    Abstract Introduction: Omental lipomas are rare tumors that are often asymptomatic. They may produce vague abdominal complaints. Radiological diagnosis may be challenging. We report a large omental tumor with pre-operative diagnostic ambiguity due to suspicious radiological appearance.
    Case presentation: A 61-year-old man presented to the emergency department with abdominal pain. Upon investigation, computed tomography and magnetic resonance imaging showed a large predominantly fatty omental tumor that was suspicious of liposarcoma. The patient underwent laparotomy with excision of a huge 5706-g tumor. The final diagnosis of lipoma was made histopathologically.
    Discussion: Although most omental lipomas are diagnosed by cross sectional imaging, radiological features may be confused with that of a liposarcoma. Pre-operative biopsy confirmation can be considered but is associated with the risk of complications and seeding. Surgical excision serves as a diagnostic and therapeutic modality, and can be achieved by open or laparoscopic approaches guided by tumor size.
    Conclusion: Omental lipomas are rare and most commonly asymptomatic until achieving a sizable mass. In such cases, the diagnosis can be challenging due to the possible confusion with liposarcoma, which may affect the management plan.

    Abstract The flood phenomenon in the Krueng Jreue Sub-Watershed, Aceh Besar, Indonesia, in recent years indicates biophysical damage to the land. Floods are influenced by factors from biophysical conditions of the land and high rainfall with small river cross-sectional capacity causing water to overflow the embankment and flood low areas. This research aims to analyse the flood vulnerability level in the Krueng Jreue Sub-Watershed, Aceh Besar, Indonesia. The results showed that flood vulnerability in the research area consisted of four classes: very vulnerable, vulnerable, moderately vulnerable and somewhat vulnerable, with each area averaging a score of 43.0, 38.8, 30.0 and 21.7. Types of land use that are particularly vulnerable to flooding are rice fields with a mean total score of 43.0. The vulnerable classes are found in settlements and moorings, with a total score of 42.0 and 36.5, respectively. While open land, shrubs, grasslands, primary forests and secondary forests are quite vulnerable to flooding, with a mean total score of 32.5 each: 30.0, 30.0, 28.0 and 27.0. The main components affecting flood vulnerability are rainfall, temperature and land use, while additional components are soil infiltration and slope. Mechanised hydrological disaster mitigation can be performed through optimisation of weir, embung, rorak and check-dam. Vegetative hydrological mitigation efforts can be performed by reforestation and agroforestry systems, maps and flood prediction. Non-technically, hydrological disaster mitigation efforts can be undertaken with legal policies, law enforcement, map creation and prediction of droughts and socialisation of legislation.

    Abstract Primary care physicians (PCPs) are commonly approached with concerns involving patient genetics. This is a challenge because most PCPs lack expertise in genetic testing compared to their genetic counselor counterparts. Currently, the recommended best practice is to refer patients for genetic testing based on cancer-related family history questionnaires with a genetic counseling referral to discuss their results and any implications. However, the extent to which PCPs are using these questionnaires for this purpose remains poorly understood. In this cross-sectional study, PCPs were presented with the American Cancer Society's seven recommended family history questions to determine the percentage who consider each to be an indicator for referral to a genetics specialist. Questionnaires were completed by 88 of 260 attending PCPs at a national primary care review conference. The main outcome was the percentage of PCPs who identified each question as a trigger for genetic testing. Secondary outcomes included correlations with years of practice, genetics training, and methods used to obtain patient family history. Only two of the seven questions were considered triggers by most PCPs (range, 76-83%). The remaining five had lower percentages (range, 22-55%). Years of practice did not influence the number of triggers identified (Spearman correlation coefficient test: r = 0.05, p = 0.68). Few PCPs (3.4%) felt they had good to excellent genetics training during residency. Only 44.3% had genetics specialists available for referral. Overall, low percentages of PCPs consider the American Cancer Society questions to be triggers for genetic testing referrals. Furthermore, many do not have a genetics specialist or counselor available for referral. Addressing these concerns may help PCPs understand the basics of genetic testing and use standardized questionnaires to make appropriate referrals to genetic specialists, thereby reducing inappropriate referrals and improving appointment access to this precious resource for those who truly need it.

    Abstract Although the protective effect of health insurance on population health is well established, this effect may vary based on race/ethnicity. This study had two aims: (1) to test whether having health insurance at baseline protects individuals over a 10-year period against incident chronic medical conditions (CMC) and (2) to explore the race/ethnic variation in this effect. Midlife in the United States (MIDUS) is a national longitudinal study among 25⁻75 year-old American adults. The current study included 3572 Whites and 133 Blacks who were followed for 10 years from 1995 to 2004. Race, demographic characteristics (age and gender), socioeconomic status (educational attainment and personal income), and health insurance status were measured at baseline. Number of CMC was measured in 1995 and 2005. Linear regression models were used for data analysis. In the overall sample, having health insurance at baseline was inversely associated with an increase in CMC over the follow up period, net of covariates. Blacks and Whites differed in the magnitude of the effect of health insurance on CMC incidence, with a stronger protective effect for Blacks than Whites. In the U.S., health insurance protects individuals against incident CMC; however, the health return of health insurance may depend on race/ethnicity. This finding suggests that health insurance may better protect Blacks than Whites against developing more chronic diseases. Increasing Blacks' access to health insurance may be a solution to eliminate health disparities, given they are at a relative advantage for gaining health from insurance. These findings are discussed in the context of Blacks' diminished returns of socioeconomic resources. Future attempts should test replicability of these findings.

    Abstract Objective: The purpose of this study was to use ultrasonography to measure femoral articular cartilage thickness changes during marathon running, which could support MRI studies showing that deformation of knee cartilage during long-distance running is no greater than that for other weight-bearing activities.
    Materials and methods: Participants included 38 marathon runners with no knee pain or history of knee injury, aged 18-39. Ultrasound images of the femoral articular cartilage were taken two hours before and immediately after the race. Femoral articular cartilage thickness was measured at both the medial and lateral femoral condyles.
    Results: The maximum change in femoral articular cartilage thickness, measured at the left outer lateral femoral condyle, was 6.94% (P=.006). All other femoral articular cartilage thickness changes were not significant.
    Conclusion: A change in femoral articular cartilage thickness of 6.94% supports our hypothesis that long-distance running does not induce deformational changes greater than that of regular daily activities. This study using ultrasonography supports MRI evidence that knee cartilage tolerates marathon running well.

    Abstract Musculoskeletal conditions of the upper and lower extremities are commonly treated with corticosteroid injections. Ketorolac, a parenteral nonsteroidal anti-inflammatory drug, represents an alternative injectant for common shoulder, hip, and knee conditions. A review of the current literature was conducted on the efficacy of ketorolac injection in musculoskeletal diseases. Several studies support the use and efficacy of ketorolac injection in subacromial bursitis, adhesive capsulitis, and hip and knee osteoarthritis. Given the systemic effects of glucocorticoid injections, ketorolac may be a safe and effective alternative in patients with musculoskeletal disease. However, more evidence is required to better understand the effects ketorolac has on the human body during inflammatory processes.
