    Abstract Background: H3 K27M-mutated gliomas were first described as a new grade 4 entity in the 2016 WHO classification. Current studies have focused on its typical appearance in children and young adults, increasing the need to better understand the prognostic factors and impact of surgery on adults. Here, we report a multicentric study of this entity in adults.
    Methods: We included molecularly confirmed H3 K27M-mutated glioma cases in patients >18 years diagnosed between 2016 and 2022. Clinical, radiological, and surgical features were analyzed. Univariate and multivariate analyses were performed to identify prognostic factors.
    Results: Among 70 patients with a mean age of 36.1 years, the median overall survival (OS) was 13.6 + 14 months. Gross-total resection was achieved in 14.3% of patients, whereas 30% had a subtotal resection and 54.3% a biopsy.Tumors located in telencephalon/diencephalon/myelencephalon were associated with a poorer OS, while a location in the mesencephalon/metencephalon showed a significantly longer OS (8.7 vs. 25.0 months, p=0.007). Preoperative Karnofsky Performance Score (KPS) < 80 showed a reduced OS (4.2 vs. 18 months, p=0.02). Furthermore, ATRX loss, found in 25.7%, was independently associated with an increased OS (31 vs. 8.3 months, p=0.0029). Notably, patients undergoing resection showed no survival benefit over biopsy (12 vs. 11 months, p=0.4006).
    Conclusion: The present study describes surgical features of H3 K27M-mutated glioma in adulthood in a large multicentric study. Our data reveal that ATRX status, location and KPS significantly impact OS in H3 K27M-mutated glioma. Importantly, our dataset indicates that resection does not offer a survival advantage over biopsy.

    Abstract Background: H3K27-altered diffuse midline gliomas are uncommon central nervous system tumors with extremely poor prognoses.
    Case presentation: We report the case of a 24-year-old man patient with multiple, inter alia osseous metastases who presented with back pain, hemi-hypoesthesia, and hemi-hyperhidrosis. The patient underwent combined radio-chemotherapy and demonstrated temporary improvement before deteriorating.
    Conclusions: H3K27-altered diffuse midline glioma presents an infrequent but crucial differential diagnosis and should be considered in cases with rapid neurological deterioration and multiple intracranial and intramedullary tumor lesions in children and young adults. Combined radio-chemotherapy delayed the neurological deterioration, but unfortunately, progression occurred three months after the diagnosis.

    Abstract We present a rare case of a patient initially presenting with unilateral abdominal wall bulging and radicular pain caused by a lateral disc herniation at Th11/12, later suffering from a hernia recurrence with bilateral disc prolapse and motor deficits. The patient underwent sequesterectomy via a right hemilaminectomy at Th11, and after 8 weeks, a bilateral sequesterectomy with semirigid fusion Th11/12 was performed. Unilateral motor deficits at the thoracic level have been discussed in case reports; a bilateral disc protrusion with abdominal wall bulging occurring as a recurrent disc herniation has never been described before.

    Abstract A major evolution from purely clinical diagnoses to biomarker supported clinical diagnosing has been occurring over the past years in neurology. High-throughput methods, such as next-generation sequencing and mass spectrometry-based proteomics along with improved neuroimaging methods, are accelerating this development. This calls for a consensus framework that is broadly applicable and provides a spot-on overview of the clinical validity of novel biomarkers. We propose a harmonized terminology and a uniform concept that stratifies biomarkers according to clinical context of use and evidence levels, adapted from existing frameworks in oncology with a strong focus on (epi)genetic markers and treatment context. We demonstrate that this framework allows for a consistent assessment of clinical validity across disease entities and that sufficient evidence for many clinical applications of protein biomarkers is lacking. Our framework may help to identify promising biomarker candidates and classify their applications by clinical context, aiming for routine clinical use of (protein) biomarkers in neurology.

    Abstract Objectives: The point of departure for the present work is the observation that, in comparison with Anglo-American countries or the Netherlands, Germany was responsible for only a marginal number of international publications in the field of medical education research before 2004. Recent years, however, have seen an increase in the importance of medical education research in Germany. The objective of this article is to evaluate the extent to which this trend can be substantiated by increased German publishing activity since the year 2004 in international, English-language journals in the subject area of "medical education research".
    Methods: In the framework of a literature and content analysis, German-author articles from the years 2004 to 2013 in six international, English-language medical education research journals were evaluated. In order to obtain an overview of German research activity in this area, all project and original works with German first and last authors were identified and subjected to a more in-depth content analysis.
    Results: In total, 10,055 articles were examined. The evaluation shows that between the years 2004 and 2013 179 articles (of which 145 are project or original works) by German authors were published in the journals examined. Fluctuations over the course of time were evidenced. The project and original works are primarily cross-sectional studies (27.8%) and randomised control studies (25.6%) on the subject of "teaching and learning methods" (43.6%).
    Conclusions: In comparison with the years 2004-2008, a significant rise can be seen in the number of publications by German education researchers in international journals since the year 2009.

    Abstract Zielsetzung: Ausgangspunkt des vorliegenden Artikels ist die Feststellung, dass es aus Deutschland im Vergleich zu den angloamerikanischen Ländern oder den Niederlanden vor 2004 kaum internationale Publikationen im Bereich der medizinischen Ausbildungsforschung gab. In den letzten Jahren wurde jedoch eine steigende Bedeutung der medizinischen Ausbildungsforschung im deutschen Raum deutlich. Zielsetzung dieses Beitrags ist es zu prüfen, inwieweit sich in internationalen, englischsprachigen Fachzeitschriften im Themenfeld „Medizinische Ausbildungsforschung“ seit 2004 eine solche Entwicklung durch eine gesteigerte Publikationsaktivität belegen lässt.
    Methodik: Im Rahmen einer Literaturauswertung und Inhaltsanalyse wurden Artikel deutscher Autoren aus den Jahren 2004 bis 2013 in sechs internationalen englischsprachigen Fachzeitschriften der medizinischen Ausbildungsforschung analysiert. Um einen Überblick über die deutschen Forschungsaktivitäten in diesem Bereich zu bekommen, wurden alle Projekt- und Originalarbeiten deutscher Erst- und Letztautoren identifiziert und einer tiefergehenden inhaltlichen Analyse unterzogen.
    Ergebnisse: Insgesamt wurden 10.055 Artikel untersucht. Die Auswertung zeigt, dass zwischen 2004 und 2013 179 Artikel – hiervon 145 Projekt- und Originalarbeiten – deutscher Autoren in den betrachteten Zeitschriften publiziert wurden. Es zeigen sich Schwankungen im Zeitverlauf. Bei den Projekt- und Originalarbeiten handelt sich vor allem um Querschnitt- (27,8%) und randomisierte Kontrollstudien (25,6%) zum Thema „Lehr- und Lernmethoden“ (43,6%).
