    Abstract - Luminal B (HER2 negative) subtype is the most diversiform type of breast cancers, with a high Ki-67 proliferation index (>20%) or/and low progesterone (PR; <20%) with various intensity and distribution of hormone receptors. Considerable difference has also been noticed in disease outcome, wherefore there is the need for a more detailed classification of this tumor subtype. The clinical and pathologic parameters of 147 luminal B (HER2 negative) breast cancers were examined. The expression of hormone receptors in correlation with other prognostic factors and disease outcome was analyzed by Kaplan-Meier curves and multivariate Cox regression analysis. The Kaplan-Mayer analysis showed that low positivity of estrogen (ER) and PR receptors in tumors was associated with a significantly worse disease outcome (overall survival (ER), p=0.020; disease free survival (ER), p=0.019; overall survival (PR), p=0.026; disease free survival (PR), p=0.038)), unlike Ki-67, which did not show a statistically significant connection (overall survival, p=0.343; disease free survival, p=0.322). The intensity of receptor staining and Ki-67 relative to other histopathologic prognostic factors showed a statistically significant correlation solely with histologic grade of tumor. By using the Cox regression model, PR proved to be an independent prognostic factor for overall survival (p=0.004) and disease free survival (p=0.029). The luminal B (HER2 negative) breast cancer with low expression of hormone receptors, independent of the Ki-67 proliferation index, and in correlation with a higher histologic grade, could be a unique subtype of cancer.

    Abstract Background: Modified Lapidus arthrodesis (MLA) is a well-established treatment modality for hallux valgus deformities (HVD) associated with instability of the first ray. Although the three-dimensional (3D) nature of HVD has long been recognized, diagnostics still focus on plain radiographs. The objective of this study was to validate 3D Cone Beam CT (CBCT) in the perioperative assessment of HVD with focus on the alignment of the forefoot.
    Methods: In a prospective clinical study, MLA was performed on 30 patients (25 females, 5 males; mean age: 63.2 years). Pre- and postoperatively standard radiographs and CBCT with full weight-bearing were acquired. For the CBCT based assessment, reproducible criteria have been defined, measured, and correlated with established radiological indicators.
    Results: Evaluation of standard radiographic parameters (hallux-valgus angle [HVA], intermetatarsal angle 1-2 [IMA 1-2], distal metatarsal articular angle [DMAA], tibial sesamoid position [TSP]) showed significant improvement postoperatively. Comparison of measurements obtained from plain radiographs and CBCT were significantly correlated between both measuring techniques, indicating high reliability. Pronation of the first metatarsal and the sesamoids were significantly reduced by the procedure. Due to this repositioning effect, the second metatarsal head was elevated by 3.1mm, and the lateral sesamoid was lowered by 3.8mm. However, there was no correlation between the amount of pronation and conventional radiographic measures.
    Conclusions: Compared to plain radiographs, CBCT allows a more detailed view of the forefoot alignment in the coronal plain after MLA. MLA was able to recenter the sesamoids under der first metatarsal head and conversely led to elevation of the second metatarsal head.

    Abstract Purpose: To evaluate the diagnostic usefulness of myocardial Metaiodbenzylguanidin SPECT (MIBG-SPECT) in patients with different parkinsonian syndromes (PS).
    Background: The cardiac uptake of MIBG-SPECT is reduced in idiopathic Parkinson's disease (IPD), Lewy Body dementia (LBD) and idiopathic REM sleep behaviour disorder (iRBD). It is normal in PS related to tauopathies and in other dementia disorders.
    Methods: The concordance of MIBG diagnosis (MD) and clinical diagnosis (CD) was evaluated in different clinical set-ups. CD was based on established guidelines and reassessed at the last follow up visit. MD was based on the delayed heart/mediastinum (HM) ratio. Based on literature a unique cut off of 1.6 was chosen, with values below considered to be abnormal. The following clinical set-ups were retrospectively analyzed. Set-up A: differentiation between IPD and other PS or essential tremor. Set-up B: differentiation between LBD and other dementia disorders. Set-up C: prognostic evaluation of iRBD. We analyzed 50 consecutive patients: 24 men and 26 women; mean age 65; range 38–87. All patients were maintained on their antiparkinsonian or antihypertensive medications. The follow-up period after the SPECT exam was 2.72 years (range: 0.16–13)
    Results: In set-up A 38 patients were analyzed; in set-up B 9 patients and in set-up C 2 patients. In set-up A diagnostic concordance was 64%. MIBG-SPECT suggested IPD in further 26% of patients, not confirmed by clinical follow-ups. In set-up 2 the concordance rate was 88%. In set-up 3, MIBG suggested possible progression to PD/LBD in both patients included.
    Conclusion: In clinical set-ups, myocardiac MIBG-scintigraphy is of moderate diagnostic accuracy. False positive diagnostic results are possibly due to not yet recognized age-related, but PS-independent postsynaptic noradrenergic degeneration or reduced uptake caused by antihypertensive co-medications. Further investigations should include larger cohorts of presumably healthy elderly subjects and separately address the individual impact of currently used antihypertensive co-medications sustained and complete withdrawal of these medications, as presently suggested, is risky and thus hard to realize in ambulatory patients.

    Abstract Meta-iodbenzylguanidine scintigraphy (MIBG scintigraphy) shows reduced uptake in idiopathic Parkinson's disease (IPD), idiopathic REM sleep behavior disorder (IRBD) and Lewy body dementia (LBD), but not in other parkinsonian or dementia syndromes. We retrospectively reevaluated 50 patients. Concordance rate between last clinical diagnosis and scintigraphy diagnosis was only given in two-thirds of the patients. Confounding factors were: decreasing heart/mediastinum ratio (HMR) with progressive age, higher HMR in women and possibly interference with antihypertensive medication. Standardization of the methods and precise clinical guidelines are warranted for better clinical use.
