    Abstract The properties of decays that take place during jet formation cannot be easily deduced from the final distribution of particles in a detector. In this work, we first simulate a system of particles with well-defined masses, decay channels, and decay probabilities. This presents the "true system" for which we want to reproduce the decay probability distributions. Assuming we only have the data that this system produces in the detector, we decided to employ an iterative method which uses a neural network as a classifier between events produced in the detector by the "true system" and some arbitrary "test system". In the end, we compare the distributions obtained with the iterative method to the "true" distributions.

    Abstract The application of machine learning methods to particle physics often does not provide enough understanding of the underlying physics. An interpretable model which provides a way to improve our knowledge of the mechanism governing a physical system directly from the data can be very useful. In this paper, we introduce a simple artificial physical generator based on the Quantum chromodynamical (QCD) fragmentation process. The data simulated from the generator are then passed to a neural network model which we base only on the partial knowledge of the generator. We aimed to see if the interpretation of the generated data can provide the probability distributions of basic processes of such a physical system. This way, some of the information we omitted from the network model on purpose is recovered. We believe this approach can be beneficial in the analysis of real QCD processes.

    Abstract This paper presents an investigation of the discharge propagation (DP) to the readout electrode that occurs with a microsecond time delay after a primary discharge that develops inside a GEM foil hole. A single hole THGEM (THick GEM) foil that enables a controlled discharge position and the induction of primary discharge with an over-voltage in the THGEM foil has been used in the initial DP measurements. In order to justify the use of a custom-made THGEM foil, additional measurements were made with a standard GEM foil. Correlated optical (with an ordinary SLR and a high-speed camera) and electrical measurements of the delayed DP were made for Ne-CO$_2$-N$_2$ (90-10-5) mixture and with different powering configurations. Measurements show that the delayed DP happens without a drift field, with an inverted induction field, inverted THGEM voltages or an inverted drift field. After the primary discharge, there is a charge transfer in the induction region at an induction field value below that of the onset field for DP. In the time between the primary discharge and the delayed DP, three different current regimes are observed, which suggests multiple charge transfer mechanisms in the induction region. High-speed camera recordings provide valuable insight into the time evolution of the primary and the delayed DP, especially when correlated with electrical measurements.

    Abstract Four physicians working in private family medicine offices in Dalmatian Hinterland described their first hand experience of teaching sixthyear medical students. They supervised students during the 2010/2011 academic year, in an area that is economically undeveloped, rural, and where a number of people live in extended families. Although hesitant at first, the patients came to like the interaction with students, and later even yearned to provide students with as much information as possible. They also liked the letters that students had to write to them about their illness, because they could take them home and look for information without needing to see the doctor. The students showed diverse attitudes to different types of work in family medicine offices, mostly depending on their plans for future career. In general, they either complained or hesitated to perform duties that they did not fully master during earlier education, especially working with children. They needed several days to adapt to direct contact with the patients, and were more relaxed and cooperative when working in pairs than alone. The physicians themselves felt that they profited both from the novelty in the everyday routine and from the exchange of their experiences with the students. They liked their young colleagues and admitted they could not objectively review their own work, knowledge and skills.

    Abstract Because of its infrequent and protean presentation and the lack of clinical data, the management of acute infections with the foodborne trematode Fasciola hepatica is challenging. We report four serologically confirmed cases that illustrate our experience with this parasitic infection in Chile. All patients were adults presenting with upper abdominal pain. Other symptoms included fever, nausea/vomiting, and cutaneous manifestations. In all cases, marked eosinophilia was present. All patients lived in an urban environment, and three reported the consumption of raw watercress. Computed tomography (CT) scans showed hypodense hepatic lesions, whereas ultrasonography findings were unremarkable. One patient suffered portal vein thrombosis, which might be a rare complication of acute fascioliasis. All patients were successfully treated with triclabendazole. Our case series demonstrates that patients with acute fascioliasis typically present with a combination of upper abdominal pain, marked eosinophilia, and hypodense hepatic lesions on CT imaging. Diagnosis should be confirmed by serological investigation. A history of recent consumption of raw watercress is an important finding, but in some patients the source of infection remains obscure.

