    Abstract Mode (de)multiplexer is an essential device in integrated multimode photonic systems. Here, we present a dual-mode (de)multiplexer that separates two input modes, TE 0 and TE 1 , into two output ports while converting TE 1 to TE 0 mode. Based on the adjoint and level set method, the device features a small footprint of 9.4 μm × 2.9 μm, and a minimum feature size over 200 nm is achieved, affirming stable and reliable fabrication. Through simulations, we observed insertion losses of less than 0.28 dB for TE 0 mode and 0.35 dB for TE 1 mode within the wavelength range of 1500–1600 nm, accompanied by crosstalk levels lower than −30 dB. In our experimental tests, we achieved insertion losses of less than 0.89 dB for TE 0 mode and 0.44 dB for TE 1 modes within the 1530 nm to 1570 nm range, with crosstalk maintained below −25 dB. Furthermore, we conducted an experimental verification of the differences between the standard device and the boundary dilation/erosion device, observing an insertion loss degradation by 0.61 dB within a deviation range of ±40 nm, which demonstrates the device’s robustness to the fabrication. The proposed devices exhibit exceptional performance and feature a compact structure, thus holding significant potential for the development of future multimode integrated photonic circuits.

    Abstract Objective Considering the increasing integration of electronic health records (EHRs) into medical practice by healthcare organizations, it is especially pertinent to understand its actual usage by the general public in recent years. This study aims to explore factors associated with the frequency and diversity of EHR usage in the United States over time. Methods We analyzed three iterations (2017, 2018, and 2019) of the Health Information National Trends Survey (HINTS). HINTS is a national cross-sectional survey conducted by the National Cancer Institute to document attitudes and perceptions about health information access and use among American adults. Results Both frequency and diversity of EHR usage have slightly increased across the years. However, its overall usage still remained low. Three technology-related enablers (access to digital devices, access to the Internet, and perceived usefulness of EHRs) were positively related to EHR usage in all three iterations. In addition, perceived health status was a constant and negative predictor of EHR usage over years. Doctor–patient communication was positively associated with the frequency of EHR usage in two survey waves. Conclusions More initiatives to increase EHR usage in the United States are needed. We advocate for providing affordable Internet access and smartphone to underserved populations; in medical encounters, doctors should have more patient-centered communication, introduce the benefits of EHRs to patients, and promote EHR adoption in terms of frequency and diversity.

    Abstract Iron metabolism disorders are implicated in the pathogenesis of Alzheimer's disease (AD). It was previously reported that transferrin receptor (TFR1) expression was upregulated in AD mouse model. However, the precise biological functions of TFR1 in AD progression remains unclear. Herein, we observed a gradual increase in TFR1 protein expression during the differentiation of AD patient-derived induced pluripotent stem cells (AD-iPS). TFR1 knockdown inhibited the protein expression of ferritin and ferritin heavy chain 1 (FTH1), enhanced the expression of ferroportin 1 (FPN1), and decreased intracellular levels of total iron, labile iron, and reactive oxygen species (ROS). Moreover, TFR1 knockdown improved mitochondrial membrane potential (MMP), increased adenosine triphosphate (ATP) content, downregulated mitochondrial fission proteins, and upregulated mitochondrial fusion proteins. TFR1 knockdown alleviated iron overload and mitochondrial dysfunction in neural cells differentiated from AD-iPS, while TFR1 overexpression showed the opposite results. Additionally, TFR1interacted with glycogen synthase kinase 3 beta (GSK3B) and promoted GSK3B expression. GSK3B overexpression reversed the inhibitory effects of TFR1 knockdown on iron overload and mitochondrial dysfunction in AD-iPS differentiated neural cells. In conclusion, TFR1 knockdown alleviated iron overload and mitochondrial dysfunction in neural cells differentiated from AD-iPS by promoting GSK3B expression. Our findings provide a potential therapeutic target for the treatment of AD.

    Abstract The perturbation of atmosphere turbulence is a significant challenge in orbital angular momentum shift keying-based free space optical communication (OAM-SK-FSO). In this study, we propose an adaptive optical demodulation system based on deep learning techniques. A conditional convolutional GAN (ccGAN) network is applied to recover the distorted intensity pattern and assign it to its specified class. Compared to existing methods based on convolutional neural networks (CNNs), our network demonstrates powerful capability in recovering the distorted light beam, resulting in a higher recognition accuracy rate under the same conditions. The average recognition accuracy rates are 0.9928, 0.9795 and 0.9490 when the atmospheric refractive index structure constant $C_n^2$ is set at 3 × 10

