    Abstract Hidradenitis suppurativa (HS), which is characterized by the obstruction of the apocrine gland channels and development of secondary bacterial infections, is a rare clinical entity mostly seen in women. In this case report, a bacterial meningitis case, which emerged after HS in the gluteal region, was presented. To the authors' knowledge this is the first case in the literature. Sixty-years-old male patient was admitted to the infectious disease department with high fever, lack of consciousness and orientation and cooperation problems. Upon detection of neck stiffness during physical examination, cerebrospinal fluid (CSF) specimen was taken. CSF analysis revealed increased pressure, turbidity, (++++) Pandy reaction and 33.600/ml leucocytes (95% PMNL). CSF glucose was 2 mg/dl (simultaneous blood glucose was 156 mg/dl) and protein was 360 mg/dl. Gram stained smear of the CSF specimen yielded high numbers of gram-positive cocci. Empirical treatment with intravenous ceftriaxone (2 x 2 g/day) and linezolid (2 x 600 mg/day) was initiated upon the diagnosis of acute bacterial meningitis. Physical examination of the patient revealed HS characterized by common fistulas in gluteal area and drainage from the fistula with drainage in left gluteal area. White blood cell blood count was 26.700/mg, platelet count was 501.000/ml and hemoglobin was 14.8 mg/dl. Erythrocyte sedimentation rate was 120 mm/hour. In the magnetic resonance imaging of the vertebra, fistula tract was determined between inflammed tissue and sacral subarachnoid region. Since the culture of CSF and blood did not yield any growth, the probable causative agent was thought to be an anaerobic gram-positive coccus. The treatment continued for 21 days for ceftriaxone and 28 days for linezolid. The patient was successfully treated and was transferred to neurosurgery department without any sequela for the management of fistulas.

    Abstract Introduction: We describe the clinical characteristics, treatment and long-term outcome of 19 patients with Brucella epididymoorchitis treated at the Department of Urology.
    Materials and methods: Between 1998 and 2005, a total of 19 cases with epididymoorchitis due to Brucella melitensis were diagnosed at our hospital. The diagnosis of Brucella was made by isolating Brucella species from blood culture or epididymal aspiration or by standard tube agglutination testing and slide agglutination testing together with clinical findings.
    Results: Seven cases (36.8%) had undulant fever. Brucella species was isolated from blood cultures in 11 patients and from epididymal aspiration in 3 patients. Rose Bengal tests were positive in all patients. Total recovery was obtained in 12 cases with a 6-week treatment. Five patients (26.3%) did not respond to therapy and another 2 (10.5%) relapsed.
    Conclusions: Brucella orchitis should be considered in the differential diagnosis of scrotal pathologies where Brucella is endemic.

    Abstract Methicillin-resistant Staphylococcus aureus (MRSA) strains with inducible macrolide-lincosamide-streptogramin B (iMLS(B)) resistance phenotype may lead to clinical failure during clindamycin (CLI) therapy. The aim of this study was to determine the incidence of MLS(B) phenotypes by using D-test method and genotypes by using multiplex real-time PCR method in MRSA strains. A total of 265 MRSA strains were obtained from clinical samples from hospitalized and outpatients. Of the MRSA isolates, 225 (84.9%) were resistant to erythromycin (ERT), and 170 (64.1%) to CLI. Among the 225 ERT-resistant MRSA strains, the constitutive MLS(B) (cMLS(B)) rate was found in 49.3%, iMLS(B) in 39.1% and the M phenotype in 11.5%. Overall, ermA, ermC, ermA+ermC, msrA, ermC+msrA, and ermA+ermC+msrA genes were detected in 85 (37.7%), 60 (26.6%), 42 (18.6%), 26 (11.5%), 11 (4.8%), and 1 (0.4%) isolates, respectively. Most prevalent resistance determinant in MRSA strains was ermA, which was detected in 37.7% of the isolates. The 26 MRSA strains with M phenotype harboured only msrA gene. In conclusion, due to aware of the potential of CLI treatment failure, D-test should be performed and reported in MRSA strains in clinical laboratories. The multiplex real-time PCR method is easy to perform, fast and reliable method for the detection of MLS(B) resistance genotypes.

    Abstract Objective: To review retrospectively the clinical symptoms, laboratory findings and treatment outcomes of patients with Brucellar epididymo-orchitis.
    Material and method: Retrospective data of 28 patients with Brucellar epididymo-orchitis who admitted to four medical centers between 2005 and 2013 were retrospectively reviewed. Positive blood culture, positive Rose Bengal test results or high agglutination titres of ≥ 1/160 with the positive clinical and ultrasonographic findings of orchitis were accepted as the main criteria for Brucellar epididymo-orchitis.
    Results: The mean patient age was 31 ± 16.9 years. Testicular involvement was on the left side in 16 patients and on the right side in 11 patients, one had bilateral disease. Testicular pain and swelling were the most common symptoms and elevation of C-reactive protein (CRP), erythrocyte sedimentation rate (ESR) and leucocytosis were the most common laboratory findings. Initial treatment was orchidectomy in six patients due to malignancy suspicion. All but three patients were successfully treated with antibiotic combinations of rifampicin, doxycycline and streptomycin. Two of three treatment resistant patients underwent orchidectomy.
    Conclusion: Brucellosis is a common cause of epididymo-orchitis in endemic regions. Early diagnosis and treatment is crucial in the management and thus it must be kept in mind in endemic and non-endemic regions.

    Abstract Objectives: This study sought to determine causative microorganisms of chronic suppurative otitis media and their antibacterial susceptibility.
    Patients and methods: Bacteriologic results of middle ear swabs, agent microorganisms, and their susceptibility to antibiotics were evaluated in 70 patients (65 males, 5 females; mean age 21.1+/-1.3 years; range 19 to 25 years) with chronic suppurative otitis media.
    Results: No bacteria were isolated in 14% of the patients. Of 60 patients in whom bacteria were isolated, the most common bacteria was Pseudomonas aeruginosa (23%), followed by Staphylococcus aureus (18%), and Proteus spp. (17%). P. aeruginosa strains were susceptible to ceftazidime and imipenem (100%), ciprofloxacin (92%), and amikacin and gentamycin (85%); S. aureus strains were susceptible to methicillin and vancomycin (100%), ciprofloxacin (91%), sulbactam-ampicillin (73%), and gentamycin and trimethoprim-sulfamethoxazole (63%). Proteus strains were susceptible to ciprofloxacin (100%), ceftazidime (90%), and imipenem and gentamycin (70%).
    Conclusion: Appropriate knowledge of antibacterial susceptibility of microorganisms will contribute to rational antibiotic usage and the success of treatment for chronic suppurative otitis media.
