    Abstract Major earthquakes can cause crush injuries. Damage to buildings is a major cause of death and injury in earthquakes, especially when solid building materials such as concrete are used. The most common injuries following earthquakes are fractures, compartment syndromes, major soft tissue injuries, and crush injuries. This highlights the importance of planning at the national level before natural disasters occur. The aim of this study was to review what needs to be done in orthopedic and trauma services in the field, during transfer, and in advanced treatment centers after an earthquake disaster in our country, where high-intensity earthquakes occur. In conclusion, strategic planning in the field, during transfer, and in advanced treatment centers can reduce the pressure on the health system in similar mass disasters.

    Abstract INTRODUCTION: We aimed to evaluate surgical treatment results of hand and wrist ganglions those seen frequently in clinical practice. METHODS: Patients who were diagnosed with ganglion cysts clinically and radiologically in the hand and wrist region and treated with surgical excision between 2011 and 2014 were retrospectively analyzed. A total of 102 patients, 69 (67.6%) female and 33 (32.4%) male, who had a regular follow-up period of 12 months, had no additional injuries in the hand and wrist region, were included in the study. All cases were evaluated in terms of age, gender, region of ganglion cyst, treatment method and local recurrence. RESULTS: Mean age was 37.73+-9.5 (20-61) years. Local recurrence was seen in 5 (.3) patients whom have volar ganglion cyst and 1 (.7) in dorsal localized. There was no significant difference in treatment of localization of cysts (p>0.05). In addition, there was significant difference in presence of recurrence (p: 0.005; p<0.05). There was .8 local recurrence in volar cysts and it is higher than other localizations, this finding was statistically significant (p<0.05). There wasn't statistically significant difference in ratio of local recurrence (p>0.05). DISCUSSION AND CONCLUSION: The reason of high incidence in local recurrence in volar ganglion cysts can be insufficient excision of sac and pedicle of ganglion cyst because of anatomical relations in this region. It can be assumed that, if the surgeon has a high knowledge of anatomy of this area and develop his/her experience in this surgery local recurrence incidence can be decreased.

    Abstract Purpose: The purpose of this report was to present the case of a patient with bleeding diathesis on whom we performed ankle arthroplasty for the first time. Materials and methods: A 29-year old male patient with bleeding diathesis, who had been treated and followed up over a long period, underwent ankle arthroplasty because of osteoarthritis of the ankle. The patient was prepared for surgery by the haematology department in accordance with the guidelines for surgical preparation, diagnosis and management of haemophilia. After ankle arthroplasty was performed, the preoperative and two-year postoperative clinical outcomes were evaluated using a subjective foot score and Maryland foot score. Results: Preoperative factor VIII levels were raised to the target level of haemostasis prior to the surgical procedure. The factor VIII levels were maintained within the normal range up to postoperative day 14. The subjective foot score of the patient was 40 preoperatively and 85 postoperatively, whereas their Maryland foot scores were 33 preoperatively and 90 postoperatively. Both scoring systems indicate an excellent clinical outcome. Conclusion: As observed, the early and mid-term functional outcomes are promising for patients with bleeding diathesis who undergo ankle arthroplasty.

    Abstract ÖZETAmaç: Bu çalışmada çocukluk çağı femur kırıklarında dolaylı redüksiyon ve kilitli plak uygulamalarının sonuçlarını değerlendirmek.Materyal ve Metod: Bu prospektif çalışmaya izole femur kırığı olan 17 (7 kız 10 erkek) hasta dahil edildi. Çalışmaya alınan femur kırıkları, uzun spiral, oblik ve uzunluk instabil olan femur diafiz kırıkları idi. Çalışmaya alınan hastaların yaş ortalaması 6,2 (5-12) yıl idi.Hastaların hepsine dolaylı redüksiyon ve kilitli plak uygulandı. Hastaların radyolojik sonuçları standart olarak çekilen ön arka ve yan grafiler ile değerlendirildi. Hastalarda kaynama sonrası çekilen grafide 10 dereceden fazla olan angulasyonlar deformite olarak belirlendi.Bulgular: Hastaların ortalama takip süreleri 10 (3-17) ay ve hastanede kalma süreleri ortalama 4,3 (2-6 ) gün idi. Hastaların tamamına az yüzey temaslı kilitli titanyum plaklar kullanıldı. Hastalara kullanılan ortalama plak uzunluğu 10 (8-12) delikli idi. Plak ile beraber ortalama kullanılan vida sayısı 6,7 (6-9) adet idi. Vidaların kırık hattının proksimal ve distal olarak dağılımları, proksimal ortalama 3,2 (3-4) ve distal ortalama 3,4 (3-5) adet olarak belirlendi. Hastaların kaynama zamanı ortalama 5,4 (4-7) hafta idi. Hastaların hepsinde tam kaynama gerçekleşti ve komplikasyon görülmedi. Hastaların dört tanesinden implant çıkarımı yapıldı.Sonuç: Çalışmamızın sonucunda 5 yaş ile iskelet olgunlaşmasına kadarki süreçte oluşan uzunluk instabil femur kırıklarında dolaylı redüksiyon ile kilitli plak uygulaması ile başarılı sonuçlar elde edilmektedir. Düşük komplikasyon oranı ve yüksek kaynama başarısı ile titanyum elastik çivisi yerine kullanılabilecek alternatif bir yöntemdir. Erken mobilizasyon ve kısa süreli hastane yatışı ile yüksek oranda aile uyumu sağlanabilmektedir.Anahtar Kelimeler: Femur kırığı, kilitli plak.

