    Abstract Objective: To observe the effect of Electroacupuncture of Acupoints of the Neiguan(PC6) and Ximen(PC4) on its myocardial energy metabolism with acute myocardial ischemia rats caused by coronary artery ligation. Methods: Forty standard Spargue-Dawely(SD) rats were divided into four groups randomly, namely, group of Neiguan, group of Ximen, model group, sham group. The group of rats of myocardial ischemia were induced by coronary ligation, and the model rat of acute myocardial ischemia was prepared. The sham group had no ligation of the needle. After anesthesia was awakened, acupuncture was applied bilateral at Neiguan and Ximen of the group of Neiguan and Ximen, once a day at a fixed time for 14 days. The rats of model group and the sham group were not given acupunctured, but use the samely methods of Neiguan and Ximen groups to grap the rats at the same time everyday .After 14 days the experiment was over,observed the pathological changes of myocardial tissue by HE staining, detected the levels of free fatty acid (FFA), adenosine triphosphate (ATP) content, and the activities of Na+-K+-ATPase according to the reagents´demand. Results: Pathological results showed that acupuncuted at Neiguan and Ximen could improve the ischemic cardiomyocyte injury.Compared with the sham group, the levels of FFA in model group were increased (p<0.01), the levels of ATP,the activities of Na+-K+-ATPase were all decreased (p<0.01); Compared with the model group, the levels of FFA were decreased (p<0.05,p<0.01), the levels of ATP , the activities of Na+-K+-ATPase were improved. Conclusion: Acupuncturing at the Pericardium Meridian can alleviate the cell injury by myocardial ischemia,and also can improve the myocardial energetic metabolism in acute myocardial ischemia.

    Abstract Eutrophication is a global issue associated with increasing anthropogenic activities. Previous studies have mainly focused on nutrients and phytoplankton biomass in some typical estuaries and bays along the Guangdong coast, while integrated evaluations of eutrophication status based on ecological symptoms is still rare in this area. To better understand the health of the Guangdong coastal waters, two comprehensive methods including the Assessment of Estuarine Trophic Status (ASSETS) and the Northwest Pacific Action Plan Common Procedure (NOWPAP CP) were employed with slight modifications. The study area was divided into eight coastal zones (Z1~Z8) based on multiple criteria including salinity, catchment range, and administrative division. The results of the modified NOWPAP CP method demonstrated a generally increasing trend in the degree and effects of nutrient enrichment along the Guangdong coast in the past 30 years mainly due to the increasing nutrients and chlorophyll a (Chl-a). The results of the modified ASSETS method revealed that the water quality was between moderate and high for most coastal zones during 2015-2018, with the highest score (0.83) in the northern part of the Pearl River Estuary (PRE). However, the ecological symptoms showed inconsistent spatial patterns with the water quality, being high or moderate high in Z2 (including Zhanjiang Harbor and Leizhou Bay), Z4~Z5 (representing the northern and southern parts of the PRE, respectively), and Z6 (containing Mirs Bay and Daya Bay) for severe ecological symptoms, such as high levels of Chl-a, frequent harmful algal blooms (HABs). Moreover, eutrophication in Z4~Z6 may further deteriorate due to the increasing nutrient loads driven by growing economy and population. Synthetically, Z2, Z4~Z6 were graded between poor and bad for the overall eutrophication conditions (OEC), while Z1 (including the western and southern parts of the Leizhou Peninsula) and Z7 (consisting of Honghai Bay and Jieshi Bay) had a good OEC.     Abstract Firstly, according to the working characteristic diagram of engine and hydraulic torque converter, the matching working point is found. On this basis, the theoretical model of gear shifting law is derived when the engine and the hydraulic torque converter work together by taking the acceleration of adjacent two gears equal as the shift point. Finally, the theoretical model is simulated, the shift pattern diagram of tractor automatic transmission is obtained and the shift pattern diagram is analyzed, and the results are of important reference value for further study of shift delay and other characteristics.

