, a comparable ratio of anti- and pro-inflammatory cytokines was observed in the plasma of Ad-PPTLS-APE1/Ref-1-injected mice. Mechanistically, the disturbed cytokine receptor by reducing activity of PPTLS-APE1/Ref-1 inhibited inflammatory signaling leading to the inactivation of the p21-activated kinase 1-mediated signal transducer and activator of transcription 3/nuclear factor-κB axis in tumor tissues. These results suggest that the regulation of inflammatory signaling with adenoviral-mediated PPTLS-APE1/Ref-1 in tumors modulates the secretion of pro-inflammatory cytokines in TME, thereby inhibiting aggressive cancer cell progression, and could be considered as a promising and safe therapeutic strategy for treating ...

    Abstract Abstract This study aimed to identify the prevalence of sarcopenia, obesity, and sarcopenic obesity and examine their association with radiographic knee osteoarthritis (OA) and knee pain in Korean postmenopausal women. This cross-sectional study utilized the data from Korean National Health and Nutrition Examination Surveys 2009–2011. The participants were categorized into 4 groups based on body composition: either sarcopenic (appendicular skeletal muscle < 23%) or not, either obese (body mass index ≥ 25.0 kg/m2) or not. The prevalence of radiographic knee OA and knee pain was calculated. The effect of hormone replacement therapy (HRT) was also evaluated. The prevalence of radiographic knee OA, knee pain, and both were all highest in the sarcopenic obese group and lowest in the control group (61.49% vs. 41.54%, 39.11% vs. 27.55%, 32.04% vs. 17.82%, all p < 0.001). Without sarcopenia, obese women showed significantly higher ratio of radiographic knee OA only (57.64% vs. 41.54%, p < 0.001). With sarcopenia, the coexistence of obesity presented higher ratio of radiographic knee OA, knee pain, and both compared to sarcopenia without obesity (61.49% vs. 41.82%, 39.11% vs. 27.61%, 32.04% vs. 17.60%, all p < 0.001). The use of HRT for more than 1 year was not associated with radiographic knee OA, knee pain, or both (p = 0.147, 0.689 and 0.649, respectively). Obesity with sarcopenia had greater effect on knee OA compared to obesity without sarcopenia. Moreover, HRT use for more than 1 year was not associated with the prevalence of knee OA. Therefore, more efforts should focus on reducing body fat and increasing muscle in postmenopausal women with knee OA.

    Abstract BACKGROUND:Although walking speed is associated with important clinical outcomes and designated as the sixth vital sign of the elderly, few walking-speed estimation algorithms using an inertial measurement unit (IMU) have been derived and tested in the older adults, especially in the elderly with slow speed. We aimed to develop a walking-speed estimation algorithm for older adults based on an IMU. METHODS:We used data from 659 of 785 elderly enrolled from the cohort study. We measured gait using an IMU attached on the lower back while participants walked around a 28 m long round walkway thrice at comfortable paces. Best-fit linear regression models were developed using selected demographic, anthropometric, and IMU features to estimate the walking speed. The accuracy of the algorithm was verified using mean absolute error (MAE) and root mean square error (RMSE) in an independent validation set. Additionally, we verified concurrent validity with GAITRite using intraclass correlation coefficients (ICCs). RESULTS:The proposed algorithm incorporates the age, sex, foot length, vertical displacement, cadence, and step-time variability obtained from an IMU sensor. It exhibited high estimation accuracy for the walking speed of the elderly and remarkable concurrent validity compared to the GAITRite (MAE = 4.70%, RMSE = 6.81 /, concurrent validity (ICC (3,1)) = 0.937). Moreover, it achieved high estimation accuracy even for slow walking by applying a slow-speed-specific regression model sequentially after estimation by a general regression model. The accuracy was higher than those obtained with models based on the human gait model with or without calibration to fit the population. CONCLUSIONS:The developed inertial-sensor-based walking-speed estimation algorithm can accurately estimate the walking speed of older adults.

    Abstract Spirochetes, a genetically and morphologically distinct group of bacteria, are thin, spiral-shaped, and highly motile. They are known causes of several human diseases such as syphilis, Lyme disease, relapsing fever, and leptospirosis. We report a case of colonic spirochetosis in a healthy patient presenting for surveillance colonoscopy. The diagnosis of intestinal spirochetosis was made accidentally during the histological examination of colonic polyps, which were removed during colonoscopy. We also performed an extensive review on intestinal spirochetosis with a focus on clinical presentation and outcomes of reported cases from the past two decades.
