    Abstract Background: Obstructive chronic diseases are a continuous challenge for healthcare perfusionists all over the world. Asthma and Chronic Obstructive Pulmonary Disease (COPD) are the most frequent of these diseases. Respiratory or pulmonary rehabilitation started to gain momentum and it is more frequently used to improve muscle strength, cardiac and respiratory endurance and joint flexibility. The objective of this paper is to establish the current programs of physical activity or kinesiotherapy used in the past year and to reveal if there are any gaps or mismatches in the development of the program or in the instruments used to quantify the results of the rehabilitation programs. Materials and methods: We reviewed a total number of 12 articles, randomized control trials using the search words kinesiotherapy, physical therapy, COPD and asthma from the last year on PubMed.gov, from 11 different countries in order to establish which have similar types of exercises, parameters used in order to compare results and which were the outcomes. Results: The final results are positive, although it is hard to determine a unity because there were so many different parameters used to monitor the patients. The most frequent parameter was the 6MWT used to compare the effectiveness of the physical program in 6 studies. Discussion and conclusions: This review had its limitation in comparing the 12 articles researched because of the different number of patients, the various physical activity and rehabilitation exercises used, but especially because of the many varieties of scales and scores used to monitor the effectiveness of the treatment. In order to successfully compare such papers, an international guideline is necessary to relay specific rehabilitation programs for every type of respiratory pathology and also which are the most recommended scales or scores or parameters in general to asses such rehabilitation programs.

    Abstract Introduction Very recent in December 2017 –“The International Union against tuberculosis and lung diseases” published his position on “Heat-Not-Burn Tobacco Products”(HNB) .It is important to know this report because it’s one of the first official reactions against the new generation of tobacco products . This new “attractive” offers are now everywhere, even in Romania. IQOS is one of them with very clear objectives announced by tobacco industry. Aims The authors are trying to find out the key of the new politics of the tobacco industry in offering this type of product Material and Methods Method We are comparing what the studies of the tobacco industry are saying and what are the realities of the consequences of implementing on the market of this product. Results We are emphasising with new evidence that’s another lie and another type of trap for the potential smokers. Arguments are more evident from one month to another. Conclusions This is a new attitude and another kind of strategy of tobacco industry where we have to know more for counteracting in time.     Abstract The objective of this narrative review is to describe the complex, bi-directional relation between diabetes and COVID-19, with a special attention to quarantine period and lifestyle implications. The scope will be to draw attention through the importance of a healthy lifestyle implemented at populational level. A dramatic pandemic event raised at the beginning of 2020, COVID-19 became an important threat for health all over the world. COVID-19 occurs more frequently in patients withpoor glycaemic control, and SARS-CoV-2 infection can cause loss of glycaemic control and can even inducesevere ketoacidosis if blood glucose monitoring is inadequate and antidiabetic treatment is not adjusted if necessary. Main risk factors for COVID-19 are: age more than 65 years,comorbidities such as diabetes, hypertension, obesity, cardiovascular disease, chronic kidney, lung, liver diseases, etc. The occurrence of severe forms of COVID-19 in patients with diabetes (type 1, type 2, or gestational) may be due to the fact that over 50% of these patients have at least one of the comorbidities listed before. All patients diagnosed with COVID-19 will be screened for diabetes, as SARS-CoV-2 can destroy pancreatic β cells, and on the other hand, it can increase insulin resistance, causing new-onset diabetes.Moreover, all patients with COVID-19 will be monitored in the medium and long term, as they may develop insulin-dependent diabetes by triggering SARS-CoV-2 induced immune mechanisms. Described as ”an obesogenic period” quarantine is favourising an unhealthy lifestyle, marked by anxiety. Recognized benefits of mandatory mass quarantine must be weight against long term adverse effects mainly on cardiovascular risk burden. Social distance measures may induce stress, anxiety or even depression. Isolation, quarantine limits social interactions and may have consequences on weight both by energy intake and physical activity. In conclusion, the future will build more on healthiness, prevention and creating a healthy lifestyle.

