    Abstract Case: Rod fracture (RF) occurred at L5-S level in a 79-year-old woman 7 months after spinal corrective surgery for adult spinal deformity (ASD). Four years after the surgery, pyogenic spondylitis occurred at the same level as RF. After the reinforcement of broken rods posteriorly, a transperitoneal approach was used for debridement and bone graft. However, prolonged infection, adhesive ileus, and small bowel perforation led to a total of 3 reoperations, resulting a colostomy. Three years after the reoperation, she was ambulatory with assistance.
    Conclusion: We need to follow-up postoperative ASD patients carefully because a single complication can lead to serious consequences.

    Abstract Sixth-generation mobile communication (6G) aims to further improve capacity and reliability by controlling the radio propagation environment. Millimeter wave (mmWave) high-frequency band communication offers large bandwidth at the cost of high attenuation, even for smaller distances. Due to this, fewer multiple input multiple outputs (MIMO) multiplexing is possible at the base station (BS). Distributed analog relay nodes with beamforming capability improve the received power and MIMO multiplexing of mmWave communication. Due to limited signal processing, the analog relay node cannot perform beam search and tracking using these mmWave reference signals. The beam search and tracking are possible at BS or user equipment at the cost of increased control overhead. To reduce this overhead and provide relay-based 6G communication, we propose user-driven relay beamforming methods which can obtain the benefits of a massive analog relay MIMO. Assuming vehicular-to-everything (V2X) as a 6G application, we considered a relay-beam control method that uses the user information (location, velocity, acceleration, and direction of the terminal) contained in intelligent transport systems (ITS) messages called Cooperative Awareness Message (CAM). Simulation results show that the proposed method significantly reduces the overhead and the obtains benefits of the massive analog-relay MIMO. Furthermore, the accuracy of CAM's location information, the control period, and the effects of UE mobility are evaluated and presented. The results also show that the proposed method can work effectively in future V2X applications.

    Abstract Purpose: To investigate whether the minimally invasive spinal fusion can provide the better outcome than conventional fusion surgery in the treatment of degenerative lumbar spinal diseases.
    Patients and methods: One hundred and thirteen patients who had undergone single-level fusion surgery for degenerative lumbar spinal diseases were examined with a minimum of one-year follow-up. There were 56 men and 57 women with a median age of 70s ranging 47-88. The following three-types of fusion surgery were performed; minimally invasive transforaminal interbody fusion after microscopic decompression through a unilateral approach with percutaneous pedicle screwing (MTLIF), transforaminal interbody fusion after microscopic decompression through a unilateral approach (TLIF), and posterior lumbar interbody fusion with posterolateral fusion after open decompression through a bilateral approach (PLIF). The purpose for limiting on single level degenerative spinal disease was that it would be easy to compare the surgical outcomes among the three groups.
    Results: There were no statistically significant differences among three groups in terms of VAS scores, RDQ scores, and all of the domains in the JOABPEQ scores at the baseline. The JOABPEQ score for pain-related disorders at 6 months after surgery was statistically significantly higher in MTLIF group than the other two groups (P = 0.023). There were no statistically significant differences in the scores of the other outcome measures among three groups in whole follow-up period.
    Conclusion: The current study demonstrated that the JOABPEQ score for pain-related disorders at 6 months postoperatively was significantly better in MTLIF group than in the other groups. Since lumbar degenerative diseases mostly consisted in elderly patients, less invasive surgeries are desirable. MTLIF resulted in a better health-related QOL at 6 months after surgery, and its outcomes at the final follow-up were non-numerical inferiority. The results strongly indicate that MTLIF is desirable surgery especially for elderly patients with degenerative spinal diseases.

    Abstract With the rise of COVID-19, the use of aerosol boxes when interacting with COVID-19 patients has increased. However, their use has been controversial. We have been involved in the development of a dome-shaped aerosol containment device with negative pressure (DAWN), an aerosol box that can maintain negative pressure inside at all times. There are two types of DAWN: one is mounted on a bed (bed type) and the other is mounted on a stretcher (stretcher type). Each device has its own characteristics and can be selected depending on the situation. The bed type has enough space inside to allow procedures to be performed easily. The stretcher type can be attached to a stretcher and can maintain negative pressure when the patient is being moved. Due to the negative pressure structure and easy change of nonwoven fabric adopted in both types of DAWN, it is expected to prevent the scattering of aerosol when it is removed, which is a problem of conventional aerosol boxes. DAWN will contribute to reducing the enormous psychological stress of medical personnel who treat infections, and will contribute to reducing aerosol dispersion.

