    Abstract Coronary artery bypass graft (CABG) surgery has a major role in the management of obstructive coronary artery disease, especially in patients with diabetes or multiple vessel disease. Currently, in the USA, the annual incidence rate of CABG has been reported to be approximately 400,000. Overall, gastrointestinal (GI) complications occur in less than 2% of patients undergoing open-heart surgery. Acute colonic pseudo-obstruction, also known as Ogilvie's syndrome, is a disorder characterized by dilatation of the colon in the absence of an anatomic lesion that obstructs the flow of intestinal contents. This condition occurs in 0.06% of patients following cardiac surgery, and in CABG patients, the reported incidence is approximately 0.046%. In this report, we discuss a case of a patient who developed Ogilvie's syndrome after undergoing CABG.

    Abstract The best results in surgical resection are obtained in patients with solitary tumors without clinically significant portal hypertension (hepatic venous pressure gradient >10 mmHg). In such settings, 5-year survival rates exceed 70%. When portal hypertension exceeds this cut-off value, 5-year survival decreases to 55%, as is also the case in patients with more than one nodule. Surgery may be technically feasible, in other words, with acceptable 30-day mortalities although the clinically relevant survival outcome is significantly reduced. In such instances, patients may be better served by liver transplantation. If this option is not available, the outcome may not differ to that obtained by ablation for small solitary hepatocellular carcinoma or for chemoembolization for those patients with multifocal hepatocellular carcinoma within the Milan criteria. This philosophy is the backbone for the Barcelona Clinic Liver Cancer decision-making process.

    Abstract In team sports, load management has become one of the most common areas of investigation, given that effective control over load is the key to being able to optimize performance and avoid injuries. Despite the constant evolution and innovation in the latest theories, we can see a clear tendency in load management that focuses on physiological and mechanical aspects and neglects its cognitive character, which generates the variability inherent in the performance of athletes in a changing environment. Indicators of response that inform methods of control over cognitive load can include cognitive, physiological and behavioral indicators. However, limited investigations exist to support the reliability of each indicator regarding cognitive load. For this reason, the objective of this literature review is to present strategies used to manage cognitive load in team sports, as well as the indicators utilized for such a proposition and their relationships in specific contexts.

    Abstract Introduction: Previous studies suggest that co-fluctuations in neural activity within V1 (measured with fMRI) carry information about observed stimuli, potentially reflecting various cognitive mechanisms. This study explores the neural sources shaping this information by using different fMRI preprocessing methods. The common response to stimuli shared by all individuals can be emphasized by using inter-subject correlations or de-emphasized by deconvolving the fMRI with hemodynamic response functions (HRFs) before calculating the correlations. The latter approach shifts the balance towards participant-idiosyncratic activity.
    Methods: Here, we used multivariate pattern analysis of intra-V1 correlation matrices to predict the Level or Shape of observed Navon letters employing the types of correlations described above. We assessed accuracy in inter-subject prediction of specific conjunctions of properties, and attempted intra-subject cross-classification of stimulus properties (i.e., prediction of one feature despite changes in the other). Weight maps from successful classifiers were projected onto the visual field. A control experiment investigated eye-movement patterns during stimuli presentation.
    Results: All inter-subject classifiers accurately predicted the Level and Shape of specific observed stimuli. However, successful intra-subject cross-classification was achieved only for stimulus Level, but not Shape, regardless of preprocessing scheme. Weight maps for successful Level classification differed between inter-subject correlations and deconvolved correlations. The latter revealed asymmetries in visual field link strength that corresponded to known perceptual asymmetries. Post-hoc measurement of eyeball fMRI signals did not find differences in gaze between stimulus conditions, and a control experiment (with derived simulations) also suggested that eye movements do not explain the stimulus-related changes in V1 topology.
    Discussion: Our findings indicate that both inter-subject common responses and participant-specific activity contribute to the information in intra-V1 co-fluctuations, albeit through distinct sub-networks. Deconvolution, that enhances subject-specific activity, highlighted interhemispheric links for Global stimuli. Further exploration of intra-V1 networks promises insights into the neural basis of attention and perceptual organization.

    Abstract Milan criteria are currently the benchmark related to liver transplantation (LT) for hepatocellular carcinoma. However, several groups have proposed different expanded criteria with acceptable results. In this article, we review the current status of LT beyond the Milan criteria in three different scenarios-expanded criteria with cadaveric LT, downstaging to Milan criteria before LT, and expansion in the context of adult living donor LT. The review focuses on three main questions: what would the impact of the expansion beyond Milan criteria be on the patients on the waiting list; whether the dichotomous criteria (yes/no) currently used are appropriate for LT or continuous survival estimations, such as the one of "Metroticket" and whether it should enter into the clinical practice; and, whether the use of living donor LT in the context of expansion beyond Milan criteria is justified.

    Abstract Background: Multiple treatment options at glioblastoma progression exist, including reintervention, reirradiation, additional systemic therapy, and novel strategies. No alternative has been proven to be superior in terms of postprogression survival (PPS). A second surgery has shown conflicting evidence in the literature regarding its prognostic impact, possibly affected by selection bias, and might benefit a sparse subset of patients with recurrent glioblastoma. The present study aims to determine the prognostic influence of salvage procedures in a cohort of patients treated in the same institution over 15 years.
    Methods: Three hundred and fifty patients with confirmed primary glioblastoma diagnosed and treated between 2005 and 2019 were selected. To examine the role of reoperation, we intended to create comparable groups, previously excluding all diagnostic biopsies and patients who were not actively treated after the first surgery or at disease progression. Uni- and multivariate Cox proportional hazards regression models were employed, considering reintervention as a time-fixed or time-dependent covariate. The endpoints of the study were overall survival (OS) and PPS.
    Results: At progression, 33 patients received a second surgery and 84 were treated with chemotherapy only. Clinical variables were similar among groups. OS, but not PPS, was superior in the reintervention group. Treatment modality had no impact in our multivariate Cox regression models considering OS or PPS as the endpoint.
    Conclusions: The association of reoperation with improved prognosis in recurrent glioblastoma is unclear and may be influenced by selection bias. Regardless of our selective indications and high gross total resection rates in second procedures, we could not observe a survival advantage.

    Abstract We present an intriguing and rare case of a 71-year-old male who presented with a non-ST elevation myocardial infarction (NSTEMI). Initial coronary angiography revealed severe and unusual systolic extrinsic compression of the left main coronary artery (LM), warranting further advanced imaging investigations. Computed tomography angiography (CTA) and transesophageal echocardiography (TEE) were employed to determine the underlying cause, which was identified as a contained aortic rupture leading to the formation of a pseudoaneurysm in the left coronary sinus of Valsalva and aortic root. This condition was found to be a sequela of previously undiagnosed endocarditis, likely secondary to lower extremity osteomyelitis and bacteremia, for which the patient received prolonged intravenous (IV) antibiotic therapy. This case highlights the critical role advanced imaging techniques play in accurately diagnosing and characterizing complex cardiovascular abnormalities, enabling early intervention and optimizing patient outcomes. Healthcare providers should remain vigilant for such atypical presentations to ensure timely and appropriate management.
