    Abstract Objectives: THD Anopress® is a new portable anal manometry device which can be used in an outpatient clinic setting. In this study, we aimed to: STUDY DESIGN: A retrospective analysis was conducted of women with OASI seen in a specialist clinic at 3- and 6-months post-delivery from November 2016 to December 2019. 72 women who attended their 6-month appointment and underwent anal manometry with Anopress were included. St Mark's Faecal incontinence score (FI) was calculated and for the purpose of analysis patients were classified into two groups - FI score less than 5 and 5 or more. EAUSS findings were classified as sphincter defect or no defect. RAVP and MSP were measured with Anopress and compared with the variables (symptoms and EAUSS findings) using the Mann-Whitney U test.
    Results: A total of 72 women were included in the study. There were 19 (27 %), 41 (57 %), 6 (8 %) and 6 (8 %) 3a, 3b, 3c and 4th degree perineal tears respectively. The median RAVP was 17 mm Hg (Inter-Quartile Range 10.75-24 mmHg) and median MSP was 47.5 mm Hg (IQR 33-68 mmHg). 38 patients (53 %) had a demonstrable sphincter defect on EAUSS. 37 patients (51.3 %) had FI score < 5 and 35 patients (48.6 %) had FI score of 5 or more. RAVP and MSP were significantly lower with EAUSS demonstrable sphincter defect (p < 0.001). Symptom severity correlated with RAVP (p = 0.016) though its correlation with MSP was not statistically significant (p = 0.096).
    Conclusion: Anopress seems promising in the assessment of anal sphincter function in women with OASI and can potentially make anal manometry testing easily accessible to urogynecologists.

    Abstract Background: The WHO classification of mucinous appendix neoplasms and pseudomyxoma peritonei (PMP) describes low- and high-grade histology and is of prognostic importance. The metastatic peritoneal disease grade can occasionally be different from the primary appendix tumor. This analysis aimed to report outcomes from a high-volume center in patients with pathological discordance.
    Methods: This was a retrospective analysis of prospective data of patients treated by cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) for PMP at a single institution between January 2016 and December 2020. Reporting was by pathologists with a special interest in peritoneal malignancy. Discordant pathology was classified as a low-grade primary appendix tumor with high-grade peritoneal disease, or a high-grade primary appendix tumor with low-grade peritoneal disease. Outcomes analyzed were overall and recurrence-free survival, and Kaplan-Meier survival curves and the log-rank test were used to analyze the outcomes.
    Results: Between 2016 and 2020, 830 patients underwent CRS and HIPEC for PMP, of whom 37 (4.4%) had discordant pathology. The primary appendix tumors were low-grade in 23 patients and high-grade in 14 patients. The median Peritoneal Cancer Index (PCI) was significantly higher in patients with a low-grade primary tumor (31 vs. 16; p = 0.001), while complete cytoreduction (CC0/1) was achieved in 31/37 (83.8%) patients. The median follow-up was 19 months. Overall survival was worse in those with high-grade peritoneal disease (p = 0.029), whereas recurrence-free survival was similar in both groups (p = 0.075).
    Conclusion: In PMP with pathological discordance, the peritoneal disease grade influences prognosis and survival.

    Abstract Purpose: The aim of this study was to assess the effects of socioeconomic deprivation on short-term outcomes and long-term overall survival following major resection of colorectal cancer (CRC) at a tertiary hospital in England.
    Method: This was an observational cohort study of patients undergoing resection for colorectal cancer from January 2010 to December 2017. Deprivation was classified into quintiles using the English Indices of Multiple Deprivation 2010. Primary outcome was overall complications (Clavien-Dindo grades 1 to 5). Secondary outcomes were the major complications (Clavien-Dindo 3 to 5), length of hospital stay and overall survival. Outcomes were compared between most affluent group and most deprived group. Multivariate regression models were used to establish the relationship taking into account confounding variables.
    Results: One thousand eight hundred thirty-five patients were included. Overall and major complication rates were 44.9% and 11.5% respectively in the most affluent, and 54.6% and 15.6% in the most deprived group. Most deprived group was associated with higher overall complications (odds ratio 1.48, 95% CI 1.13-1.95, p = 0.005), higher major complications (odds ratio 1.49, 1.01-2.23, p = 0.048) and longer hospital stay (adjusted ratio 1.15, 1.06-1.25, p < 0.001) when compared with most affluent group. Median follow period was 41 months (interquartile range 20-64.5). Most deprived group had poor overall survival compared with most affluent, but it was not significant at the 5% level (hazard ratio 1.27, 0.99-1.62, p = 0.055).
    Conclusion: Deprivation was associated with higher postoperative complications and longer hospital stay following major resection for CRC. Its relationship with survival was not statistically significant.

    Abstract Paget's disease is an intraepithelial adenocarcinoma arising from the apocrine gland component of the skin. Paget's disease is most common in the breast but extra mammary disease is also seen. Perianal Paget's disease is a rare form of extramammary disease with few cases reported in literature. It can be primary-arising from the skin or secondary-cutaneous metastases of anorectal or genitourinary malignancy. We hereby wish to report a case of perianal Paget's disease that presented as an eczematous lesion and was diagnosed incidentally on biopsy. After appropriate staging, the patient underwent wide local excision till negative margins were obtained. The resultant tissue defect was successfully covered by split-thickness skin grafting.

    Abstract Diaphragmatic hernia is an important cause of emergency hospital admission associated with significant morbidity. It usually results from congenital defect or rupture in the diaphragm due to trauma. Prompt and appropriate diagnosis is necessary in patients with this condition, as surgical intervention by either abdominal or thoracic approach may be necessary. Here, we report a case of left-sided diaphragmatic hernia presenting with sudden onset of breathlessness, respiratory distress and left-sided chest pain radiating to the abdomen, mimicking pneumothorax, treated successfully with surgical intervention.
