    Abstract Objectives: The present study aimed to investigate the effectiveness of treatment with romosozumab for one year and association between bone turnover markers and changes in bone mineral density (BMD) in patients with postmenopausal osteoporosis (PMO).
    Methods: Participants were 53 treatment-naïve PMO patients. Correlations of percent changes (Δ) in lumbar (L) and total hip (TH) BMD 12 months after initiating romosozumab with baseline demographic factors and parameters of N-terminal propeptide of type 1 collagen (P1NP) and tartrate-resistant acid phosphatase (TRACP)-5b at baseline and months 1, 3 and 6 were assessed. Multiple regression analysis was performed on factors significantly correlated with ΔL-BMD and ΔTH-BMD at month 12.
    Results: ΔL-BMD and ΔTH-BMD at month 12 were 17.5% and 8.1%, respectively. Multiple regression analysis revealed that a high P1NP value at month 3 predicted large increases in L-BMD and TH-BMD at month 12. High total amount of P1NP values from baseline to month 6 was associated with large increases in L-BMD and TH-BMD at month 12, and was most strongly correlated with the P1NP value at month 3.
    Conclusions: A high P1NP value at month 3 predicted large increases in both L-BMD and TH-BMD at month 12 in PMO patients treated with romosozumab.

    Abstract Heart failure (HF) is a major global health problem afflicting millions worldwide. Despite the significant advances in therapies and prevention, HF still carries very high morbidity and mortality, requiring enormous healthcare-related expenditure, and the search for new weapons goes on. Following initial treatment strategies targeting inotropism and congestion, attention has focused on offsetting the neurohormonal overactivation and three main therapies, including angiotensin-converting enzyme inhibitors or angiotensin II type 1 receptor antagonists, β-adrenoceptor antagonists, and mineralocorticoid receptor antagonists, have been the foundation of standard treatment for patients with HF. Recently, a paradigm shift, including angiotensin receptor-neprilysin inhibitor, sodium glucose co-transporter 2 inhibitor, and ivabradine, has been added. Moreover, soluble guanylate cyclase stimulator, elamipretide, and omecamtiv mecarbil have come out as a next-generation therapeutic agent for patients with HF. Although these pharmacologic therapies have been significantly successful in relieving symptoms, there is still no complete cure for HF. We may be currently entering a new era of treatment for HF with animal experiments and human clinical trials assessing the value of antibody-based immunotherapy and gene therapy as a novel therapeutic strategy. Such tempting therapies still have some challenges to be addressed but may become a weighty option for treatment of HF. This review article will compile the paradigm shifts in HF treatment over the past dozen years or so and illustrate current landscape of antibody-based immunotherapy and gene therapy as a new therapeutic algorithm for patients with HF.

    Abstract The ionospheric anomalies possibly associated with large earthquakes have been reported by many researchers. In this paper, Total Electron Content (TEC), Ionosonde, and tomography analyses have been applied to investigate the spatial and temporal anomalies of ionospheric electron density prior to the 2011 Off the Pacific Coast of Tohoku earthquake (Mw9.0). Results show significant TEC enhancements and an interesting three dimensional structure prior to the main shock. As for temporal TEC changes, the TEC value increases 3-4 days before the earthquake remarkably, when the geomagnetic condition was relatively quiet. In addition, the abnormal TEC enhancement area in space was stalled above Japan during the period. Tomographic results show that three dimensional distribution of electron density decreases around 250 km altitude above the epicenter (peak is located just the east-region of the epicenter) and increases the mostly entire region between 300 and 400 km. Details and statistical tendency and differences from the case of geomagnetic storms will be also shown in our presentation.

    Abstract Conventionally, two-dimensional spatio-temporal variations and one-dimensional temporal variations such as GNSS-based TEC fluctuations and ionosonde data have been used for ionospheric disturbances. Ionospheric disturbances such as Pre-earthquake phenomena, and triggered by seismic ground motions, tsunamis, and typhoons, for which the physical mechanisms have not yet been elucidated, were also studied using GNSS-based TEC and Ionosonde data. In this study, we introduce the newly developed an ionospheric tomography system that is stable even during disturbances and can reconstruct the three-dimensional ionospheric electron density distribution. And we apply the developed tomography method to the GEONET data with typhoon signature and earthquake-related disturbances around Japan. Based on the obtained the three-dimensional structure, it is highly suggestive that ionospheric disturbances caused by typhoons were excited by atmospheric gravity waves, seismic fluctuations by sound waves, tsunami-induced fluctuations by atmospheric gravity waves, and earthquake precursors by electric fields. Details will be shown in the presentation.

    Abstract With the outbreak of COVID-19, attention has focused on measures to prevent droplet infection. Operating rooms, where we anesthesiologists mainly work, are equipped with various theories and techniques for performing surgical procedures and general anesthesia on patients with various infectious diseases, whether airborne, droplet, or contact infection, and are an environment where surgical procedures and general anesthesia can be safely performed on patients with compromised immune functions. Here, we describe the anesthesia management standards assuming COVID-19 from the viewpoint of medical safety, as well as the structure for supplying clean air in the operating room and the structure of a negative-pressure operating room.

    Abstract Aichi prefecture, Japan is predicted to be hit by Mega-earthquake. Aichi Prefectural Association of Midwives has been making efforts to improve disaster preparedness for pregnant women. This project aims to acquire area data of pregnant women for simulated studies of rescue activities. Number of women in census survey areas in Nagoya City was acquired from nationwide data of pregnant women by machine learning (Cascade-Correlation Learning Architecture). Quite high correlation coefficients between actual data and estimation data were observed. Rescue simulations have been carried out based on the data acquired by this study.

    Abstract The blood-brain barrier (BBB) plays pivotal roles in synaptic and neuronal functioning by sealing the space between adjacent microvascular endothelial cells. BBB breakdown is present in patients with mild cognitive impairment (MCI) or Alzheimer disease (AD). Claudin-5 (CLDN-5) is a tetra-spanning protein essential for sealing the intercellular space between adjacent endothelial cells in the BBB. In this study, we developed a blood-based assay for CLDN-5 and investigated its diagnostic utility using 100 cognitively normal (control) subjects, 100 patients with MCI, and 100 patients with AD. Plasma CLDN-5 levels were increased in patients with AD (3.08 ng/mL) compared with controls (2.77 ng/mL). Plasma levels of phosphorylated tau (pTau181), a biomarker of pathological tau, were elevated in patients with MCI or AD (2.86 and 4.20 pg/mL, respectively) compared with control subjects (1.81 pg/mL). In patients with MCI or AD, plasma levels of CLDN-5-but not pTau181-decreased with age, suggesting some age-dependent BBB changes in MCI and AD. These findings suggest that plasma CLDN-5 may a potential biochemical marker for the diagnosis of AD.
