    Abstract In this editorial, we have analyzed the historical evolution of rectal and breast cancer surgery, focusing on the progressive reduction of demolitive approaches and the increasing use of more conservative strategies, accompanied by a growing emphasis on perioperative treatments aimed at enhancing surgical outcomes. All of these changes have been made possible due to an increased awareness and understanding of oncological diseases and improved perioperative treatments.

    Abstract This paper describes the global eddying ocean–sea ice simulation produced at the Euro-Mediterranean Center on Climate Change (CMCC) obtained following the experimental design of the Ocean Model Intercomparison Project phase 2 (OMIP2). The eddy-rich model (GLOB16) is based on the NEMOv3.6 framework, with a global horizontal resolution of 1 / 16 <svg:svg xmlns:svg="http://www.w3.org/2000/svg" width="27pt" height="14pt" class="svg-formula" dspmath="mathimg" md5hash="3eab029591d1ac4385b9279b611a0b8a"><svg:image xmlns:xlink="http://www.w3.org/1999/xlink" xlink:href="gmd-16-6127-2023-ie00001.svg" width="27pt" height="14pt" src="gmd-16-6127-2023-ie00001.png"/></svg:svg> ∘ and 98 vertical levels and was originally designed for an operational short-term ocean forecasting system. Here, it is driven by one multi-decadal cycle of the prescribed JRA55-do atmospheric reanalysis and runoff dataset in order to perform a long-term benchmarking experiment. To assess the accuracy of simulated 3D ocean fields and highlight the relative benefits of resolving mesoscale processes, the GLOB16 performances are evaluated via a selection of key climate metrics against observational datasets and two other NEMO configurations at lower resolutions: an eddy-permitting resolution (ORCA025) and a non-eddying resolution (ORCA1) designed to form the ocean–sea ice component of the fully coupled CMCC climate model. The well-known biases in the low-resolution simulations are significantly improved in the high-resolution model. The evolution and spatial pattern of large-scale features (such as sea surface temperature biases and winter mixed-layer structure) in GLOB16 are generally better reproduced, and the large-scale circulation is remarkably improved compared to the low-resolution oceans. We find that eddying resolution is an advantage in resolving the structure of western boundary currents, the overturning cells, and flow through key passages.     Abstract We examine the past and projected changes in Arctic sea ice properties in 6 climate models participating in the High Resolution Model Intercomparison Project (HighResMIP) in the Coupled Model Intercomparison Project Phase 6 (CMIP6). Within HighResMIP each of the experiments are run using a reference resolution configuration (consistent with typical CMIP6 runs) and higher resolution configurations. The role of horizontal grid resolution in both the atmosphere and ocean model components in reproducing past and future changes in the Arctic sea ice cover is analysed. Model outputs from the coupled historical (hist-1950) and future (highres-future) runs are used to describe the multi-model, multi-resolution representation of the Arctic sea ice and to evaluate the systematic differences (if any) that resolution enhancement causes. Our results indicate that there is not a strong relationship between the representation of sea ice cover and the ocean/atmosphere grid: the impact of horizontal resolution depends rather on the examined sea ice characteristic and the model used. However, the refinement of the ocean grid has a more prominent effect compared to the atmosphere: eddy-permitting ocean configurations provide more realistic representations of sea ice area and sea ice edge. All models project substantial sea ice shrinking: the Arctic loses nearly 95 % of sea ice volume from 1950 to 2050. The model selection based on historical performance potentially improves the accuracy of the model projections and predicts the Arctic to turn ice-free as early as in 2047. Along with the overall sea ice loss, changes in the spatial structure of the total sea ice and its partition in ice classes are noticed: the marginal ice zone (MIZ) dominates the ice cover by 2050 suggesting a shift to a new sea ice regime much closer to the current Antarctic sea ice conditions. The MIZ-dominated Arctic might drive developments and modifications of model physics and parameterizations in the new generation of GCMs.