    Abstract Diabetic retinopathy (DR), the most common microvascular complication that occurs in diabetes mellitus (DM), is the leading cause of vision loss in working-age adults. The prevalence of diabetic retinopathy is approximately 30% of the diabetic population and untreated DR can eventually cause blindness. For decades, diabetic retinopathy was considered a microvascular complication and clinically staged by its vascular manifestations. In recent years, emerging evidence has shown that diabetic retinopathy causes early neuronal dysfunction and neurodegeneration that may precede vascular pathology and affect retinal neurons as well as glial cells. This knowledge leads to new therapeutic strategies aiming to prevent dysfunction of retinal neurons at the early stage of DR. Early detection and timely treatment to protect retinal neurons are critical to preventing visual loss in DR. This review provides an overview of DR and the structural and functional changes associated with DR, and discusses neuronal degeneration during diabetic retinopathy, the mechanisms underlying retinal neurodegeneration and microvascular complications, and perspectives on current and future clinic therapies.

    Abstract Based on the sustainable livelihoods framework (DFID) and the theory of farmer behaviour, this study uses data points from 505 field-based household surveys conducted by the group in Liaoning Province in 2021–2022 to investigate the impact of livelihood capital on farmers’ adoption of integrated agricultural services (IASs). Using the entropy method and the intermediary effect model, this paper tests the mediating effect of value perceptions on the path of influence. The findings of this study show that the adoption rate of IASs is low and that farmers lack sufficient knowledge of these services. Moreover, natural, human, and social capital significantly and positively effect farmers’ IAS application behaviour. Farmers’ perceptions of economic value and ecological value also have a mediating effect on their IAS adoption. Accordingly, this paper proposes the following recommendations: attention should be given to the cultivation and publicity of IASs, policy support for IASs should be strengthened, the level of farmers’ livelihood capital should be improved, farmers should be guided to break through the constraints of their current livelihood capital, and farmers’ value perceptions should be improved enough to encourage them to take the initiative to participate in these services.

    Abstract Background While China has implemented reimbursement-linked drug price negotiation annually since 2017, emphasizing value-based pricing to achieve a value-based strategic purchase of medical insurance, whether drug prices became better aligned with clinical value after price negotiation has not been sufficiently established. This study aimed to assess the changes in prices and their relationship with the clinical value of anticancer drugs after the implementation of price negotiations in China. Methods and findings In this observational study, anticancer drug indications that were negotiated successfully between 2017 and 2022 were identified through National Reimbursement Drug Lists (NRDL) of China. We excluded extensions of indications for drugs already listed in the NRDL, indications for pediatric use, and indications lacking corresponding clinical trials. We identified pivotal clinical trials for included indications by consulting review reports or drug labels issued by the Center for Drug Evaluation, National Medical Products Administration. We calculated treatment costs as outcome measures based on publicly available prices and collected data on clinical value including safety, survival, quality of life, and overall response rate (ORR) from publications of pivotal clinical trials. The associations between drug costs and clinical value, both before and after negotiation, were analyzed using regression analyses. We also examined whether price negotiation has led to a reduction in the variation of treatment costs for a given value. We included 103 anticancer drug indications, primarily for the treatment of blood cancer, lung cancer, and breast cancer, with 76 supported by randomized controlled trials and 27 supported by single-arm clinical trials. The median treatment costs over the entire sample have been reduced from US$34,460.72 (interquartile range (IQR): 19,990.49 to 55,441.66) to US$13,688.79 (IQR: 7,746.97 to 21,750.97) after price negotiation (P < 0.001).     Abstract Purpose: Acute kidney injury (AKI) is a complication commonly occurred in patients with sepsis, and AKI has become the leading cause associated with mortality. PKM2, as a rate-limiting enzyme of glycolysis, was considered to be involved in AKI in vitro and animal models. However, there have been no studies reported on the expression of PKM2 in humans and its association with AKI.
    Methods: A retrospective study including 57 patients (35 males and 22 females) that were admitted into hospital in 2019 was carried out in our research. The basic characteristics and clinical parameters of each patient were collected from patients' medical records. We assessed changes in the expression of serum and urinary PKM2 using ELISA and its association with clinical manifestations in patients with sepsis through correlation analysis. Besides, ROC analysis was applied for evaluating the role of PKM2 in predicting AKI and death rate.
    Results: Urinary PKM2 is obviously increased in patients with sepsis-associated AKI (P < 0.05), while no significant change was found in the expression of serum PKM2. Moreover, the expression of urinary PKM2 is positively correlated with serum creatinine (r=0.577, P < 0.01) and blood-urea-nitrogen (r=0.531, P<0.01). In addition, it is negatively correlated with glomerular filtration rate (r=-0.583, P<0.01). Besides, ROC analysis indicated that urinary PKM2 could be a predictor of AKI in patients with sepsis (AUC-ROC, 0.819; SE, 0.086, P = 0.004, 95% CI 0.651-0.986).
    Conclusions: Urinary PKM2 could be a marker predicting acute kidney injury in patients with sepsis.

