Abstract Problem considered: Medical students are vulnerable to poor sleep quality which may lead to attention difficulties and poor academic performance. Good quality sleep is needed for optimal neurocognitive and psychomotor functions as well as physical and mental health. This study aimed to assess the prevalence of poor sleep quality among undergraduate medical students during home confinement at the time of Covid-19 pandemic and analyze the relationship between sleep quality and relevant socio-demographic and psychological variables. Methods: A cross-sectional study was conducted among 284 undergraduate medical students of a teaching hospital of Bhubaneswar from October–November 2020. Using a semi-structured questionnaire, relevant information was collected. Sleep quality of the students was assessed using the Pittsburgh Sleep Quality Index (PSQI) questionnaire and mental health status of the students using Depression, Anxiety, and Stress Scale - 21 (DASS-21). Results: The prevalence of poor sleep quality among undergraduate medical students was 45%. Multivariate logistic regression revealed that students doing exercise for <3 days/week (AOR: 1.81, 95% CI: 1.01–3.23), spending ≥8 h screen time/day (AOR: 2.02, 95%: 1.12–3.66), having anxiety symptoms (AOR: 3.61, 95% CI: 1.72–7.57), and those who were not satisfied with own self (AOR: 2.69, 95% CI: 1.35–5.38) were more likely to report poor sleep quality. Conclusion: Poor sleep quality was prevalent among undergraduate medical students during their home confinement at the time of Covid-19 pandemic. Sleep educational programs, anxiety management, and lifestyle modifications can be recommended to improve sleep quality among medical students.

Abstract Problem considered: The study aimed to assess the body composition changes after dialysis in chronic kidney disease (CKD) patients undergoing hemodialysis and to determine the strengths of relationships between various body composition parameters and blood lipid levels in these patients. Methods: The cross-sectional study was conducted during May–September 2019 involving 97 patients (58 males and 39 females, age: 40–70 years) of CKD undergoing maintenance hemodialysis. The multifrequency bioelectrical impedance analyzer was used to measure various body composition parameters like body fat percent, lean mass percent, body mass index, body fat mass index, fat-free mass index, total body water percent, extracellular water percent, intracellular water percent, nutrition index, prediction marker, basal metabolic rate, and estimated average energy requirement. Blood lipid levels were collected from the patients’ clinical records. Results: All the body composition parameters except nutrition index and prediction marker differed significantly before and after dialysis. Various body composition parameters were significantly correlated with one or more lipid levels before and after dialysis. Conclusion: The body composition parameters change significantly after hemodialysis among CKD patients. Our findings suggest that body composition parameters, whether measured before or after dialysis could be useful in assessing dyslipidemia in patients receiving hemodialysis.

Abstract Introduction: Thyroid disorders, mostly hypothyroidism, being one of the common diseases during pregnancy, if untreated, affect the neuro-psycho development of neonates. Many factors influence thyroid function tests in pregnancy; ethnic variations or different environmental conditions being one of them. Aim: This study was done to find the prevalence of overt hypothyroidism, subclinical hypothyroidism, overt hyperthyroidism and subclinical hyperthyroidism in the pregnant women in all trimesters. Materials and Methods: The present cross-sectional study was carried out by the Department of Obstetrics and Gynaecology and the Department of Physiology over a period of one year. Pregnant women who consented; with uncomplicated singleton pregnancy attending the antenatal check up for first time at antenatal OPD were included for the study. Blood samples for the estimation of FT4 and TSH were collected during a specific time period of the day. Serum FT4 and TSH estimation were done using an electro-chemiluminescent immunoassay technique. Results: Amongst the total 505 enrolled pregnant ladies 428 were included in the study. There were (n=278, 65.0%), (n=146, 34.1%) and (n=4, 0.9%) women in their first trimester, second trimester and third trimester respectively. With TSH cut off at 2.5 mIU/L, prevalence of subclinical hypothyroidism was (n=159, 37.15%) and overt hypothyroidism was (n=48, 11.21%). Prevalence of subclinical hypothyroidism was (n=13, 3.04%) when TSH cut off 4.5 mIU/L was considered. There was no significant difference in prevalence of subclinical hypothyroidism or overt hypothyroidism in pregnant ladies in different trimesters. Prevalence of subclinical hypothyroidism and overt hypothyroidism was lesser in women in primi gravida than women in multi gravida. There were no women with subclinical hyperthyroidism or overt hyperthyroidism. Conclusion: Hypothyroidism is more prevalent among the thyroid disorders during pregnancy. Prevalence of subclinical and overt hypothyroidism is more in multiparous pregnant ...

