    Abstract Campylobacter infection usually starts in the jejunum and ileum and progresses distally. The case fatality rate is low and most occur in elderly or patients with comorbidity as in this case. Antibiotics should be used in severe cases or patients at risk. The choices are macrolides and fluoroquinolones. However, in some countries quinolone resistance is increasing, as in Spain. We shouldn´t forget this fact for the proper treatment approach and specifically in refractory cases.

    Abstract Introducción: La disección submucosa endoscópica (DSE) se usa en el tratamiento de lesiones neoplásicas precoces. El modelo de entrenamiento animal es útil para aprender la técnica y el manejo endoscópico de complicaciones como la perforación.
    Objetivo: Analizar el éxito y seguridad del cierre endoscópico de perforaciones en modelo animal de supervivencia tras DSE.
    Material y Métodos: Se realizó DSE en modelo porcino vivo (sus scrofa) en diferentes localizaciones (esófago, estómago, sigma-recto) utilizando gastroscopios modelos GIF V2/GIF Q145 (Olympus) y diversos modelos de bisturí: IT-knife 2® y Hook-knife®(Olympus); Flush-knife 1.5 BT® (Fujifilm). Los casos con perforación voluntaria o involuntaria fueron tratados endoscópicamente con clips (Resolution Clip®, Boston Scientific; QuickClip®, Olympus) y adminstración profiláctico de antibiótico (ceftriaxona 500 mg iv/im).
    Resultados: Entre Marzo de 2011 y Julio de 2013 se completaron 16 DSE en 11 animales (peso medio 26,3 Kg) donde hubo al menos 1 foco de perforación. La tasa de resección en bloque fue del 75% y el tiempo medio 87 min (rango 5 – 205). Los casos por localización fueron: Estómago 8 (50%) (4 antro, 3 cuerpo, 1 fundus), Sigma-Recto 7, Esófago 1. Se utilizaron una media de 1,9 clips por cierre, con un éxito inicial del 93% (14/15 intentos). En 2 casos (12%), ambos con perforación gástrica, se produjo muerte precoz: neumoperitoneo a tensión (muerte intraoperatoria) y peritonitis grave a las 72 horas (eutanasia). En los restantes casos no hubo incidencias, realizando sacrificio programado tras seguimiento medio de 22,7 días (3 – 49).
    Conclusiones: El modelo porcino vivo facilita el aprendizaje del cierre de perforaciones asociadas a DSE, con un alto éxito técnico inicial y excelente curso clínico, incluyendo localizaciones no favorables (esófago y colon-recto). El retraso en el cierre o el desarrollo de peritonitis asociada pueden conducir a complicaciones fatales, incluso en localizaciones teóricamente favorables (estómago).

    Abstract The objective of the study was to assess the efficacy of antiviral therapy in patients with hepatitis C virus (HCV) recurrence after liver transplantation (OLT). We included 30 patients of mean age 56 years, who experienced HCV recurrence after OLT. Mean time from OLT to the beginning of therapy was 57 months (median: 43 months). All of them were on monotherapy: tacrolimus (n = 21), cyclosporine (n = 6), and mycophenolate mofetil (n = 3). Fourteen had previously been diagnosed with allograft HCV cirrhosis. Patients were treated with peginterferon alpha 2b (1.5 mug/kg/weekly SC) and ribavirin (10.6 mg/kg/d) for 48 (genotypes 1, 4) or 24 weeks (genotypes 2, 3). After a mean follow-up of 20 months, two patients had died due to biliary sepsis (while on therapy) and acute myocardial infarction (7 months after the end of therapy). End of treatment virological response was achieved in 19 patients (63.3%) and sustained virological response (SUR) in 14 (46.7%). Comparing cirrhotic and noncirrhotic patients, SVR was achieved in seven patients in both groups (50% vs 43.8%; P = .732). Every patient had some adverse event; in 11 patients (36.7%) it was withdrawn (seven cirrhotic and four noncirrhotic; P < .05), and in 12 the starting dose was decreased (40%). There were neither rejection episodes nor cirrhotic complications during therapy, but infections were more common in cirrhotic patients (57% vs 25%; P < .05). In HCV cirrhotic transplanted patients the sustained virological response to combined antiviral therapy was similar to that in noncirrhotic patients, but severe adverse events including infections were much more common.

    Titelübersetzung Liver transplant in fibrolamellar hepatocarcinoma: apropos of a case. Review of the literature.
    Abstract A case is presented of fibrolamellar hepatocarcinoma in a 12 year-old male treated by liver transplantation; there is no evidence of tumoral recurrence at 28 months. Donos and receptor were ABO incompatible. The immunosuppressive regimen used was cyclosporine A and low doses of steroids. Fibrolamellar hepatocarcinoma is an infrequent histologic variety that usually affects young people and is generally not associated with hepatitis B infection or cirrhosis. It is often a single tumor, is more susceptible to surgical resection than other varieties of hepatocarcinoma, and is characterized by a relatively unagressive tumoral biology.
