    Abstract Objective: Therapeutic ultrasound remains a highly discussed topic in physical therapy due to uncertainty between treatment regimens and biological benefits. Its impact on aged populations, who are vulnerable to insufficient healing after muscle injury because of sarcopenia, is understudied. Despite the coupling between muscle inflammation and regeneration, research on the immune response after therapeutic ultrasound is limited. The objective of this study was to evaluate structure, inflammatory cytokine signaling and immune cell infiltration after therapeutic ultrasound in young and aging murine muscle.
    Methods: Young (6-week-old) and Adult (52-week-old) male and female mouse non-injured gastrocnemii were treated with either low-intensity pulsed ultrasound at 2 W/cm
    Results: Low-intensity pulsed ultrasound induced an early, transient inflammatory response where interleukin (IL)-15 and macrophages (M2 > M1) were increased 1 hour post-sonication. High-intensity pulsed focused ultrasound caused a late, extended immune response where monocyte chemoattractant protein 1 (MCP-1), neutrophils, monocytes and macrophages (M1 > M2) were increased 24 hours post-sonication. Notably, these changes manifested solely in Young gastrocnemius. The Adult gastrocnemius exhibited decreased cytokine expression (IL-1α, IL-6, IL-15, macrophage colony-stimulating factor [M-CSF]) and no alteration in immune cell recruitment post-sonication. There was no damage to muscle structure.
    Conclusion: Therapeutic ultrasound induced a pressure-dependent inflammatory response that can augment or mitigate intrinsic muscle cytokine signaling and cell recruitment in adolescent or aged muscle, respectively.

    Abstract Sound estimates of drought characteristics are very important for planning intervention measures in drought-prone areas. Due to data scarcity, many studies are increasingly using less data-intensive approaches, such as the evapotranspiration deficit index (ETDI), in estimations of agricultural droughts. However, little is known about the sensitivity of this specific ETDI formula to its parameters, and to data at different temporal scales. In this study, a general ETDI formula, homologous to the specific ETDI formula, was introduced and used to test the sensitivity of the ETDI to its parameters and to data at different temporal scales. The tests used time series of remotely sensed evapotranspiration data in the Ruvu River basin in Tanzania. The parameter sensitivity tests revealed that ETDI is sensitive to its parameters, and different parameter combinations resulted in different drought characteristics. The temporal scale sensitivity test showed that drought characteristics, such as the number of drought events and the total drought durations, decreased as the temporal scale increased. Thus, an inappropriate temporal scale may lead to the misrepresentation of drought characteristics. To reduce uncertainty and increase the accuracy of ETDI-based agricultural drought characteristics, ETDI requires parameter calibration and the use of data with small temporal scales, respectively. These findings are useful for improving estimations of ETDI-based agricultural droughts.

    Abstract Many impact assessment studies have been using reanalysis precipitation data as an alternative to observed rainfall data even where few rainfall records are available. For that reason, this study investigated fit-to-observations and fit-for-purpose of hybrid rainfall data using four strategically located rainfall stations in the Wami-Ruvu basin, Tanzania. Hybrid rainfall data were generated by randomly filling 15% to 70% hypothetical missing data in the observed records at the rainfall stations with reanalysis precipitation data from nearby grids. Fit-to-observations was used to evaluate the performance of the whole time series of hybrid rainfall data in mimicking that of observed rainfall data at the stations. Fit-for-purpose was used to evaluate the performance of trend and seasonal components of the whole time series of hybrid rainfall data in mimicking those of the whole time series of observed rainfall data at the stations. The findings showed that hybrid rainfall data are superior to reanalysis precipitation data in mimicking observed rainfall data. The findings also revealed that in most cases, trend and seasonal components have greater performances than the whole time series at high degrees of hybridization. Therefore, hybridization of rainfall data is highly encouraged especially in data-scarce regions.

    Abstract Study region: The Wami River basin in east-central Tanzania features the Eastern Arc Mountains and dichotomous climate regimes. Study focus: Drought characteristics are vital in planning and managing water resources in river basins. However, the estimation of drought characteristics from the evapotranspiration deficit index (ETDI) is susceptible to the uncertainty associated with alternative datasets from hydrological models and remote sensing images. Thus, this study investigated scenario uncertainty in ETDI-based drought characteristics in the river basin using evapotranspiration and potential evapotranspiration data from a classical Soil and Water Assessment Tool (SWAT) and the Moderate Resolution Imaging Spectroradiometer (MODIS) images between the years 2000 and 2010. New hydrological insights: The findings showed a substantial scenario uncertainty in drought duration, severity, and intensity at various locations in the river basin. Localized severe MODIS-based overall drought intensity (MODIS-ODI) in the middle of the basin was attributed to prolonged low rainfall amounts. In the downstream, localized severe SWAT-based overall drought intensity (SWAT-ODI) was attributed to an imperfection of the SWAT model in simulating the true hydrologic cycle of the river basin. However, at the basin level, both SWAT-ODI and MODIS-ODI between the years 2000 and 2010 were moderate. These findings can be used as the baseline information in improving drought estimation in the river basin. Moreover, this study also demonstrates the importance of assessing scenario uncertainty in drought characterization.

    Abstract Dermatology in the United States is among the global leaders in advancing scientific discovery and practicing evidence-based medicine; however, many patients face limited access to dermatologic care due to a national shortage of dermatologists, provider clustering, and the interplay of sociodemographic and socioeconomic factors. These intertwined barriers have created a landscape where race, ethnicity, insurance subtype, education, and socioeconomic status negatively impact access to dermatologic services. Leveraging the utilization of technology and community engagement provides inroads to improve access to care. We outline how teleconsultations, telementoring, specialty care extension for community health outcomes (Project ECHO), community health centers, donated specialty care models, and community health workers can be bolstered to create more equitable access to care.

    Abstract En el quechua Ayacucho-Chanca, los términos corpóreos evocan distintos dominios o marcos conceptuales. El objetivo de este trabajo consiste en esclarecer la naturaleza del comportamiento polisémico de dichos términos corpóreos. Teóricamente se parte de un conjunto de principios que rigen el fenómeno de la polisemia en el marco de la lingüística cognitiva. Metodológicamente, la data ha sido obtenida a partir de un proceso de elicitación. Los resultados permiten reafirmar la metáfora y la metonimia como procedimientos necesarios, pero (a la vez) insuficientes para explicar la generación de nuevos sentidos. La explicación requiere también de un proceso interaccional.