Abstract Background: Cardiovascular diseases constitute one class of common contributors to morbidity and mortality worldwide. Prevalence of overweight and obesity has dramatically increased in developing countries and is related to cardiovascular risk factors. Anthropometric parameters have the advantages in daily clinical practice of being a simple to measure tool with good reproducibility, especially in a developing country like India. Aim of this study is to correlate some anthropometric variables with lipid parameters in healthy subjects and to assess the anthropometric variable which best reflects the altered lipid profile. Materials and Methods: A hospital based cross-sectional study was conducted after the Institutional Ethical Committee Clearance. Included participants (1187) were subjected to anthropometric measurements such as height, weight, waist circumference (WC), and hip circumference using standard procedures on the same morning of the day, as the blood sample was collected after overnight fast and estimated for fasting blood sugar and lipid profile. Results: There is a weak correlation between body mass index (BMI) and lipid parameters. Among all the anthropometric variables studied, WC is best correlated to lipid parameters. The mean values of lipid parameters were not significantly different in BMI <25 and BMI ≥25 groups. Conclusions: WC remains one of the simple and reliable variables which best reflects the lipid profile. In a developing country like India, where measurement of cardiovascular risk factors such as body fat saturation and lipid profile remains difficult in the rural population, WC may be used as an effective tool, without being used as a substitute.

Abstract Background: Recent burden on health care organization due to acute pain and simultaneous rise in the depression cases have been ringing alarms among the medical professionals. Aims: This study was taken up with a hypothesis that acute pain and depression are inter-linked and any acute painful condition leads to some behavioural and psycho-somatic effects which needs to be detected at the earliest possible. Settings and Designs: This relationship was established by designing a cross-sectional study where four most common causes of acute pain (i.e. post-labour, post-caesarean, postoperative, dental conditions) were studied in 70 subjects each and compared with 70 subjects of control group who presented to a tertiary care hospital in Bhubaneswar, without any painful condition or underlying depression. Materials and Methods: Various pain scales (Visual Analogue Scale- VAS, Verbal Rating Scale – VRS, Box Scale) were used to assess the degree of pain while Montgomery-Åsberg Depression Rating Scale (MADRS) was used to assess the level of depression in these subjects. Statistical Analysis: Using SPSS version 16 software, the correlation coefficient was determined between the depression levels and acute pain. Results: It was seen that the geriatric populations were more depressed, while the older adults perceived more pain. The females differed from males in their pain perception; they were more depressed and proved to be more tolerant to painful conditions such as post-partum acute painful conditions. Dental pain was found to be the second most reported painful condition after postoperative acute pain. A statistically significant strong correlation was established between depression and acute pain. Conclusion: Thus the psychosomatic component of pain needs to be evaluated on a priority basis by health providers.

Abstract Background: India estimates third highest number of HIV infections in the world, with about 2.4 million people currently living with HIV/AIDS. Adequately trained and sensitized healthcare professionals can play a vital role in combating this epidemic. Limited studies have explored knowledge and attitudes of medical students relating to HIV/AIDS, particularly in the eastern part of India. Methods: The present cross sectional study explored knowledge and attitudes of first year MBBS, BDS & BPT students of Kalinga Institute of Medical Sciences (KIMS), Bhubaneswar, Odisha on HIV/AIDS using a self-administered questionnaire. Data thus collected were analyzedand relevant statistics were calculated. Knowledge and attitude scores were determined and analysis of variance (ANOVA) test was used to examine the equality between the groups. Results: All students scored low on the overall knowledge scale (<10/15). Specifically, knowledgewas low on modes of transmission and treatment. Attitudinal scores in the areas of precautions and need for training on HIV was low for all the three streams. The willingness to treat HIV/AIDS patient was found to be high amongst study participants. Conclusion: There is a need and scope to provide correct and detailed information on HIV/AIDS for new entrants in medical and allied health sciences to help them acquire adequate knowledge and develop appropriate attitudes towards HIV/AIDS.
