    Abstract Biliary fibrosis is seen in cholangiopathies, including primary biliary cholangitis (PBC) and primary sclerosing cholangitis (PSC). In PBC and PSC, biliary fibrosis is associated with worse outcomes and histologic scores. Within the liver, both hepatic stellate cells (HSCs) and portal fibroblasts (PFs) contribute to biliary fibrosis, but their roles can differ. PFs reside near the bile ducts and may be the first responders to biliary damage, whereas HSCs may be recruited later and initiate bridging fibrosis. Indeed, different models of biliary fibrosis can activate PFs and HSCs to varying degrees. The portal niche can be composed of cholangiocytes, HSCs, PFs, endothelial cells, and various immune cells, and interactions between these cell types drive biliary fibrosis. In this review, we discuss the mechanisms of biliary fibrosis and the roles of PFs and HSCs in this process. We will also evaluate cellular interactions and mechanisms that contribute to biliary fibrosis in different models and highlight future perspectives and potential therapeutics.

    Abstract Avian anesthesia can be challenging due to birds’ anatomical and physiological peculiarities. Because of paucity of information on falcon anesthesia most drug doses and combinations are empirical. The objective of this study was to evaluate the anesthetic quality and physiological changes after dexmedetomidine-ketamine sedation and isoflurane anesthesia in 12 saker falcons (Falco cherrug), weighing (mean ± standard deviation) 915 ± 175 g. Pre-sedation heart rate (HR) and respiratory rate (fR) were assessed. A combination of ketamine (3 mg kg⁻¹) and dexmedetomidine (25 µg kg⁻¹) was administered intramuscularly. Sedation times were recorded, physiological variables assessed, and various blood parameters measured with a blood gas analyzer providing electrolyte levels, blood gas, glucose, hemoglobin and hematocrit. Induction was performed with isoflurane at 2% for 2 minutes and anesthesia was maintained at 1%. During maintenance, physiological variables were assessed at 5-minute intervals and blood parameters were re-evaluated before administration of atipamezole (250 µg kg⁻¹). Recovery times were documented. Quality of sedation, maintenance, and recovery were assessed. Maximum sedation effect was observed within 2.9 ± 1.0 minutes and quality of sedation and maintenance had a median score of « excellent » in all falcons. HR values decreased during sedation (pre-sedation value: 137 ± 38 versus sedated value: 99 ± 13 beats minute⁻¹; mean difference = -33; 95% confidence interval [CI] [-65; -10]; p = 0.001) and remained constant during maintenance. No significant differences were seen in fR from pre-sedation values (pre-sedation value: 23 ± 5 versus sedated value: 31 ± 13 breaths minute⁻¹; mean difference = 8; 95% CI [-1.7; 17.7]; p = 0.1). Mean arterial blood pressure (203 ± 14 versus 168 ± 22 mmHg; mean difference = -35.8; 95% CI [-51.1; -20.6]; p < 0.001) and cloacal temperature (40.3 ± 0.8 versus 38.9 ± 1.0°C; mean difference = -1.4; 95% CI [-2.2; -0.6]; p < 0.001) decreased after induction. Hypoxemia was observed in the sedated falcons but resolved during maintenance (65.9 ± 6.1 versus 402.2 ± 142.3 mmHg; mean difference = 336.3; 95% CI [227.4; 447.2]; p < 0.001), while hypercarbia (35.0 ± 5.8 versus 54.2 ± 11.5 mmHg; mean difference = 19.3; 95% CI [9.9; 28.7]; p < 0.001) and acidemia (pH 7.42 ± 0.06 versus 7.28 ± 0.036; mean difference = -0.13; 95% CI [-0.8; -0.08]; p < 0.001) developed. Full recovery was quick (12.3 ± 3.9 minutes). The proposed drug combination provided for high quality sedation and maintenance, and a satisfactory recovery. Nevertheless, alterations in the falcons’ physiology were documented, thus careful monitoring is mandatory.

    Abstract Background & aims: Patients with intrahepatic cholangiocarcinoma can now be managed with targeted therapies directed against specific molecular alterations. Consequently, tissue samples submitted to the pathology department must produce molecular information in addition to a diagnosis or, for resection specimens, staging information. The pathologist's role when evaluating these specimens has therefore changed to accommodate such personalised approaches.
    Methods: We developed recommendations and guidance for pathologists by conducting a systematic review of existing guidance to generate candidate statements followed by an international Delphi process. Fifty-nine pathologists from 28 countries in six continents rated statements mapped to all elements of the specimen pathway from receipt in the pathology department to authorisation of the final written report. A separate survey of 'end-users' of the report including surgeons, oncologists, and gastroenterologists was undertaken to evaluate what information should be included in the written report to enable appropriate patient management.
    Results: Forty-eight statements reached consensus for inclusion in the guidance including 10 statements about the content of the written report that also reached consensus by end-user participants. A reporting proforma to allow easy inclusion of the recommended data points was developed.
    Conclusions: These guiding principles and recommendations provide a framework to allow pathologists reporting on patients with intrahepatic cholangiocarcinoma to maximise the informational yield of specimens required for personalised patient management.
    Impact and implications: Biopsy or resection lesional tissue from intrahepatic cholangiocarcinoma must yield information about the molecular abnormalities within the tumour that define suitability for personalised therapies in addition to a diagnosis and staging information. Here, we have developed international consensus guidance for pathologists that report such cases using a Delphi process that sought the views of both pathologists and 'end-users of pathology reports. The guide highlights the need to report cases in a way that preserves tissue for molecular testing and emphasises that reporting requires interpretation of histological characteristics within the broader clinical and radiological context. The guide will allow pathologists to report cases of intrahepatic cholangiocarcinoma in a uniform manner that maximises the value of the tissue received to facilitate optimal multidisciplinary patient management.
