    Abstract Ischemic preconditioning (IPC) appears to improve exercise performance although there is uncertainty about the intensity dependence of this effect. The present study sought to clarify effects of IPC on physiological responses at and below peak oxygen uptake, including the gas exchange threshold (GET).     Abstract In 1847, the Austrian painter Leopold Kupelwieser (1796-1862) was commissioned to execute a fresco cycle in the Ceremonial Hall of the Niederösterreichische Statthalterei (Lower Austrian Government Building, Vienna), depicting scenes from Austrian history. Iconographic aspects of each painting, using pictorial and literary sources as well as pointing out biographical connections and artistic forebears and influences, are being discussed. The complex interplay between art technology and conceptual statements manifests itself particularly articulate in the focal point of cartoon and fresco; accordingly, this study approaches Kupelwiesers work through the description of the materials he used and the way he processed them

    Abstract Perioperative anaphylaxis is a rare and unpredictable event that continues to cause patient harm. More work is needed to decrease the risk to patients through measures to limit sensitisation, optimise management and investigation, and ensure that patients are not inadvertently re-exposed to allergens. Robust epidemiological data such as that provided by the consecutive GERAP surveys over the past 30 yr have been invaluable in defining the problem, identifying emerging allergens, acting as a catalyst for change, and stimulating research.

    Abstract The human body has evolved greatly over time and the hand has shown some of the most intricate changes. Most evolutionary experts attribute this to the greater use of tools facilitated by the early hominid's transition to a bipedal gait. Increased tool use drove these changes by providing a convincing reproductive advantage for early humans. In particular, hand adaptations resulted from two types of grips that were fundamental in our development: the precision grip and the power grip. To fully understand how these changes in development occurred, a review of evolutionary theory will be proffered and further discussion of the unique architecture of the hands of our closest living relatives compared with that of humans will follow. Finally, we will examine the two uniquely human grips, the anatomic adaptations that allowed for their development, and the reasoning behind how these grips provided a compelling reproductive advantage.

    Abstract Purpose of review: Lymphodepleting chemotherapy (LD) has emerged as a key determinant of chimeric antigen receptor T cell (CAR) efficacy across pediatric/adult B cell malignancies. Clinical trials demonstrate the superiority of fludarabine/cyclophosphamide (Flu/Cy) regimens, resulting in the adoption of Flu/Cy as the pre-CAR LD standard. In the context of a global fludarabine shortage, consideration of alternative regimens is timely, yet limited clinical data exists, specifically in the pediatric B-ALL CAR setting.
    Recent findings: Bendamustine has been used as an effective LD prior to CD19-CAR in adult lymphoma. Although use in the pediatric CAR setting is limited, tolerability has been established in pediatric Hodgkin's lymphoma. Clofarabine is a purine nucleoside analog with mechanistic overlap with fludarabine; however, toxicity is high in the upfront leukemia setting, and thus use as an LD pre-CAR should be pursued with caution. We review the experience using bendamustine and clofarabine to serve as a resource when considering LD regimens as an alternative to fludarabine for pediatric B-ALL.