    Abstract Purpose: To determine whether or not there were any differences in the measurement techniques used by orthopedic and radiology specialists in the evaluation of magnetic resonance (MR) images for the diagnosis of patella alta in adolescents. Materials and Methods: Evaluations were performed by three orthopedic specialists (Group I) and three radiology specialists (Group II) regarding the presence of patella alta in 40 adolescents cases using the Insall–Salvati, Caton–Deschamps, Blackburne–Peel, and modified Insall–Salvati indices on MR images obtained to diagnose patellar instability. Results: The Fleiss Kappa conformity levels for Insall–Salvati, Caton–Deschamps, Blackburne–Peel, and modified Insall–Salvati measurements were 0.531, 0.559, 0.246, and 0.272, respectively, in Group I, and 0.699, 0.346, 0.516, and 0.394, respectively, in Group II. Conclusion: The radiology specialists were found to have greater conformity in the evaluation of all patella alta indices, which was probably due to their greater familiarity with radiological measurements than that of the orthopedic specialists.

    Abstract Background. The aim of the study was to evaluate whether or not there was any incompatibility between four-strand hamstring tendons taken from the same knee and the dimensions of the ACL and PCL. Methods. 15 fresh frozen cadaver hamstrings were prepared as four-strand grafts and measurements made of the ACL and PCL circumferences in the midsection were made in the narrowest part of the midsection. The cross-section areas and diameters were calculated with geometric calculations used to measure the cross-sectional area of cylinders. Accepting that the geometric insertions were elliptical, the length, width, and area were calculated for entry areas. Results. A significant relationship at 96.2% was determined between the ACL mid and the hamstring diameter. A significant relationship at 96.7% was determined between the ACL and the hamstring mid area. A significant relationship at 96.4% was determined between the PCL mid and the hamstring diameter. A significant relationship at 95.7% was determined between the PCL and the hamstring mid area. Conclusion. For the reconstruction of ACL and PCL, it was determined that there is less incompatibility between the four-strand hamstring tendons taken from the same knee and the dimensions of the midsection PCL compared to the ACL dimensions.

    Abstract The purpose of this study was to evaluateour result in patients with congenital clubfoot deformitywho had been treated with Ponseti’s technique.Materials and methods: The study included 28 patients(18 males, 10 females, mean age 2.2 months; range 6days to 5 months). All patients had Dimeglio grade II, III,and IV deformities. All patients had manipulation andcasting according to the Ponseti’s technique. Achillestenotomy was performed in the persistent equines.Maintenance of correction was obtained with the use offoot abduction orthosis. Mean follow-up period was 17months (range 6-36 months).Results: Forty-one feet of 28 patients were treated accordingto the Ponseti’s technique. Achilles tenotomywas performed in 31 feet (81%). Posterior release wasnot performed in any patient. Although, correction wasmaintained in all feet at the beginning, 14 feet (31%)were relapsed. The relapses were due to decreased complianceto foot abduction orthoses.Conclusion: Ponseti technique is an effective, and reliablein correction of idiopatic congenital clubfoot deformity.The results also suggest that the role of familyeducation to enhance compliance to brace applicationand experienced orthotist are important to prevent relapses.J Clin Exp Invest 2011; 2(1):30-34