    Abstract To better protect the rights and interests of farmers, the evolutionary game theory and deep learning (DL) technology are used to analyze the conservation tillage behavior of farmers in black soil areas. Firstly, the basic hypotheses are put forward and an evolutionary game model is constructed. Secondly, the evolutionary game model between farmers and the government is analyzed, and dynamic equations are built. Finally, the model is deduced and studied, and a Convolutional Neural Network (CNN) with a double-sized convolution kernel is constructed to classify the land of remote sensing images. The experimental results manifest that after the dynamic evolutionary game, the net income generated by farmers adopting the conservation tillage strategy without government regulation is positive. The net income of government regulation is positive, and the game equilibrium point is (1, 0). After a dynamic evolutionary game, the game is balanced when the government does not regulate, the net income generated by farmers adopting conservation tillage strategies and the net income generated by government regulation and farmers adopting conservation tillage strategies are negative, the net income from government regulation is positive, and the game equilibrium point is (0, 1). The constructed CNN can achieve 91.32% overall accuracy for black soil classification, and the proposed scheme provides some references for the application of CNN in evolutionary games.

    Abstract In order to improve the performance of the diagonal loading-based minimum variance distortionless response (MVDR) beamformer, a full loading-based MVDR beamforming method is proposed in this paper. Different from the conventional diagonal loading methods, the proposed method combines the backward correction of the steering vector of the target source and the reconstruction of the covariance matrix. Firstly, based on the linear combination, an appropriate full loading matrix was constructed to correct the steering vector of the target source backward. Secondly, based on the spatial sparsity of the sound sources, an appropriate loading matrix was constructed to further suppress interferences. Thirdly, the spatial response power was utilized to derive a more accurate direction of arrival (DOA) of the target source, which is helpful for obtaining a more accurate steering vector of the target source and a more effective covariance matrix iteratively. The simulation results show that the proposed method can effectively suppress interferences and noise.

    Abstract Objective To construct and validate predictive models based on socioeconomic factors for predicting overall survival (OS) in cervical cancer and compare them with the American Joint Council on Cancer (AJCC) staging system.Design Retrospective cohort study.Setting and participants We extracted data from 5954 patients who were diagnosed with cervical cancer between 2007 and 2011 from the Surveillance, Epidemiology, and End Results Database. This database holds data related to cancer incidence from 18 population-based cancer registries in the USA.Outcome measures 1-year and 5-year OS.Results Of the total 5954 patients, 5820 patients had 1-year mortality and 5460 patients had 5-year mortality. Lower local education level [Hazard ratios (HR): 1.15, 95% confidence interval (CI): 1.04 to 1.27, p= 0.005] and being widowed (HR 1.28, 95% CI 1.06 to 1.55, p=0.009) were associated with a worse OS for patients with cervical cancer. Having insurance (HR 0.75, 95% CI 0.62 to 0.90, p=0.002), earning a local median annual income of ≥US$56 270 (HR 0.83, 95% CI 0.75 to 0.92, p<0.001) and being married (HR 0.79, 95% CI 0.69 to 0.89, p<0.001) were related to better OS in patients with cervical cancer. The predictive models based on socioeconomic factors and the AJCC staging system had a favourable performance for predicting OS in cervical cancer compared with the AJCC staging system alone.Conclusion Our proposed predictive models exhibit superior predictive performance, which may highlight the potential clinical application of incorporating socioeconomic factors in predicting OS in cervical cancer.