    Abstract Wind stress and turbulent heat fluxes are the major driving forces which modify the ocean dynamics and thermodynamics. In the NEMO ocean general circulation model, these turbulent air-sea fluxes (TASFs), which are components of the ocean model boundary conditions, can critically impact the simulated ocean characteristics. This paper investigates how the different bulk parametrizations to calculated turbulent air-sea fluxes in the NEMO4 (revision 12957) drives substantial differences in sea surface temperature (SST). Specifically, we study the contribution of different aspects and assumptions of the bulk parametrizations in driving the SST differences in NEMO global model configuration at ¼ degree of horizontal resolution. These include the use of the skin temperature instead of the bulk SST in the computation of turbulent heat flux components, the estimation of wind stress and the estimation of turbulent heat flux components which vary in each parametrization due to the different computation of the bulk transfer coefficients. The analysis of a set of short-term sensitivity experiments, where the only experimental change is related to one of the aspects of the bulk parametrizations, shows that parametrization-related SST differences are primarily sensitive to the wind stress differences across parametrizations and to the implementation of skin temperature in the computation of turbulent heat flux components. Moreover, in order to highlight the role of SST-turbulent heat flux negative feedback at play in ocean simulations, we compare the TASFs differences obtained using NEMO ocean model with the estimations from Brodeau et al. (2017), who compared the different bulk parametrizations using prescribed SST. Our estimations of turbulent heat flux differences between bulk parametrizations is weaker with respect to Brodeau et al. (2017) differences estimations.

In the Nucleus for European Modelling of the Ocean (NEMO) ocean general circulation model, these turbulent air–sea fluxes (TASFs) can critically impact the simulated ocean characteristics.     Abstract Impact studies of the Atlantic Multidecadal Variability (AMV) on the climate system are severely limited by the lack of sufficiently long observational records. Relying on a model-based approach is therefore mandatory to overcome this limitation. Here, a novel experimental setup, designed in the framework of the CMIP6-endorsed Decadal Climate Prediction Project, is applied to the CMCC climate model to analyse the remote climate impact of the AMV on the Northern Eurasian continent. Model results show that, during Boreal summer, an enhanced warming associated to a positive phase of the AMV, induces a hemispheric-scale wave-train response in the atmospheric circulation, affecting vast portions of Northern Eurasia. The overall AMV-induced response consists in an upper-tropospheric anomalous flows leading to a rainfall increase over Scandinavia and Siberia and to an intensified river runoff by the major Siberian rivers. A strengthening of Eurasian shelves' stratification, broadly consistent with the anomalous river discharge, is found in the proximity of the river mouths during positive-AMV years. Considering that Siberian rivers (Ob', Yenisei and Lena) account for almost half of the Arctic freshwater input provided by terrestrial sources, the implications of these findings for decadal variability and predictability of the Arctic environment are also discussed.

    Abstract Skillful sea-ice prediction in the Antarctic Ocean remains a big challenge due to paucity of sea-ice observations and insufficient representation of sea-ice processes in climate models. Using a coupled general circulation model, this study demonstrates skillful prediction of the summertime sea-ice concentration (SIC) in the Weddell Sea with wintertime SIC and sea-ice thickness (SIT) initializations. During low sea-ice years of the Weddell Sea, negative SIT anomalies initialized in June retain the memory throughout austral winter owing to horizontal advection of the SIT anomalies. The SIT anomalies continue to develop in austral spring owing to more incoming solar radiation and the associated warming of mixed layer, contributing to further sea-ice decrease during late austral summer-early autumn. Concomitantly, the model reasonably reproduces atmospheric circulation anomalies during austral spring in the Amundsen-Bellingshausen Seas besides the Weddell Sea. These results provide evidence that the wintertime SIT initialization benefits skillful summertime sea-ice prediction in the Antarctic Seas.

    Abstract Liver involvement after abdominal blunt trauma must be expected, and in up to 30% of cases, spleen, kidney, and pancreas injuries may coexist. Whenever hemodynamics conditions do not contraindicate the overcoming of the ancient dogma according to which exploratory laparotomy should be performed after every major abdominal trauma, a CT scan has to clarify the liver lesions so as to determine the optimal management strategy. Except for complete vascular avulsion, no liver trauma grade precludes nonoperative management. Every attempt to treat the injured liver by avoiding a strong surgical approach may be considered. Each time, a nonoperative management (NOM) consisting of a basic "wait and see" attitude combined with systemic support and blood replacement are inadequate. Embolization should be considered to stop the bleeding. Percutaneous drainage of collections, endoscopic retrograde cholangiopancreatography (ERCP) with papilla sphincterotomy or stent placement and percutaneous transhepatic biliary drainage (PTBD) may avoid, or at least delay, surgical reconstruction or resection until systemic and hepatic inflammatory remodeling are resolved. The pathophysiological principle sustaining these leanings is based on the opportunity to limit the further release of cell debris fragments acting as damage-associated molecular patterns (DAMPs) and the following stress response associated with the consequent immune suppression after trauma. The main goal will be a faster recovery combined with limited cell death of the liver through the ischemic events that may directly follow the trauma, exacerbated by hemostatic procedures and surgery, in order to reduce the gross distortion of a regenerated liver.
