    Abstract Current research on the optimal amount of oxygen delivery during general anesthesia is controversial. It is well described that anesthesia providers create supraphysiological arterial oxygen levels in patients with the delivery of supplemental oxygen. Over the past 20 years, hyperoxia has been studied more thoroughly and potential consequences are better understood. Atelectasis and radical oxygen species can lead to postoperative complications such as infection, prolonged respiratory support, and increased length of hospital stay. Anesthetists should consider the risk and benefit, potential effects, and differentiate the amount of oxygen dependent on the clinical situation. More research is needed in varying patient populations and surgical procedures to better understand the implications of hyperoxia in patients requiring general anesthesia.

    Abstract Entangled twin-beams generated by parametric down-conversion are among the favorite sources for imaging-oriented applications, due their multimodal nature in space and time. However, a satisfactory theoretical description is still lacking. In this work we propose a semi-analytic model which aims to bridge the gap between time-consuming numerical simulations and the unrealistic plane-wave pump theory. The model is used to study the quantum correlation and the coherence in the angle-frequency domain of the parametric emission, and demonstrates a [Formula: see text] growth of their size as the gain g increases, with a corresponding contraction of the space-time distribution. These predictions are systematically compared with the results of stochastic numerical simulations, performed in the Wigner representation, of the full model equations: an excellent agreement is shown even for parameters well outside the expected limit of validity of the model.

    Abstract The study of the electroencephalogram signals recorded from subjects during an experience is a way to understand the brain processes that underlie their physical and emotional involvement. Such signals have the form of time series, and their analysis could benefit from applying techniques that are specific to this kind of data. Neuroaesthetics, as defined by Zeki in 1999, is the scientific approach to the study of aesthetic perceptions of art, music, or any other experience that can give rise to aesthetic judgments, such as liking or disliking a painting. Starting from a proprietary dataset of 248 trials from 16 subjects exposed to art paintings, using a real ecological context, this paper analyses the application of a novel symbolic machine learning technique, specifically designed to extract information from unstructured data and to express it in form of logical rules. Our purpose is to extract qualitative and quantitative logical rules, to relate the voltage at specific frequencies and in specific electrodes, and that, within the limits of the experiment, may help to understand the brain process that drives liking or disliking experiences in human subjects.

    Abstract Automating a factory where robots are involved is neither trivial nor cheap. Engineering the factory automation process in such a way that return of interest is maximized and risk for workers and equipment is minimized, is hence of paramount importance. Simulation can be a game changer in this scenario but requires advanced programming skills that domain experts and industrial designers might not have. In this paper we present the preliminary design and implementation of a general-purpose framework for creating and exploiting Virtual Environments via Natural language Agents (VEsNA). VEsNA takes advantage of agent-based technologies and natural language processing to enhance the design of virtual environments. The natural language input provided to VEsNA is understood by a chatbot and passed to a cognitive intelligent agent that implements the logic behind displacing objects in the virtual environment. In the VEsNA vision, the intelligent agent will be able to reason on this displacement and on its compliance to legal and normative constraints. It will also be able to implement what-if analysis and case-based reasoning. Objects populating the virtual environment will include active objects and will populate a dynamic simulation whose outcomes will be interpreted by the cognitive agent; explanations and suggestions will be passed back to the user by the chatbot.

    Abstract Background: Coronavirus disease 2019 (COVID-19) has been recognized as a disease with a broad spectrum of clinical manifestations. In this report, we illustrate an extraordinary case of severe cardioinhibitory reflex syncope with prolonged asystole associated with COVID-19.
    Case summary: A 35-year-old male patient presented to the emergency department with a 10-day history of postural syncope and fever. Electrocardiogram monitoring during positional change revealed reflex syncope with cardioinhibitory response, exhibiting sinus bradycardia, subsequent asystole, and syncope. The patient tested positive for severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) and was admitted to the intensive care unit where temporary transvenous pacing was necessary because of prolonged episodes of asystole. Work-up included extensive cardiac and neurological diagnostic testing, but did not yield any structural abnormalities. Although temporary pacing was able to abort syncope, a decision was made to hold off on permanent pacing as the most likely aetiology was felt to be temporary cardioinhibitory reflex syncope associated with COVID-19. The patient was discharged with mild symptoms of orthostatic intolerance and responded well to education and lifestyle modification. Outpatient follow-up with repeat tilt testing after 3 and 6 months initially showed residual inducible syncope but was eventually normal and the patient remained asymptomatic.
    Discussion: We believe that autonomic imbalance with a strong vagal activation due to acute SARS-CoV-2 infection played a pivotal role in the occurrence of transient syncope in this patient's condition. Although pacemaker implantation would have been a reasonable alternative, a watch-and-wait approach should be considered in similar instances.

    Abstract We develop the theory of Energy Conserving Descent (ECD) and introduce ECDSep, a gradient-based optimization algorithm able to tackle convex and non-convex optimization problems. The method is based on the novel ECD framework of optimization as physical evolution of a suitable chaotic energy-conserving dynamical system, enabling analytic control of the distribution of results - dominated at low loss - even for generic high-dimensional problems with no symmetries. Compared to previous realizations of this idea, we exploit the theoretical control to improve both the dynamics and chaos-inducing elements, enhancing performance while simplifying the hyper-parameter tuning of the optimization algorithm targeted to different classes of problems. We empirically compare with popular optimization methods such as SGD, Adam and AdamW on a wide range of machine learning problems, finding competitive or improved performance compared to the best among them on each task. We identify limitations in our analysis pointing to possibilities for additional improvements.
