    Abstract In the present study, we aimed to extend the characterization of the proposed naturalistic experimental model of piglets born with hypoxia by assessing the relationship between birth weight, intra partum asphyxia and gross indicators of neurophysiological alterations in newborn piglets. Three groups of 50 piglets each were classified according to their birth weight into normal (1000-1350 g), low (below 1000 g), and high (over 1350 g). In comparison to piglets within normal weight, those born with high birth weights showed acid-base imbalance as reflected by lower pH levels (7.03±0.01), hypercapnia (88.50±13.20 mmHg), and lactic acidosis (lactate levels: 89.40±26.30). These piglets had lower viability scores (5.40±0.60) and longer periods of time to contact the udder (52.30±8.30) than piglets with normal birth weight. In conclusion, data show that piglets with birth weight over 1350 g are at a higher risk of gross neurophysiological deficits, probably secondary to neonatal hypoxia.

    Abstract The objective of this study was to carry out a review of the different therapeutic methods used to reduce stress in slaughter animals during transport to abattoirs. As things stand today we have no precise means of quantifying animals responses to stress or established techniques for reducing or preventing this condition, though a wide variety of studies have focused on the use of drugs to mitigate or prevent stress in domesticated wild and laboratory animals. In the specific case of animals destined for human consumption however the use of drugs is strictly limited due primarily to such factors as elimination times and the duration of drug residues or their metabolites in tissues before and after slaughter. In some experiment the use of supplements (additives) as alternatives to applying chemical-based drugs has been shown to have beneficial effects on animal welfare and the quality of the meat destined for human consumption. Nonetheless, additional studies are necessary to assure compliance with local transport laws and the norms of slaughtering and to make sure that animal handlers and carriers throughout the meat processing chain are adequately trained and sufficiently well-informed on these issues. Also it is important to create national-level legislation to regulate the administration of drugs utilized to reduce stress in animals destined for human consumption during transport and mitigate the possible effects of their use on public health.

    Abstract Introduction: T tubes can be placed in the bile ducts either open or laparoscopically for several reasons such as: extraction of stones, biliary reconstruction after liver transplant and in end-to-end anastomosis in iatrogenic injuries. Inadequate placement of the T tube, long term stay and technical difficulties that can affect the outcome, can lead to an injury that usually requires a biliodigestive reconstruction.Methods: In a 15-year period (1990–2005) a total of 343 patients have been referred to our university hospital for biliary reconstruction. Files of those patients in which the injury was due to misplacement of a T tube or associated with a long-term stay were reviewed. We evaluated the type of injury, technique used for the reconstruction, longterm staying of the T tubes (1–6 months), hospital in stay, long term outcomes as well as associated comorbidities.Results: In 42 cases a biliary injury related to a T tube was identified (13%). All the injuries were classified as Strasberg E, with demonstration of a fistula (internal or external); 18 to the duodenum, 5 to the jejunum – ileum and 3 to the colon. A hepatojejunostomy was done to all patients; the duodenum and small gut fistulas were closed and in the 3 cases with colonic injury a right hemicolectomy was performed. The postoperative evolution was adequate without major complications but with a longer hospital stay. In 39 of the 42 patients (92%), good postoperative results were obtained. Only one case required a new surgery (22 months after the first one), due to recidivant cholangitis.Conclusion: Inadequate placement of the T tubes and long-term stay can produce complex biliary injuries with associated comorbidities such as fistulas to the adjacent viscera. Placement of T tubes need a careful surgical technique and their indication must be carefully assessed.

    Abstract Multidrug- and extensively drug-resistant tuberculosis have emerged as grave threats to public health worldwide. Very few active drugs are available or likely to become available soon. To address these problems we revisited a classical observation, the applicability of phenothiazines as antimicrobial drugs. Within this pharmacological class we selected thioridazine, which is most efficacious and least toxic, when used as an antipsychotic drug. We tested thioridazine monotherapy in the Balb/c mouse model for its activity to treat both susceptible and multidrug-resistant tuberculosis by a two months daily oral administration of 32 and 70 mg/kg. In addition, we tested its additive value when combined with a standard first-line regimen for susceptible tuberculosis. Thioridazine treatment resulted in a significant reduction of colony-forming-units of the susceptible (-4.4 log CFU, p<0.05) and multidrug-resistant tuberculosis bacilli (-2.4 log CFU, p<0.009) in the lung both at one and two months after infection, compared to controls. Moreover, when thioridazine was added to a regimen containing rifampicin, isoniazid and pyrazinamide for susceptible tuberculosis, a significant synergistic effect was achieved (-6.2 vs -5.9 log CFU, p<0.01). Thioridazine may represent an effective compound for treatment of susceptible and multidrug-resistant tuberculosis. The phenothiazines and their targets represent interesting novel opportunities in the search for antituberculosis drugs.

