    Abstract Introduction: Percutaneous Nephrolithotomy (PCNL) has become the standard of care for large renal calculi. The aim of the surgery is to achieve maximum stone clearance with minimal postoperative complications. Various scoring systems have been described to predict both of these outcomes. S.T.O.N.E. Score and Guy's Stone Score (GSS) are two of the most widely used scoring systems. S.T.O.N.E. Score comprises of Size of the stone, Tract length, degree of Obstruction of the urinary system, Number of stones, and Essence. Aim: To compare the predictive power for stone clearance and postoperative complications of the two scoring systems, namely S.T.O.N.E. Score and GSS. Materials and Methods: This cross-sectional study was conducted at the Department of Genitourinary Surgery at Government Medical College, Kottayam, Kerala, India from March 2019 to August 2020. All patients above 18 years undergoing PCNL were included. A total of 122 patients were studied by calculating the preoperative S.T.O.N.E. Score and GSS and comparing them with post-PCNL stone clearance and complications. The association of both S.T.O.N.E. Score and GSS with stone clearance was estimated by plotting the Receiver Operating Curve (ROC) curve using Statistical Package for Social Sciences (SPSS) version 20, International Business Machines (IBM) SPSS Statistics windows, version 20.0 (Armonk, NY: IBM Corp.). A p-value <0.05 was considered statistically significant. Results: The mean age of the subjects was 49.8±12.47 years. A total of 76 males and 46 females were included in the present study. The mean S.T.O.N.E. Score among the study subjects was 7.12±1.57, and the mean GSS was 2.09±0.48. Complete stone clearance was achieved in 96 (78.7%) patients. A total of 22 (18%) patients had postoperative complications. Both scores had a significant association with stone clearance (p<0.001 for both) and postoperative complications (p-value for S.T.O.N.E.     Abstract Introduction: Hypospadias is a common condition which needs to be repaired within the first few years of life. However, the surgery is usually associated with several complications of which Urethrocutaneous Fistula (UCF) is the most common. Several surgical techniques have been described to correct these UCF. Use of a vascularized flap to repair this fistula is the current standard. Aim: To assess the feasibility of using tunica vaginalis flap in repair of UCFs. Materials and Methods: The present study was an observational study where 32 patients with UCF resulting from a previous failed hypospadias repair were subjected to tunica vaginalis flap repair. These patients were followed-up to a mean duration of 18 months to look for recurrence of fistula and other fresh complications. Results: The study included 32 patients of whom 19 had a fistula after primary surgery, 9 had undergone 2 failed surgeries and 4 had undergone 3 previous attempts at hypospadias repair. Majority (29 patients) had a single UCF. No scrotal haematoma or testicular complications were noted. Penile cosmesis was satisfactory as determined by HOSE scoring system. Erection seemed to be unaffected but could be a spurious result as majority of the patients were of pre-pubertal age. After median follow-up of 18 months, two cases had recurrence of fistula; both occurred within 1 month of the surgery. Conclusion: Tunica vaginalis interposition flap repair of urethrocutneous fistula has good outcome.

    Abstract Malignant mesothelioma of the testis is a rare condition. It usually affects the elderly. The common clinical manifestations include painless testicular mass. The tumor markers are normal. Ultrasonography helps in differentiating it from testicular tumors as these are situated at the periphery of the testis. Inguinal orchidectomy is diagnostic. Staging is usually done with abdominal computed tomography scan and chest X-ray. Role of adjuvant chemotherapy, radiotherapy, and lymph node dissection is controversial.

    Abstract Carcinoma developing at the Ureterosigmoidostomy (US) anastomosis is a complication known to us. Patients generally present late because of non specific symptoms like loin pain, loss of appetite and weight loss. Colonoscopy and abdominal imaging studies are recommended for early detection and prompt management of these malignancies. We report a case of mucin secreting adenocarcinoma of the colon, 26 years after US.

