    Abstract Contraceptive failure is the primary cause of unintended pregnancy in the United States. With obesity rates at epidemic proportions, any association between obesity and strategies that prevent undesired pregnancies constitutes a significant public health and economic concern. Unfortunately, the relationship between obesity and contraception has not been extensively studied. Evidence from several epidemiological studies suggests that obesity may increase failure of some hormonal contraceptives resulting in unplanned pregnancies. Obesity may make procedure-dependent contraceptive methods (i.e., sterilization and intrauterine devices) more technically challenging for the provider to perform. Hormonal contraceptives, on the whole, do not appear to adversely affect body weight and provide important noncontraceptive benefits (i.e., cancer protection). Some surgical interventions to treat bariatric issues may compromise the efficacy of orally dosed contraceptive methods. Overall, the Society of Family Planning strongly encourages the use of both hormonal and nonhormonal methods of contraception in obese women desiring pregnancy prevention with very few restrictions. Further studies are needed to determine the interrelationship between obesity and contraception. In addition, future contraceptive efficacy studies need to include women of differing BMIs to better reflect the population of women using these methods.

    Abstract Objective: To assess the use and public perception of naloxone through distribution and education by pharmacists at local health screenings in low-income communities in Western Pennsylvania.
    Methods: A prospective study was conducted, offering education on opioid use disorder and naloxone to individuals attending community outreach events in Allegheny County, PA. Participants with interest and willingness to use naloxone in the event of an opioid overdose were offered naloxone nasal spray and additional training. These individuals were asked to complete a survey at the time of naloxone provision and were recontacted by telephone at a later date to collect follow-up data. The primary objective of the study was to ascertain naloxone use at follow-up. Secondary objectives of the study were to measure the difficulty of obtaining naloxone, identify any change in knowledge of naloxone before and after pharmacist intervention, and assess public perception of stigma associated with naloxone initiatives. Data were analyzed using descriptive and univariate comparative statistics.
    Results: A total of 265 people received naloxone, and 132 (49.8%) completed the initial survey. Fifty-seven participants subsequently completed both surveys (43.2% follow-up). Naloxone was utilized by 3.5% of respondents over an average of 3 months. Participants' perception of obtaining naloxone before the study was evenly distributed (35.8% never tried, 32.1% difficult, and 30.2% not difficult). A total of 52.6% of respondents believed that naloxone initiatives decreased the stigma of illicit opioid use.
    Conclusion: Naloxone provided in the community setting was utilized by a small percentage of the general public in the 2- to 4-month follow-up. Pharmacists can provide naloxone education and training within communities affected by high opioid overdose rates.

    Abstract Transforming growth factor-beta 1 (TGF-β1), a pro-fibrotic tumour-derived factor promotes fibroblast differentiation in the tumour microenvironment and is thought to contribute to the development of pro-tumourigenic cancer-associated fibroblasts (CAFs) by promoting myofibroblast differentiation. miRNA dysregulation has been demonstrated in myofibroblast transdifferentiation and CAF activation, however, their expression varies among cell types and with the method of fibroblast induction. Here, the expression profile of miRNA in human primary oral fibroblasts treated with TGF-β1, to derive a myofibroblastic, CAF-like phenotype, was determined compared to untreated fibroblasts. Myofibroblast transdifferentiation was determined by the expression of alpha-smooth muscle actin (α-SMA) and fibronectin-1 extra domain A (FN-EDA1) using quantitative real-time PCR (qRT-PCR) and western blot. The formation of stress fibres was assessed by fluorescence microscopy, and associated changes in contractility were assessed using collagen contraction assays. Extracellular vesicles (EVs) were purified by using size exclusion chromatography and ultracentrifugation and their size and concentration were determined by nanoparticle tracking analysis. miRNA expression profiling in oral fibroblasts treated with TGF-β1 and their extracellular vesicles was carried out using tiling low-density array cards. The Database for Annotation, Visualization, and Integrated Discovery (DAVID) was used to perform functional and pathway enrichment analysis of target genes. In this study, TGF-β1 induced a myofibroblastic phenotype in normal oral fibroblasts as assessed by expression of molecular markers, the formation of stress fibres and increased contractility. TaqMan Low-Density Array (TLDA) analysis demonstrated that miR-503 and miR-708 were significantly upregulated, while miR-1276 was significantly downregulated in TGF-β1-treated oral fibroblasts (henceforth termed experimentally-derived CAF, eCAF). The gene functional enrichment analysis showed that the candidate miRNAs have the potential to modulate various pathways; including the Ras associated protein 1 (Rap1), PI3K-Akt, and tumour necrosis factor (TNF) signalling pathways. In addition, altered levels of several miRNAs were detected in eCAF EV, including miR-142 and miR-222. No differences in size or abundance of EV were detected between eCAF and normal oral fibroblast (NOF). Little overlap was observed between changes in cellular and EV miRNA profiles, suggesting the possibility of selective loading of EV miRNA. The study reveals miRNA expression signature could be involved in myofibroblast transdifferentiation and the miRNA cargo of their EV, providing novel insight into the involvement of miRNA in CAF development and function.

    Abstract Purpose: An innovative community residency program that provides training in the clinical and administrative aspects of specialty pharmacy practice is described.
    Summary: An ongoing rapid rise in U.S. approvals of specialty therapies for chronic diseases and conditions (e.g., Crohn's disease, cystic fibrosis, infertility, hepatitis C infection, multiple sclerosis) is fueling demand for pharmacists trained to meet the complex needs of patients receiving those therapies, which are typically subject to risk evaluation and mitigation strategy (REMS) requirements and limited distribution arrangements. In 2011, Duquesne University Mylan School of Pharmacy partnered with Walgreens Specialty Pharmacy to launch a one-year community residency program designed to (1) provide enhanced education on diseases and conditions targeted by specialty therapies, (2) develop well-rounded clinicians who are fully knowledgeable of the medications used to treat those disorders, and (3) prepare trainees for the operational and business-related challenges of specialty pharmacy practice. The first half of the residency year emphasizes direct patient care experiences, with team-based rotations focusing on specific disease states and health conditions; the second half of the residency program emphasizes operational-administrative training in areas such as clinical program development, resource utilization review, REMS compliance, contracting, and navigating manufacturer-sponsored patient assistance programs. Other program components include a one-month external rotation at a managed care organization, research projects, and teaching experiences.
    Conclusion: A unique community residency program jointly developed by Duquesne University and Walgreens Specialty Pharmacy prepares trainees for careers in diverse clinical, managed care, community-based, and academic practice settings.