    Abstract Background. Stomach adenocarcinoma (STAD) has an extremely high fatality rate worldwide, and survival after metastasis is extremely poor. Cytokine-like protein 1 (CYTL1) has prognostic significance in various tumors. We aimed to explore the impact and underlying molecular mechanisms of CYTL1 in STAD through bioinformatics analysis. Methods. We used R software to analyze CYTL1 expression in STAD samples (n=375) and normal samples (n=32) in The Cancer Genome Atlas database. Kaplan–Meier analysis was used to verify the relationship between CYTL1 expression and overall survival (OS) and disease-specific survival (DSS) based on the clinical characteristics and subgroups of patients with STAD. Furthermore, univariate and multivariate Cox regression analyses were used to verify the outcome variables of OS and DSS in patients with STAD. Receiver operating characteristic curves were used to test the predictive power of CYTL1. The biological functions and signaling pathways of CYTL1 were determined using gene set enrichment analysis (GSEA), and the immune infiltration patterns of CYTL1 and correlation of immune-related markers were analyzed using single-sample GSEA (ssGSEA) and an estimate algorithm. Results. In our research, low CYTL1 expression (tumor vs. normal) was noted in patients with STAD. High CYTL1 expression was detrimental to OS and DSS and had good diagnostic performance (AUC=0.731). In the subtype analysis of STAD, T3 and T4 stages, N0 and N1 stages, M0 stage, gender (female), and age (≤65 years) showed different performances between OS and DSS. Univariate and multivariate Cox analyses identified CYTL1 as an independent factor, and logistic regression analysis indicated that CYTL1 was associated with M stage (OR=3.406) and sex (OR=1.535).     Abstract Objective. The present study observed the therapeutic effect and possible side effects of Q-switch 1064-nm laser with large-spot and low-energy technology in the treatment of patients with combined freckles and chloasma. Methods. A Q1064-nm laser with a large-spot diameter of 6–8 mm, energy level of 2.0–3.3 J/cm2, frequency of 10 Hz, and pulse width of 10 ns was employed for the treatment. Each patient underwent treatment 10–15 times, with an interval of 10–15 days each time. Facial care was administered before and after treatment; attention was paid to cleaning and moisturizing, avoiding light, and using sunscreens strictly. The therapeutic effects were observed and evaluated. Results. Freckles basically subsided (effective rate = 100%) and chloasma faded (effective rate = 39.4%). Furthermore, whitening and delicacy improvement were observed in the surrounding normal skin area. After laser treatment, confocal laser scanning microscopy revealed a large number of melanin particles in the upper part of the granular layer. Moreover, the amount of melanin in the middle and lower parts of the basal layer and spinous layer was significantly decreased. None of the patients developed postinflammatory pigmentation. Conclusion. In the treatment of freckles with chloasma, Q 1064-nm laser large-spot, low-energy technology not only removed freckles and faded chloasma but, most importantly, also reduced the incidence of postinflammatory pigmentation and improved patient satisfaction. This provided new methods and ideas for freckle laser treatment.

    Abstract Bone structure metrics are vital for the evaluation of vertebral bone strength. However, the gold standard for measuring bone structure metrics, micro-Computed Tomography (micro-CT), cannot be used in vivo, which hinders the early diagnosis of fragility fractures. This paper used an unpaired image-to-image translation method to capture the mapping between clinical multidetector computed tomography (MDCT) and micro-CT images and then generated micro-CT-like images to measure bone structure metrics. MDCT and micro-CT images were scanned from 75 human lumbar spine specimens and formed training and testing sets. The generator in the model focused on learning both the structure and detailed pattern of bone trabeculae and generating micro-CT-like images, and the discriminator determined whether the generated images were micro-CT images or not. Based on similarity metrics (i.e., SSIM and FID) and bone structure metrics (i.e., bone volume fraction, trabecular separation and trabecular thickness), a set of comparisons were performed. The results show that the proposed method can perform better in terms of both similarity metrics and bone structure metrics and the improvement is statistically significant. In particular, we compared the proposed method with the paired image-to-image method and analyzed the pros and cons of the method used.

    Abstract Successful in situ chemical oxidation (ISCO) applications require real-time monitoring to assess the oxidant delivery and treatment effectiveness, and to support rapid and cost-effective decision making. Existing monitoring methods often suffer from poor spatial coverage given a limited number of boreholes in most field conditions. The ionic nature of oxidants (e.g., permanganate) makes time-lapse electrical resistivity tomography (ERT) a potential monitoring tool for ISCO. However, time-lapse ERT is usually limited to qualitative analysis because it cannot distinguish between the electrical responses of the ionic oxidant and the ionic products from contaminant oxidation. This study proposed a real-time quantitative monitoring approach for ISCO by integrating time-lapse ERT and physics-based reactive transport models (RTM). Moving past common practice, where an electrical-conductivity anomaly in an ERT survey would be roughly linked to concentrations of anything ionic, we used PHT3D as our RTM to distinguish the contributions from the ionic oxidant and the ionic products and to quantify the spatio-temporal evolution of all chemical components. The proposed approach was evaluated through laboratory column experiments for trichloroethene (TCE) remediation. This ISCO experiment was monitored by both time-lapse ERT and downstream sampling. We found that changes in inverted bulk electrical conductivity, unsurprisingly, did not correlate well with the observed permanganate concentrations due to the ionic products. By integrating time-lapse ERT and RTM, the distribution of all chemical components was satisfactorily characterized and quantified. Measured concentration data from limited locations and the non-intrusive ERT data were found to be complementary for ISCO monitoring. The inverted bulk conductivity data were effective in capturing the spatial distribution of ionic species, while the concentration data provided information regarding dissolved TCE. Through incorporating multi-source data, the error of quantifying ISCO efficiency was kept at most 5 %, compared to errors that can reach up to 68 % when relying solely on concentration data.
