    Abstract Aufrechtes Stehen und Gehen sind bei Gesundheit so selbstverständliche Fähigkeiten, dass sie nebenbei gelingen. Im Fall von Erkrankungen verdeutlicht sich die Komplexität der Haltungskontrolle. Dieser Beitrag zeigt, welche Aspekte für Gleichgewicht und Gang wichtig sind. Das Verständnis dafür, was im Einzelfall gestört ist, bildet die Voraussetzung für eine gezielte und erfolgreiche Therapie.:

    Abstract Gangunsicherheit und Mobilitätseinschränkungen sind bei neurologischen Patienten ein häufiges Leitsymptom. Gerade bei älteren Patienten wird oft nicht gut differenziert, welche Funktionsstörungen im Einzelnen vorliegen. Qualitative und quantitative Analysen helfen, Teilursachen des Defizits zu differenzieren und den Verlauf nachvollziehbar zu dokumentieren. Die Messung kann die Therapie steuern, deren Erfolg dann erneut aufgezeichnet werden kann.

    Abstract Introduction: Critical illness polyneuropathy and myopathy (CIP/CIM) are frequent complications in the intensive care unit (ICU) with major consequences for the progress and outcome of subjects. CIP/CIM delays the weaning process, prolongs the hospital stay and increases the mortality rate. Additionally, it may have long-term consequences beyond the hospitalisation phase with prolonged disability. Even though there is growing interest in CIP/CIM, research about the clinical and post-clinical course as well as the middle-term and long-term outcomes of subjects with CIP/CIM is scarce. A large prospective study of critically ill subjects is needed with accurate diagnosis during the acute stage and comprehensive assessment during long-term follow-up.
    Methods and analysis: This prospective observational cohort study aims to compare the clinical and post-clinical course of chronically critically ill subjects with and without the diagnosis of CIP/CIM and to determine predictors for the middle-term and long-term outcomes of subjects with CIP/CIM. In addition, the influence of the preclinical health status and the preclinical frailty on the long-term outcome of subjects with CIP/CIM will be investigated.This single-centre study will include 250 critically ill patients who were invasively ventilated for at least 5 days at the ICU and show reduced motor strength. At five study visits at admission and discharge to neurological rehabilitation, and 12, 18 and 24 months after disease onset, a comprehensive test battery will be applied including assessments of functioning and impairment, independence, health-related quality of life, activity and participation, cognition, gait and balance, fatigue, mental health and frailty.Secondary objectives are the documentation of therapy goals, therapy content and achieved milestones during the rehabilitation, to evaluate the clinimetric properties of the Mini-BESTest in critically ill patients, and to evaluate the time course and outcome of subjects with CIP/CIM after SARS-CoV-2 infection.
    Ethics and dissemination: The study was approved by the ethical committee of the Ludwig-Maximilians University Munich. Participants will be included in the study after having signed informed consent.Results will be published in scientific, peer-reviewed journals and at national and international conferences.

    Abstract Background: After standard care, 55%-75% of patients after stroke show a persistent paresis of the upper limb (UL). Assistive devices are developed to increase the patients' level of independence in daily life.
    Objectives: To investigate the potential of Functional Electrical Stimulation (FES) to assist object manipulation in activities of daily life.
    Design: Seventeen patients after stroke were tested and analyzed in a randomized cross-over design.
    Methods: Functional grasping was assessed by means of the Action Research Arm Test (ARAT) and the modified Box and Block Test (mBBT), in one session with and another without FES assistance. The order of sessions was randomized. Patients' motivation was assessed after each session. Task performance and motivation were compared between conditions using the Wilcoxon test and subgroup analyses were performed for impairment severity by distribution-based mixed-factor analyses.
    Results: When analyzing the total ARAT, FES did not effectively assist the overall performance (
    Conclusion: Functional Electrical Stimulation has the potential to support object manipulation, but is dependent on impairment severity and object type. To observe a consistent orthotic effect, features of the stimulator should be further developed to generate appropriate grasps and forces across subjects and objects.

    Abstract Aging in vestibular structures with loss of hair cells in the inner ear starts early in life, but vestibular function usually remains relatively unimpaired up to advanced ages. However, dizziness and imbalance are common in the elderly and have substantial impact on the quality of life. Dizziness interferes with the everyday activities of 30% of persons over age 70. As causes can be multiple, including vestibular and non-vestibular components, it is crucial to detect the factors leading to dizziness and imbalance to maintain mobility and avoid secondary complications like falls and anxiety. The more common causes of dizziness and imbalance in old age are sensory deficits, such as bilateral vestibular hypofunction, polyneuropathy, and impaired visual acuity; benign paroxysmal positional vertigo; and central disorders such as cerebellar ataxia and normal-pressure hydrocephalus. Further relevant factors include sedative or antihypertensive medication, loss of muscle mass (sarcopenia), and the fear of falling. Many elderly persons have multiple risk factors at the same time. Treatable components include benign paroxysmal positional vertigo that can be effectively treated with specific physical maneuvers, sedating drugs that can be reduced in many cases, and sarcopenia that can be treated with physical training. If a specific cause for dizziness and imbalance can be identified, treatment is often successful even in advanced age. The common causes can be revealed by systematic clinical examination and selective laboratory testing.