    Abstract Most iatrogenic bile duct injuries are recognized in the early postoperative period (first 48 hours). These patients usually have additional complications such as a suboptimal hydroelectrolitic status, subhepatic collections, external biliary fistula and malnutrition. In these circumstances, besides the elevation of bilirrubin and transaminases associated with the injury, hypoalbuminemia is frequently encountered. The timing for repair is decided according to the condition of each patient. We report the impact of preoperative abnormal low serum albumin levels on the results of biliary tract reconstruction after a iatrogenic biliary lesion. Method: Patients who underwent biliary reconstruction in our center from 1998 to 2002 were analyzed. Only patients with complex injuries (Strasberg E, Bismuth III-IV, Stewart-Way III) were included. Major postoperative complications were recorded and correlated with preoperative liver function tests. Results: Seventy seven patients were analyzed. In 41 cases, the injury was a consequence of a laparoscopic operation. All patients were treated by a Roux-en-Y hepatojejunostomy. No operative mortality was recorded. The most frequent postoperative complications were postoperative biliary fistula (8/77-9%, p < 0.017) and subhepatic collections (9/77-9%, p < 0.39). All fistulae closed spontaneously and the subhepatic collections were drained. Overall, complications were more common in the group with hypoalbuminemia (p < 0.002). Conclusion: Early repair is indicated if there is no systemic contraindication (sepsis, multiple organic failure, electrolytic imbalance). Abnormalities in the liver function tests, particularly a low serum albumin, should not delay the operation. Although significantly more postoperative complications are observed in an early repair, long-term results are comparable to those of an elective repair.

    Abstract Bile duct injuries occur with a frequency of 1 to 5 per 1000 cases as a result of an increase in the number of procedures performed. Elderly patients have more severe lithiasis-related diseases than the younger population. This fact increases the likelihood of conversion from laparoscopic to open surgery, and the probability of injury. We report the results of bile duct reconstruction after injury in these patients. Setting: A tertiary care academic university hospital. Methods: The files of patients over 65 years of age who had biliary tract reconstruction after iatrogenic injury were retrospectively reviewed. Post operative results, quality of life and failures from the repair were analyzed. Results: 20 patients over the age of 65 were referred for biliary tract reconstructive surgery over a ten year period. Mean age was 71 (65 – 83). Nineteen cases were referred from other hospitals. Fourteen cases (60%) had comorbidities (type 2 diabetes mellitus, systemic hypertension). All patients were treated by means of Roux en Y hepatojejunostomy. No operative morbility was recorded and only one major complication (abdominal collection) was found. Two long term mortalities unrelated to surgery were found. Treatment success (as defined by the Johns Hopkins criteria) was obtained in 17 cases (85%) and only one patient (5%) required reoperation two years after the initial attempt. Conclusions: Elderly patients that survive biliary injury and are reconstructed have long-term results comparable to the younger population; Roux en Y hepatojejunostomy offers the best surgical choice.

    Abstract INTRODUCCIÓN: Los tumores de vías biliares son tumoresraros que tienen una incidencia de 1 en 100, 000 habitantes.En un paciente con este tipo de tumor la cirugía puede sercurativa en etapas tempranas, con una supervivencia a 5años de 20%, los pacientes con enfermedad avanzada tienenuna supervivencia de 6 a 7 meses y en ellos están indicadoslos procedimientos paliativos.OBJETIVO: Mostrar la experiencia en un período de 14 añosen pacientes con tumores de vías biliares y que recibieronalgún tratamiento quirúrgico con intento curativo o paliativo.METODOLOGÍA: Se trata de un estudio metodológicamentediseñado como serie de casos, con 23 pacientes operadosen un período de 14 años, de 1987 a 2001.RESULTADOS: La colangitis fue el principal motivo deconsulta en 55% (13) de los pacientes, al ingreso se observóictericia y malestar generalizado en 95% de los pacientes, eldolor y el prurito fueron la segunda manifestación en el77%. La sensibilidad diagnóstica del ultrasonido y latomografía fue mayor al 50%. Las lesiones Bismuth II fueronlas más frecuentes en 39%. Se realizó Hepato-yeyunoanastomosis en el 59%, de los cuales se colocó sonda en Ten el 77% de los pacientes. Otras alternativas utilizadasfueron hepatectomía derecha o izquierda y procedimientode Whipple. En 4 casos sólo se realizó derivación con sondaT o la colocación percutánea de sonda transhepática.El adenocarcinoma no papilar fue la variedad histológicamás frecuente, los estadios avanzados T4 se observaron enun 50% y sólo el 32% tenían un estadio T2. El promedio desupervivencia fue de 7.5 meses (intervalo 0.07 a 22.2).CONCLUSIONES: Los tumores de vías biliares son unaenfermedad poco frecuente, el mayor número de casos sepresentan en etapas avanzadas cuando la cirugía ya noofrece la posibilidad de curación, por tanto, las alternativasquirúrgicas paliativas son de elección para mejorar lacalidad de vida.
