    Abstract Introduction: As a non-invasive method of tumor hyperthermia, high intensity focused ultrasound (HIFU) has been widely used in the treatment of various solid tumors in recent years. The purpose of this study was to investigate the effect of HIFU combined with ethanol on biological tissue lesions. Material and Methods: Firstly, 0.5ml 95% ethanol was injected into the porcine liver tissue in vitro, then HIFU was used to irradiate the porcine liver. The B-mode ultrasound and needle hydrophone were used to monitor the cavitation. A thermocouple was also used to measure the real-time focal temperature. The ultrasonic signal scattered at the focal point of HIFU irradiation was collected by the fiber hydrophone, and the attenuation coefficient was calculated. Finally, the attenuation coefficient was input into the Khokhlov-Zabolotskaya-Kuznetov (KZK) equation and combined with the Pennes equation. The thermal lesion of the porcine liver was simulated by MATLAB software. Results: The length of the long axis of the lesion area simulated by the attenuation coefficient of cavitation was closer to the length of the long axis of the actual measured lesion area with ethanol injection, but the length of the short axis of the simulated lesion area was smaller than that of the measured lesion area. However, the length of the long axis of the lesion area simulated by the attenuation coefficient of cavitation was larger than the length of the long axis of the lesion area simulated by the attenuation coefficient of liver at room temperature. The same results were obtained for the length of short axis. Conclusion: HIFU combined with ethanol can produce larger lesions to biological tissues and improve the therapeutic effect.

    Abstract Introduction: The large aperture concave spherical focused ultrasonic transducer has stronger acoustic focusing effect and can obtain good temperature rise effect. The purpose of this study was to explore the effect of different frequency, duty cycle and inner radius parameters on temperature rise of multi-layer biological tissue. Material and Methods: The simulation model of high-intensity focused ultrasound (HIFU) irradiated multi-layer biological tissue was constructed. By changing the irradiation frequency, duty cycle and inner radius of large aperture concave spherical focused ultrasonic transducer, the sound field and temperature field of multi-layer biological tissue were simulated and calculated by using Westervelt nonlinear acoustic wave equation and Pennes biological heat conduction equation, respectively. Results: The intensity of sound field increased with the increase of frequency, while it decreased with the increase of inner radius, but the duty cycle almost had no effect on the intensity of sound field. The focal temperature increased with the increase of frequency and duty cycle, but decreased with the increase of inner radius. Conclusion: By selecting appropriate parameters of transducer, the optimum temperature rise in the target area of biological tissue can be obtained by using a large aperture concave spherical focused ultrasonic transducer.

    Abstract Phthalic acid esters (PAEs) are the most commonly used plasticizers, and long-term or high levels of exposure to PAEs have a huge potential risk to human health. In this study, the theories of Hartree-Fock (HF) and density functional theory (DFT) with different hybrid methods and basis sets were used to calculate the theoretical Raman spectra of five PAEs, and the comparison of calculated spectra between different theories, hybrid methods, and basis sets was conducted to determine the suitable theory with hybrid method and basis set for PAEs. Also, the Raman vibrations were assigned to the Raman peaks of PAEs according to the theoretical and experimental Raman spectra. The results indicate that DFT is more suitable for the theoretical study of PAEs than HF. In DFT, the hybrid method of B3LYP is more applicable to the theoretical study of PAEs than B3PW91, and the basis set of 6-311G(d, p) obtains the most consistent theoretical Raman spectra with the experimental spectra for PAEs. This study finds the optimal combination of the theoretical method and basis set for PAEs, and it will contribute to the establishment of the Raman fingerprint and the development of rapid detection for PAEs in the future.

    Abstract Objective: The aim of this study was to analyze the characteristics of alleged medical malpractice litigation involving spine surgery in Beijing between 2013 and 2018.
    Methods: Two online legal databases, Wusong and Weike, were queried for court verdicts involving spine surgery from January 2013 to December 2018 in Beijing. For all included cases, data pertaining to defendants, plaintiffs, case outcomes, allegations, and verdicts were abstracted, and descriptive analyses were performed.
    Results: A total of 186 legal cases were identified, and 122 cases were excluded because of irrelevance or insufficient information. Of the patients in the 64 cases included in this investigation, 40.6% were male. The mean age of the plaintiffs was 53.2 ± 18.6 years. The most common complaint addressed in this study is inadequate consent (53.1%; n = 34), followed by additional surgery required (40.2%; n = 26), being unsatisfied with the outcome of surgery (17.6%; n = 11), postoperative paralysis (15.6%; n = 10), and postoperative infection (15.6%; n = 10). The most common primary disease among all the cases is lumbar spinal stenosis (28.1%; n = 18), followed by spinal tumor (18.8%; n = 12), cervical spondylosis (17.2%; n = 11), vertebral fracture (14.1%; n = 9), deformity (12.5%; n = 8), and others (9.3%; n = 6). Spine surgeons successfully defended themselves in 13 cases (20.3%), which resulted in no indemnity payment. The remaining 51 cases (79.7%) were closed with an average verdict payout of U.S. $22,597, which was significantly lower than the average compensation claimed by the plaintiff (U.S. $11,3762) (P < 0.05).
    Conclusions: This study comprehensively summarized the medical alleged malpractice litigation after spine surgery in Beijing. Given the rapid growth rate of spine surgery and the burden of related alleged medical malpractice litigation, it is valuable for spine surgeons to understand the potential legal impact of spine surgery. The most common complaint addressed in this study is inadequate consent. The findings in the present study highlight that in China, spine surgeons should pay more attention to communication with patients and perform spine surgery based on abnormal imaging as opposed to what the history and physical examination dictate, which can help reduce the litigation rate and improve the patient experience.