    Abstract Background: Uncontrolled blood pressure remains an urgent issue in clinical practice worldwide. This study aimed to compare the characteristics and effectiveness of hypertension control in family medicine pratice in the first treatment year, in relation to the geographical position, socio-economic standard, and access to medical services and public pharmacies in urban, rural and island environments (city of Split vs. Dalmatian Hinterland vs. islands in Southern Croatia).
    Methods: A historical cohort study included 213 patients diagnosed from 2008 to 2014 with essential arterial hypertension (AH) and without related complications or diabetes mellitus. Each patient was followed up for 365 days from the visit when the diagnosis of hypertension was ascertained. Normotension was defined as arterial pressure < 140/90 mmHg. The annual cost of drugs prescribed for treating newly diagnosed hypertensive patient and the total price for defined daily dose per patient were also evaluated.
    Results: More than half patients achieved normotension within a year from the initial diagnosis in all family medicine practices (57.3%), without significant differences among the three geographic regions (P = 0.981). Higher initial systolic blood pressure was a positive predictive prognostic factor on achieveing normotension (odds ratio (OR) 0.96, 95% confidence interval 0.95-0.98). ACE inhibitors were the most commonly prescribed antihypertensive agents in monotherapy (35.1%), as well as considering overall prescriptions (25.2%). Calcium channel blockers were the most commonly prescribed initial BP-lowering single agents in urban areas (28.6%), whereas angiotensin-converting enzyme inhibitors were more common in rural (28.0%) and island areas (22.7%) (P = 0.037). The median annual antihypertensive drug cost was 169.4 (95% CI 151.5-201.8) Croatian kunas and was similar across the study sites.
    Conclusion: Multiple antihypertensive drugs, prescribed in accordance with the guidelines, lead to similar pharmacological effects. Primary care physicians seem to be able to overcome potential interfering socio-economic factors and successfully achieve normotension in newly diagnosed patients with uncomplicated AH after 1 year of treatment.

    Abstract Aim: To determine the use of evidence-based medicine (EBM) information and the level of awareness and knowledge of EBM among patients in Croatia.
    Methods: A cross-sectional study was conducted among 987 patients in 10 family medicine practices in Croatia. Patients from both urban (n=496) and rural (n=482) areas were surveyed. A 27-item questionnaire was used to collect data about sources that patients searched for medical information, patient awareness and use of Cochrane systematic reviews and other EBM resources, and their demographic characteristics.
    Results: Half of the patients searched for medical information from sources other than physician. Internet was the most common place they searched for information. Very few patients indicated using EBM sources for medical information; one fifth of patients heard of EBM and 4% of the patients heard of the Cochrane Collaboration. Patients considered physician's opinion as the most reliable source of medical information. A logistic regression model showed that educational level and urban vs rural residence were the predictors of awareness about EBM and systematic reviews (P<0.001 for both).
    Conclusion: Our finding that patients consider a physician's opinion to be the most reliable source of health-related information could be used for promotion of high-quality health information among patients. More effort should be devoted to the education of patients in rural areas and those with less formal education. New avenues for knowledge translation and dissemination of high-quality health information among patients are necessary.

    Abstract Fifteen cases of human pseudoterranovosis are reported for Chile, representing an emerging parasitic infection in this country caused by larvae of the nematode Pseudoterranova sp. Our observations also included an outbreak of pseudoterranovosis in 3 of 4 individuals who shared the same raw fish dish (cebiche). Most of the cases occurred in adult patients. The main source of infection was from consumption raw or fried marine fish, including hakes (Merluccius australis or Merlucciuts gayi), pomfret (Brama australis), Inca scad (Trachurus murphvi), and corvina (Cilus gilberti). Seasonal distribution showed most of the cases to occur in fall and spring. Parasite larvae were isolated from the mouths of most of the patients after they reported a pharyngeal tickling sensation, coughing, vomiting, or a foreign body in the mouth or throat.

    Abstract The objective of this study was to investigate if there is specific host-parasite association in Chilean populations of Trypanosoma cruzi. For this purpose, two groups of parasites were analyzed, one from chronic chagasic patients, and the other from Triatoma infestans triatomines in three regions of the country. The first group consisted of four types of samples: parasites from peripheral blood of non-cardiopathic T. cruzi infected patients (NB); parasites from their corresponding xenodiagnosis (NX); parasites from peripheral blood of T. cruzi infected cardiopathic patients (CB) and parasites from their xenodiagnostics (CX). The T. infestans sample in turn was from three regions: III, V and M (Metropolitan). The genetic differentiation by the Fisher exact method, the lineage distribution of the samples, the molecular phylogeny and the frequency of multiclonality were analysed. The results show that not only are the groups of T. cruzi clones from Chagas disease patients and vectors genetically differentiated, but also all the sub-groups (NB, NX, CB and CX) from the III, V and M regions. The analysis of lineage distribution was concordant with the above results, because significant differences among the percentages of TcI, TcIII and hybrids (TcV or TcVI) were observed. The phylogenetic reconstruction with these Chilean T. cruzi samples was coherent with the above results because the four chagasic samples clustered together in a node with high bootstrap support, whereas the three triatomine samples (III, V and M) were located apart from that node. The topology of the tree including published T. cruzi clones and isolates was concordant with the known topology, which confirmed that the results presented here are correct and are not biased by experimental error. Taken together the results presented here are concordant with a specific host-parasite association between some Chilean T. cruzi populations.