    Abstract The next generation 6G wireless systems are envisioned to have higher reliability and capacity than the existing cellular systems. The reconfigurable intelligent surfaces (RISs)-assisted wireless networks are one of the promising solutions to control the wireless channel by altering the electromagnetic properties of the signal. The dual connectivity (DC) increases the per-user throughput by utilizing radio resources from two different base stations. In this work, we propose the RIS-assisted DC system to improve the per-user throughput of the users by utilizing resources from two base stations (BSs) in proximity via different RISs. Given an α-fair utility function, the joint resource allocation and the user scheduling of a RIS-assisted DC system is formulated as an optimization problem and the optimal user scheduling time fraction is derived. A heuristic is proposed to solve the formulated optimization problem with the derived optimal user scheduling time fractions. Exhaustive simulation results for coverage and throughput of the RIS-assisted DC system are presented with varying user, BS, blockage, and RIS densities for different fairness values. Further, we show that the proposed RIS-assisted DC system provides significant throughput gain of 52% and 48% in certain scenarios when compared to the existing benchmark and DC systems.

    Abstract The 6G wireless systems are expected to have higher capacity, reliability, and energy efficiency than the existing cellular systems. Millimeter-wave (mmWave) frequencies offer high capacity at the cost of high attenuation and blockage losses. Reconfigurable intelligent surface (RIS) assisted mmWave networks consist of smaller antenna elements that control the propagation channel between the base station (BS) and the user by appropriately tuning the phase and the reflection of the incident electromagnetic signal. The deployment of RIS is considered to be an energy efficient solution to improve the coverage of regions with high blocking probability. However, if every BS is associated with one or more dedicated RIS, then the density of RIS increases proportionally with the density of BSs. Hence in this work, we propose RIS sharing mechanisms where multiple BSs share one RIS. We formulate resource allocation of RIS sharing in terms of time and the RIS elements as an optimization problem, and we propose heuristics to solve both. Further, we present detailed simulation results to compare time and the element based RIS sharing methods for various scenarios with the benchmark and the RIS system without sharing. The proposed time and element based RIS sharing methods improve throughput upto 53% and 25%, respectively, compared to the RIS system without sharing in specific scenarios.

    Abstract In the nucleus of eukaryotic cells, chromatin is tethered to the nuclear envelope (NE), wherein inner nuclear membrane proteins (INMPs) play major roles. However, in Xenopus blastula, chromatin tethering to the NE depends on nuclear filamentous actin that develops in a blastula-specific manner. To investigate whether chromatin tethering operates in the blastula through INMPs, we experimentally introduced INMPs into Xenopus egg extracts that recapitulate nuclear formation in fertilized eggs. When expressed in extracts in which polymerization of actin is inhibited, only lamin B receptor (LBR), among the five INMPs tested, tethered chromatin to the NE, depending on its N2 and N3 domains responsible for chromatin-protein binding. N2-3-deleted LBR did not tether chromatin, although it was localized in the nuclei. We subsequently found that the LBR level was very low in the Xenopus blastula but was elevated after the blastula stage. When the LBR level was precociously elevated in the blastula by injecting LBR mRNA, it induced alterations in nuclear lamina architecture and nuclear morphology and caused DNA damage and abnormal mitotic spindles, depending on the N2-3 domains. These results suggest that LBR-mediated chromatin tethering is circumvented in the Xenopus blastula, as it is detrimental to embryonic development.

    Abstract Osteoporotic vertebral fractures are recognized as a serious problem in the aging society. In this study, we found that the cumulated ambulation score predicts returning home in patients with osteoporotic vertebral fractures. The cumulated ambulation score is an important piece of information in determining the destination of patients with osteoporotic vertebral fractures.
    Purpose: Osteoporotic vertebral fractures are a serious problem affecting the health status of the elderly, and if they require inpatient treatment, they may have difficulty deciding where to discharge. The study's purpose is to investigate whether the cumulated ambulation scores predict returning home for hospitalized osteoporotic vertebral fractures patients.
    Methods: The subjects were 120 osteoporotic vertebral fractures patients aged 65 years or older who were admitted to our hospital between April 2015 and March 2022. The cumulated ambulation scores for all subjects were measured in the 3-days right after admission. A multivariable analysis was performed with the dependent variable as whether the patient returned home and the independent variable as the cumulated ambulation score. Three models were created from the measured cumulated ambulation score, and each model was analyzed as an independent variable (model 1; score on the 1st day, model 2; total score on the 2-days, model 3; total score on the 3-days).
    Results: The length of hospitalization for the osteoporotic vertebral fracture's patients were 11.8 ± 5.3 days, and 80 (66.7%) returned home. Multivariable analysis showed that cumulated ambulation score was a predictor of returning home (model 1, odds ratio: 3.151, 95% confidence interval: 2.074-5.203; model 2, odds ratio: 2.234, 95% confidence interval: 1.685-3.187; model 3, odds ratio: 1.929, 95% confidence interval: 1.535-2.599).
    Conclusion: The cumulated ambulation score of patients with osteoporotic vertebral fractures right after admission is a factor that affected returning home and is useful in determining where patients are discharged.