    Abstract Percutaneous Nephrolithotomy (PCNL) is a common endourologic procedure done for removal of renal stones. The most important initial step involves making a safe puncture into the pelvicalyceal system, and establishing an access tract into pelvicalyceal system. This is a crucial step in the success of the PCNL. Though the procedure is relatively simple and straight forward, in conditions like hyper-mobile kidney or ptotic kidney, the pelvicalyceal system is abnormally oriented and the kidneys are excessively mobile making the puncture of pelvicalyceal system difficult. Till date there is no established technique to overcome this problem. The technique of resolving this problem involved placing an air filled urobag beneath the patient in the prone position on the ipsilateral side of abdomen. This technique of placing an inflated urobag called as “The inflated urobag” technique is a simple one, which relocates the kidney and hence the pelvicalyceal system to normal anatomical position, making the initial puncture easier. This also prevents excessive mobility of the kidney during puncture and dilatation of the tract. This technique, which is not described earlier, is extremely useful in the puncture and dilatation during prone PCNL in ptotic and hyper-mobile kidneys. We here in describe our experience in 15 cases of PCNL in ptotic or hypermobile kidney.

    Abstract Introduction: Stress Urinary Incontinence (SUI) is a highly prevalent condition in women. This condition is often associated with negative body image and reduced quality of life. Transobturator midurethral tape surgery is currently the most favored type of management. Several questionnaires are available to assess the impact of this surgery on quality of life. Materials and Methods: We studied 32 patients who underwent Transobturator Tape (TOT) procedure for SUI. All patients were administered the Norwegian female incontinence questionnaire for urinary incontinence pre and post-operatively. We studied only the quality of life aspects in this questionnaire. Results were analysed using Wilcoxon paired rank test. Results: Thirty two patients with a mean age of 45.97±8 years (33-65 years) were studied. The usage of incontinence pads per week following surgery markedly reduced from a mean of 3.16 to 0.41. Patients more often got involved in various social activities following surgery. The mean score improved from 2.88 to 0.25. In patients who avoided places and situations without easy availability of toilet, the mean score improved from 2.97 to 0.28 post operatively. The negative impact of urinary leakage on the patients vacation, family life, social life and sleep markedly reduced from a mean of 2.75 to 1.03 (p-value <0.001). Conclusion: Almost all patients had significant improvement in all the domains studied in the quality of life aspect of Norwegian female incontinence questionnaire for urinary incontinence. TOT surgery in SUI patients not only cures the patient of urinary incontinence but also significantly improves the quality of life

    Abstract Inflammatory Myofibroblastic Tumour (IMT) or ‘pseudotumour’ of the kidney is a rare benign tumour of unknown aetiology affecting mostly young adults. A subset of IMT is neoplastic and harbours translocations of activin receptor-like kinase-1 (ALK-1) gene and can recur or rarely metastasize. Presentation varies from an incidentaloma to gross haematuria. Clinical examination and radiological investigations are usually inconclusive. Often, biopsy is inconclusive necessitating a management similar to that of Renal Cell Cancer (RCC). Diagnosis is based on immunohistochemistry. We are reporting a case of IMT in a 50-year-old male patient who presented with left flank mass which on evaluation was suggestive of left renal cell carcinoma. Excision of the tumour, histopathological examination and Immunohistochemistry proved the tumour to be IMT.

    Abstract We determined the effects of early statin treatment in acute myocardial infarction (AMI) on in-hospital morbidity and mortality. Experimental models of ischemia and reperfusion have shown that statins have early cardioprotective effects. However, the effect of statin use within the first 24 hours of admission on early morbidity and mortality in AMI has not been well studied. Data were collected on 300,823 patients who had AMI in the National Registry of Myocardial Infarction 4. In-hospital events were compared between patients who continued statin therapy received before the index AMI hospitalization (n = 17,118) or newly started statin therapy within the first 24 hours of hospitalization (n = 21,978) and patients who did not receive early statin treatment (n = 126,128) or whose statin therapy was discontinued (n = 9,411). New or continued treatment with a statin in the first 24 hours was associated with a decreased risk of mortality compared with no statin use (4.0% and 5.3% compared with 15.4% no statin). Discontinuation of statin treatment was associated with a slightly increased risk of mortality (16.5%). Early statin use was also associated with a lower incidence of cardiogenic shock, arrhythmias, cardiac arrest, rupture, but not recurrent myocardial infarction. Propensity analysis yielded mortality odds ratios of 0.46 for continued therapy, 0.42 for newly started therapy, and 1.25 for discontinued therapy for matched pairs versus no statin therapy (all p values <0.0001). In conclusion, the use of statin therapy within the first 24 hours of hospitalization for AMI is associated with a significantly lower rate of early complications and in-hospital mortality.
