    Abstract We explore the fractal nature of particle showers using Monte Carlo simulation. We define the fractal dimension of showers measured in a high granularity calorimeter designed for a future lepton collider. The shower fractal dimension reveals detailed information of the spatial configuration of the shower. It is found to be characteristic of the type of interaction and highly sensitive to the nature of the incident particle. Using the shower fractal dimension, we demonstrate a particle identification algorithm that can efficiently separate electromagnetic showers, hadronic showers, and nonshowering tracks. We also find a logarithmic dependence of the shower fractal dimension on the particle energy.

    Abstract Five soft tissue sarcomas with histological features of malignant fibrous histiocytoma were selected to illustrate their ultrastructural heterogeneity. One case displayed the mixture of fibroblastic and histiocytic cells characteristic of the majority of malignant fibrous histiocytomas. In 1 case the tumor was composed entirely of primitive mesenchymal cells. The other 3 cases showed lipogenic, neurogenic, and "granular-cell" differentiation, respectively. These findings emphasize the important role of electron microscopy in the precise diagnosis and classification of malignant fibrous histiocytoma.

    Abstract Background: Patient experience is increasingly used as an indicator of high quality care in addition to more traditional clinical end-points. Surveys are generally accepted as appropriate methodology to capture patient experience. No validated patient experience surveys exist specifically for adolescents and young adults (AYA) aged 13-24 years at diagnosis with cancer. This paper describes early work undertaken to develop and validate a descriptive patient experience survey for AYA with cancer that encompasses both their cancer experience and age-related issues. We aimed to develop, with young people, an experience survey meaningful and relevant to AYA to be used in a longitudinal cohort study (BRIGHTLIGHT), ensuring high levels of acceptability to maximise study retention.
    Methods: A three-stage approach was employed: Stage 1 involved developing a conceptual framework, conducting literature/Internet searches and establishing content validity of the survey; Stage 2 confirmed the acceptability of methods of administration and consisted of four focus groups involving 11 young people (14-25 years), three parents and two siblings; and Stage 3 established survey comprehension through telephone-administered cognitive interviews with a convenience sample of 23 young people aged 14-24 years.
    Result: Stage 1: Two-hundred and thirty eight questions were developed from qualitative reports of young people's cancer and treatment-related experience. Stage 2: The focus groups identified three core themes: (i) issues directly affecting young people, e.g. impact of treatment-related fatigue on ability to complete survey; (ii) issues relevant to the actual survey, e.g. ability to answer questions anonymously; (iii) administration issues, e.g. confusing format in some supporting documents. Stage 3: Cognitive interviews indicated high levels of comprehension requiring minor survey amendments.
    Conclusion: Collaborating with young people with cancer has enabled a survey of to be developed that is both meaningful to young people but also examines patient experience and outcomes associated with specialist cancer care. Engagement of young people throughout the survey development has ensured the content appropriately reflects their experience and is easily understood. The BRIGHTLIGHT survey was developed for a specific research project but has the potential to be used as a TYA cancer survey to assess patient experience and the care they receive.

    Abstract A method for the determination of erythrocyte protoporphyrin by magnetic circular dichroism is reported. The method was sufficiently sensitive to distinguish normal from elevated levels of erythrocyte protoporphyrin and may be useful for diagnostic screening purposes. Erythrocyte protoporphyrin levels determined by magnetic circular dichroism correlated well with levels determined by the fluorimetric method of Piomelli, viz. a plot of MCD versus fluorimetry yielded a straight line relationship with a slope of 0.98 +/- 0.01, y-intercept of 0.05 +/- 0.35 and correlation coefficient of 0.99.

    Abstract In order to assess minimum diagnostic criteria for synovial sarcoma, particularly the monophasic variety, and the inter-relationship between the monophasic and biphasic types, 32 examples were studied histologically, immunohistochemically (26 cases), and ultrastructurally (13 cases). Of the six biphasic synovial sarcomas examined by electron microscopy, the spindle cell component did not show evidence of epithelial differentiation or resemble the epithelial phase, but did appear fibroblastic; no tumor cells transitional between the spindle and epithelial component were evident. In contrast, all of the seven monophasic lesions had ultrastructural growth patterns and some cellular features approximating the epithelial cells of the biphasic variant. In 11 biphasic synovial sarcomas, epithelial membrane antigen was detected in the glandular epithelium of all cases and cytokeratins in eight cases; in no case were these antigens detected in the spindle cell regions of biphasic lesions. Of the 15 monophasic synovial sarcomas, two were positive for cytokeratins and four for epithelial membrane antigen. Thus, the detection of epithelial markers either immunohistochemically or by electron microscopy (or both) should be the minimal diagnostic criteria for monophasic synovial sarcomas. Based on these findings, it is suggested that monophasic synovial sarcomas do not represent the spindle cell or "stromal" phase of biphasic synovial sarcomas, but are a poorly differentiated variant of the latter. As others have suggested, these tumors are, in fact, carcinosarcomas and carcinomas of the soft tissues and the designation synovial sarcoma is inappropriate for this tumor class.