    Abstract Pediatric sleep medicine is an area of intense research. The importance of screening for obstructive sleep apnea syndrome (OSAS) has been emphasized in the recently published international guidelines for the diagnosis and management of childhood OSA. In Romania, the fi rst steps of diagnosis and treatment of sleep disorders in children have been recently made and we wanted to assess the current awareness and management option in a group of HCPs. A self-administered questionnaire was sent by email to pediatricians, family doctors, medical doctors of other specialties, including pneumologysts, psychiatrists and ENT specialists. The results, important for the qualitative information, show that pediatric sleep disorders, mainly OSAS, are diagnosed rather by specialists than by family doctors, and that in most cases the management strategy was re-evaluation of the child and/or referral to another specialist. Half of the respondents completed the fi nal open qestion showing the need for improvement, both theoretical and practical knowledge, for a better communication between specialists and improvement of access to diagnostic tools, as well as increase of education of parents and older children and adolescents. Further research is needed to confi rm these qualitative data, however the questionnaire allowed the identifi cation of poor areas for various working groups, fi nally elaborating recommendations for various interventions which might infl uence the development of this fi eld in our country as well.

    Abstract Somnologia pediatrică reprezintă un domeniu de cercetare intensă. Importanţa screeningului pentru sindromul de apnee obstructivă în somn (SASO) a fost pusă în evidenţă în ghidurile internaţionale publicate recent cu privire la diagnosticul şi managementul SASO la copii. În România, primii paşi în diagnosticul şi tratamentul tulburărilor de somn la copii au fost făcuţi de curând, iar intenţia noastră a fost de a evalua gradul de conştientizare şi opţiunile terapeutice actuale pe un grup de profesionişti în domeniul sănătăţii. S-a trimis prin e-mail un ches tionar aplicabil prin autoadministrare, către medici pediatri, de familie şi medici de alte specialităţi inclusiv pne umologi, psihiatri şi medici ORL. Rezultatele, importante din punct de vedere calitativ, arată că tulburările de somn la copii, în principal SASO, sunt diagnosticate mai ales de medicii de diferite specialităţi şi nu de medicii de familie, şi că, în majoritatea cazurilor, strategia terapeutică a fost de reevaluare şi/sau trimitere către un alt specialist. La ultima întrebare deschisă din cadrul chestionarului jumătate dintre respondenţi au subliniat nevoia de îmbunătăţire a cunoştinţelor teoretice şi practice, a comunicării între medicii de diferite specialităţi, a acce sului la instrumentele de diagnosticare, cât şi nevoia de creştere a gradului de informare în rândul părinţilor, al copiilor de vârste mai mari şi al adolescenţilor. Sunt necesare cercetări suplimentare care să confirme aceste date calitative, însă cu toate acestea, chestionarul a permis identifi carea zonelor vulnerabile de la nivelul diferitelor grupuri de lucru, şi în fi nal elaborarea de recomandări pentru diferite tipuri de intervenţii care ar putea infl uenţa dezvoltarea acestui domeniu şi în ţara noastră.

    Abstract Chronic obstructive pulmonary disease (COPD) is a disease with increasing prevalence and burden for health systems worldwide. Every country collects its own epidemiological data regarding COPD prevalence, morbidity and mortality while taking steps to educate the population and medical community to improve early detection and treatment. The rising COPD prevalence creates a need for comprehensive guidelines. In 2012 and 2017–2018, the Romanian Society of Pneumology (SRP) organised national inquiries for COPD, while lung physicians in Romania began receiving education regarding the correct algorithms for COPD diagnosis and therapy. During 2019, a Romanian clinical guideline for diagnosis and treatment of COPD was published, and a condensed version of key points from this guideline are presented herein. COPD is diagnosed based on the presence of three major components: relevant exposure history, respiratory symptoms, and airway limitation that is not fully reversible. Clinical evaluation of patients diagnosed with COPD should include the level of symptoms, exacerbation rate, the presence of comorbidities and determination of phenotypes. The present abridged guideline is designed to be accessible and practical for assessing and managing patients with COPD. The application of up-to-date COPD guidelines may enhance the optimism of physicians and patients in managing this disease.

