    Abstract Buerger's disease is a distal segmental nonatherosclerotic vasculopathy that involves the inferior and superior limbs of smoker males younger than 45 years old. This article aims to describe a clinical case and revise the literature about Buerger's disease. A 45-year-old smoker male repeatedly visited the emergency department for refractory pain and inflammatory signs in the right hallux. After developing ulcers in the right foot, Doppler ultrasonography revealed segmental occlusion of distal arteries of that limb. It was also observed in arteriography "corkscrew" collaterals. Autoimmune, thrombophilic and cardiovascular diseases were excluded. Analgesia, antibiotics and alprostadil were implemented. As a result, the patient stopped smoking and was submitted to minor amputation with complete healing, after which he remained asymptomatic. Buerger's disease is a diagnosis of exclusion. Therefore, smoking cessation is the most effective treatment and is crucial to prevent disease progression.

    Abstract This study concentrates on the formation of fine particulate matter (PM) in a domestic pellet-fired boiler and includes a detailed characterization of the combustion process inside the combustion chamber of the boiler. Measurements of local mean gas temperatures, major gas species concentrations, and PM concentrations and size distributions are reported for a representative boiler operating condition. In addition, a number of selected PM samples were also morphologically and chemically characterized. The results revealed that (i) most particles present aerodynamic diameters below 1 μm, regardless of the measurement location, and the mass size distributions peak in the size range of 50–130 nm; (ii) early in the flame, the PM size distributions present a bimodal size distribution, but this attribute vanishes as the final stages of the combustion process are approached; (iii) soot particles are rapidly formed early in the combustion process, but these carbon-rich particles also oxidize rapidly in the near burner region; (iv) the aerodynamic diameter corresponding to the maximum amount of particles increases slightly from the fuel bed until ≈⁴/₅ of the combustion chamber height; (v) for a given measurement location and particle sizes below 1 μm, the PM chemical composition varies marginally among size ranges, indicating that the existing chemical compounds tend to form different size aggregates of similar composition; (vi) the element mass concentration varies according to the measurement location; early in the combustion process, the PM is dominated by carbon, with smaller amounts of O and Si, while in the final stages of the combustion process, the PM composition is dominated by inorganic material; and (vii) large fractions of the alkali metals are present as chlorides in the fine PM, and small fractions of the alkalis are present as sulfates.

    Abstract Mucinous adenocarcinoma of the appendix is a rare neoplasm with a low propensity for lymph node metastasis. The present case refers to an appendicular mucinous adenocarcinoma with inguinal lymph node metastasis. A 71-year-old woman underwent an appendectomy due to a clinical presentation of acute appendicitis. However, the histological examination of the surgical specimen revealed a mucinous adenocarcinoma of the appendix. After staging, the patient underwent a right hemicolectomy and was proposed for adjuvant chemotherapy. At the 3rd year of follow-up, inguinal lymphadenopathy was diagnosed, which biopsy confirmed inguinal node metastases from primary colorectal cancer, with areas of extracellular mucin. Restaging revealed liver and peritoneal metastasis, and the patient was proposed for palliative chemotherapy. Appendicular neoplasms, due to their rarity, represent a diagnostic and therapeutic challenge. This clinical case depicts an unusual metastasis pathway for an unusual neoplasm.

    Abstract Handlebar hernia is a rare entity, mainly resulting from blunt abdominal trauma with a sudden deceleration mechanism. Diagnosis of handlebar at admission may be difficult because the rupture of abdominal wall layers often is not clinically recognized in the emergency department, which requires a high degree of suspicion to identify theses lesions. It is very important to rule out the presence of intra-abdominal injuries, and in adults, surgical repair is needed. Herein, the case of an adult man who presented to the emergency department with blunt abdominal trauma caused by a motorcycle handlebar is described.

    Abstract This article concentrates on the evaluation of the potential of biomass residues, both forest and agricultural residues, for energy production and utilization in a region of Portugal (Marvão). Marvão has been identified as an excellent region for the installation of small-scale biomass plants for heating purposes given the relatively high potential of biomass residues that it presents. The assessment of the potential of the biomass residues was carried out using Geographical Information Systems (GIS) database and statistical analysis. It was concluded that the annual biomass residues potential for Marvão is about 10,600 tonnes, which corresponds to an energy production potential of about 106,000 GJ per annum. In addition, to illustrate the potential of biomass residues for energy utilization in the region of Marvão, the heating system of a hotel located in Marvão village has been analyzed as a case study. It was concluded that the conversion of the existing fossil fuel-based heating system to a biomass-based system would have economical and environmental advantages for local investors.

