    Abstract Background & aims: Fatigue has high negative impact on many patients with primary biliary cholangitis (PBC) and treatment options are limited. Recently we showed favorable effects of four weeks of high-dose thiamine treatment on fatigue in patients with inflammatory bowel disease. We aimed to investigate the effect and safety of high-dose (600-1800 mg daily) oral thiamine treatment on chronic fatigue in patients with PBC.
    Methods: Randomized, double-blinded, placebo-controlled crossover trial including patients with severe PBC-related fatigue. Participants were allocated 1:1 to either group 1) 4 weeks of high-dose thiamine, 4 weeks of washout, and 4 weeks of placebo; or group 2) 4 weeks of placebo, washout, and high-dose thiamine, respectively. Fatigue severity was quantified using the fatigue subscale of the PBC-40 questionnaire. The primary outcome was a fatigue reduction of ≥ 5 points after 4 weeks of high-dose thiamine treatment.
    Results: We enrolled 36 patients; 34 completed the study. The overall mean reduction in fatigue was 5.0 points (95% CI: 2.5 to 7.5; p < 0.001) for the combined group 1 and group 2. Crossover analysis showed a mean increase in fatigue of 0.3 points (95% CI: -4.2 to 3.8) after high-dose thiamine treatment compared to a 1.4 points (95% CI: 6.2 to -3.4) mean reduction after placebo (p = 0.55). Only mild and transient adverse events were recorded.
    Conclusion: Four weeks of high-dose oral thiamine treatment in patients with PBC was well tolerated and safe. However, high-dose thiamine was not superior to placebo in reducing PBC-related fatigue.
    Trial registration: The trial was registered in the ClinicalTrials.gov (NCT04893993) and EudraCT (2020-004935-26).

    Abstract Autoimmune liver diseases are rare serious diseases causing chronic inflammation and fibrosis in the liver parenchyma and bile ducts. Yet, the prevalence and burden of autoimmune liver diseases are largely unexplored in Arctic native populations. We investigated the prevalence and management of autoimmune liver diseases in Greenland using nationwide cross-sectional register data and subsequent medical chart reviews validating diagnoses and extracting liver histology examinations and medical treatments. The overall prevalence of autoimmune liver diseases in Greenland was 24.6 per 100,000 (95% CI: 14.7-41.3). This was based on 7 patients with autoimmune hepatitis (AIH) (12.3 per 100,000), 3 patients with primary biliary cholangitis (PBC) (5.3 per 100,000), 4 patients with AIH/PBC overlap disease (7.0 per 100,000), and no patients with primary sclerosing cholangitis. All diagnoses were confirmed by liver histology examinations. Medical treatments adhered to internal recommendations and induced complete remission in most patients with AIH, and complete or partial remission in 1 patient with PBC and 3 patients with AIH/PBC overlap disease. One patient had established cirrhosis at the time of diagnosis, while 2 patients progressed to cirrhosis. In conclusion, the prevalence of autoimmune liver diseases was lower in Greenland than in Scandinavia and among Alaska Inuit.

    Abstract Introduction: Pheochromocytomas and paragangliomas are rare neuroendocrine tumours that originate from chromaffin cells within the adrenal medulla or extra-adrenal sympathetic ganglia.     Results: We included 28 patients; 10 were diagnosed with pheochromocytoma and 18 with paraganglioma. Median age at first PRRT was 47 (IQR 15-76) years.     Abstract Data concerning non-invasive discrimination of simple steatosis from steatohepatitis in metabolic-associated fatty liver disease (MAFLD) and risk of disease progression in patients with MAFLD are conflicting. We aimed to investigate these factors in an MAFLD cohort at a Danish tertiary liver centre. We retrospectively assessed 129 patients with biopsy-proven MAFLD. Patients were divided according to the presence of simple steatosis or steatohepatitis in liver biopsies. Histological and clinical progression were assessed during follow-up. Patients with steatohepatitis had higher BMIs, liver stiffness, HbA1c and soluble (sCD163) and were more prone to have metabolic syndrome at baseline compared with simple steatosis patients. Of the 129 patients, 31 had a follow-up biopsy after a median of 287 days; simple steatosis progressed to steatohepatitis in 7 cases, while 2 regressed. Twenty patients had the same fibrosis stage according to the follow-up biopsy, seven progressed and four regressed. Only 14 patients progressed clinically (median follow-up time was 3.8 years). Clinical progression was associated with female sex, high creatinine, high sCD163 and disease severity in the diagnostic liver biopsy. Steatohepatitis was associated with metabolic and inflammatory parameters including fibroscan. Disease progression was seen in only 11% of cases and was mainly related to more severe histological disease at baseline.

    Abstract The novel term Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD) is proposed to replace non-alcoholic fatty liver disease (NAFLD) to highlight the close association with the metabolic syndrome. MASLD encompasses patients with liver steatosis and at least one of five cardiometabolic risk factors which implies that these patients are at increased risk of cardiovascular disease (CVD). Indeed, the prevalence of CVD in MASLD patients is increased and CVD is recognized as the most common cause of death in MASLD patients. We here present an update on the pathophysiology of CVD in MASLD, discuss the risk factors, and suggest screening for CVD in patients with MASLD. Currently, there is no FDA-approved pharmacological treatment for MASLD, and no specific treatment recommended for CVD in patients with MASLD. Thus, the treatment strategy is based on weight loss and a reduction and treatment of CVD risk factors. We recommend screening of MASLD patients for CVD using the SCORE2 system with guidance to specific treatment algorithms. In all patients with CVD risk factors, lifestyle intervention to induce weight loss through diet and exercise is recommended. Especially a Mediterranean diet may improve hyperlipidemia and if further treatment is needed, statins should be used as first-line treatment. Further, anti-hypertensive drugs should be used to treat hypertension. With the epidemic of obesity and type 2 diabetes mellitus (T2DM) the risk of MASLD and CVD is expected to increase, and preventive measures, screening, and effective treatments are highly needed to reduce morbidity and mortality in MASLD patients.

    Abstract A novel range hood (NRH) consisting of a high-efficiency particulate air (HEPA) filter circulation component and exhaust component was designed to improve indoor air quality. The HEPA filter circulation component with an airflow rate of 72 l/s was used to remove the particulate matter generated by cooking activities and return filtered clean air to the room. The exhaust component was used to reject both particulate matter and other indoor air pollutants to the outdoors. Field measurements were carried out in an open kitchen of a two-floor residential house. The total occupied zone was about 160 m³ with a living area of 64 m2 (including an open kitchen) and a height of 2.5 m. Frying bacon that can generate a large number of particles of different sizes was used to simulate the daily cooking activities. Four operating conditions of the NRH were studied, i.e. 38 l/s exhaust airflow rate with the HEPA circulation on and off, and 78 l/s exhaust airflow rate with the HEPA circulation on and off. The concentration of PM0.1 count, PM2.5 mass, and PM10 mass were measured during measurements. The HEPA filter circulation part of the NRH can dramatically reduce the concentration of all sizes of particles generated by cooking activities. By combining the HEPA filter circulation component, the NRH can separately reduce the concentration of PM0.1, PM2.5, PM10 by up to 91.9%, 95.7%, 94.6% compared to that when only running the exhaust part.