    Abstract Despite new epidemiological evidences, bronchiectasis remain an „orphan disease” when we refer to the number of researches that include this topic. Studies on adult population with bronchiectasis indicate that the disease begins in childhood, but symptoms may not appear until months or even years after patients have started having repeated lung infections. It is considered that an early diagnosis of bronchiectasis and an adequate treatment might improve the long-term prognosis of this disease. Chest physiotheterapy is the main form of non-pharmacological treatment. Chest physiotherapy (CPT) includes multiple techniques to improve mucus clearance in conjunction with spontaneous or directed cough.The technique and frequency of chest physiotherapy is individualised. More research is needed to confirm the place of these therapies.

    Abstract It is now possible to diagnose bronchiectasis more accurately, due to high resolution computed tomography technique. Studies on adult population with bronchiectasis indicate that the disease begins in childhood, but symptoms may not appear until months or even years after patients have started having repeated lung infections. Published data from recent studies indicate a possible similar inflammation model for bronchiectasis and neutrophilic asthma, pointing the importance of innate immune activation. It is considered that an early diagnosis of bronchiectasis and an adequate treatment might improve the long-term prognosis of this disease. Despite new epidemiological evidences, bronchiectasis remain an „orphan disease“ when we refer to the number of researches that include this topic. Data are needed for prognosis and evaluation of the effectiveness of treatments.

    Abstract Introduction Treatment with biologics for severe asthma is informed by international and national guidelines and defined by national regulating bodies, but how these drugs are used in real-life is unknown. Materials and methods The European Respiratory Society (ERS) SHARP Clinical Research Collaboration conducted a three-step survey collecting information on asthma biologics use in Europe. Five geographically distant countries defined the survey questions, focusing on seven end-points: biologics availability and financial issues, prescription and administration modalities, inclusion criteria, continuation criteria, switching biologics, combining biologics and evaluation of corticosteroid toxicity. The survey was then sent to SHARP National Leads of 28 European countries. Finally, selected questions were submitted to a broad group of 263 asthma experts identified by national societies. Results Availability of biologics varied between countries, with 17 out of 28 countries having all five existing biologics. Authorised prescribers (pulmonologists and other specialists) also differed. In-hospital administration was the preferred deliverance modality. While exacerbation rate was used as an inclusion criterion in all countries, forced expiratory volume in 1 s was used in 46%. Blood eosinophils were an inclusion criterion in all countries for interleukin-5 (IL-5)-targeted and IL-4/IL-13-targeted biologics, with varying thresholds. There were no formally established criteria for continuing biologics. Reduction in exacerbations represented the most important benchmark, followed by improvement in asthma control and quality of life. Only 73% (191 out of 263) of surveyed clinicians assessed their patients for corticosteroid-induced toxicity. Conclusion Our study reveals important heterogeneity in the use of asthma biologics across Europe. To what extent this impacts on clinical outcomes relevant to patients and healthcare services needs further investigation.

    Abstract Background Vaccination is vital for achieving population immunity to severe acute respiratory syndrome coronavirus 2, but vaccination hesitancy presents a threat to achieving widespread immunity. Vaccine acceptance in chronic potentially immunosuppressed patients is largely unclear, especially in patients with asthma. The aim of this study was to investigate the vaccination experience in people with severe asthma. Methods Questionnaires about vaccination beliefs (including the Vaccination Attitudes Examination (VAX) scale, a measure of vaccination hesitancy-related beliefs), vaccination side-effects, asthma control and overall safety perceptions following coronavirus disease 2019 (COVID-19) vaccination were sent to patients with severe asthma in 12 European countries between May and June 2021. Results 660 participants returned completed questionnaires (87.4% response rate). Of these, 88% stated that they had been, or intended to be, vaccinated, 9.5% were undecided/hesitant and 3% had refused vaccination. Patients who hesitated or refused vaccination had more negative beliefs towards vaccination. Most patients reported mild (48.2%) or no side-effects (43.8%). Patients reporting severe side-effects (5.7%) had more negative beliefs. Most patients (88.8%) reported no change in asthma symptoms after vaccination, while 2.4% reported an improvement, 5.3% a slight deterioration and 1.2% a considerable deterioration. Almost all vaccinated (98%) patients would recommend vaccination to other severe asthma patients. Conclusions Uptake of vaccination in patients with severe asthma in Europe was high, with a small minority refusing vaccination. Beliefs predicted vaccination behaviour and side-effects. Vaccination had little impact on asthma control. Our findings in people with severe asthma support the broad message that COVID-19 vaccination is safe and well tolerated.