    Abstract Introduction: Breast cancer in young women is usually considered as breast cancer occurring in women younger than 40 years and is the most frequent cancer-related cause of death in these patients. In the past few years, there seems to be an increasing trend in the prevalence of breast cancer in young women, which, associated with poorer prognosis, more aggressive histologic features, and more frequent recurrence rates, makes it a rising threat to young women. This study aimed to evaluate the biological behavior of breast cancer in young women in our institution.
    Material and methods: A retrospective, unicentric, cohort study was conducted between 2012 and 2016. All consecutive patients with breast cancer were enrolled in the study. Cases were divided into two groups: case group, those younger than 40 years, and control group, those 40 years or older. The exclusion criterion was nonoperative treatment. Several clinical and pathologic parameters were evaluated, as well as were overall survival time and disease-free survival time.
    Results: The incidence of breast cancer in young women presented a rising tendency over the study period. Significant differences were observed in the comparison of the groups according to body mass index, age at menarche, age at birth of the first child, and proliferation rate. There were no differences in overall survival and disease-free survival rates between the groups.
    Conclusions: Young women had a more symptomatic presentation, a greater tumor proliferation rate, but similar outcomes compared with older patients. Greater multicentric studies are needed to confirm or refute these results.

    Abstract This study evaluates the combustion and emission characteristics of a domestic boiler fired with pellets of pine, industrial wood wastes and peach stones. Initially, the boiler performance, firing pine pellets, was evaluated as a function of the thermal input. Subsequently, the influence of the pellets type on boiler performance was also examined. The results reveal that the type of pellets affects significantly the boiler emissions characteristics, with the pine pellets performing significantly better than the pellets of industrial wood wastes and peach stones. The boiler thermal efficiency is not negatively affected when fired with the pellets of industrial wood wastes and peach stones. Typical operating conditions of the boiler are not suitable for the use of wood wastes and peach stones pellets as CO and HC emissions are superior. The pellets of industrial wood wastes and peach stones have, however, an attractive potential for use in domestic boilers, through the optimization of the boiler operating conditions, being this a sustainable means of energetic valorisation of residues with no specific use.

    Abstract The maturation kinetics and in vitro fertilization of immature bovine oocytes injected by the intra-follicular oocyte injection (IFOT) technique into pre-ovulatory follicles of previously synchronized cows were evaluated. In Experiment 1, grade I, II and III cumulus-oocyte complexes (COCs) were randomly distributed to one of three Groups: Matvitro22 (COCs matured in vitro for 22 h), MatFol20 and MatFol28 (COCs matured in vivo after being injected into a pre-ovulatory follicle of previously synchronized cows for 19.8 ± 0.1 h and 28.3 ± 0.1 h, respectively). Cows received 12.5 mg of LH (Lutropin, Bioniche, Canada) at the time of IFOT in the MatFol20 Group or 10 h after IFOT in the MatFol28 Group. MatFol20 and MatFol28 COCs were aspirated approximately 20 h after the LH injection for nuclear maturation kinetics and recovery rate assessment. In Experiment 2, grade I, II, and III COCs were randomly distributed into two Groups: Matvitro22 Group, COCs were matured and fertilized in vitro, and MatFol20 Group, COCs were matured as in the MatFol20 Group in Experiment 1, but COCs were fertilized in vitro. Putative zygotes were classified as fertilized, unfertilized or polyspermic. In Experiment 1, the recovery rate was lower (P < 0.001) in the MatFol20 Group (52.9%, 91/172) compared with MatFol28 (72.9%, 113/155). Rate of oocytes in germinal vesicle stage, metaphase I, anaphase I and telophase I were similar among Groups. However, oocytes matured in vivo for 28.3 h had lower rate of metaphase II (P = 0.001) and greater rates of degenerated (P = 0.001) and parthenogenetically activated (P = 0.001) oocytes. In experiment 2, the rates of polyspermy and degenerated were similar between Groups. However, the rate of fertilized oocytes was greater (P = 0.05) in oocytes in the MatFol20 Group. It is concluded that oocyte in vivo maturation for 19.8 h after IFOT does not compromise the nuclear maturation kinetics and increases in vitro fertilization rates. However, the extra 10 h of intra-follicular incubation time decreased oocyte viability.
