    Abstract We apply convolutional neural networks (CNN) to the problem of image orientation detection in the context of determining the correct orientation (from 0, 90, 180, and 270 degrees) of a consumer photo. The problem is especially important for digitazing analog photographs. We substantially improve on the published state of the art in terms of the performance on one of the standard datasets, and test our system on a more difficult large dataset of consumer photos. We use Guided Backpropagation to obtain insights into how our CNN detects photo orientation, and to explain its mistakes.

    Abstract Aim and objective: The present study compared the stability of fracture fragments in the management of bilateral parasymphysis mandible fracture with Miniplate fixation and Reconstruction plate fixation using finite element analysis.
    Material and method: 3D FE Mandible model was created using CT scanner. Two bilateral parasymphysis mandible fracture models were created. Model 1 was fixed with Miniplates, and Model 2 was fixed with Reconstruction plate. Loading forces of 120 N at molar region and 62.5 N at incisor region were applied. These two models were imported to ANSY'S Workbench Software.
    Result: Miniplate fixation model showed comparatively reduced gap between fragments than Reconstruction plate. But the gap values of both the models were within the physiologic limit of healing under this specific loading. Analytically Miniplates were superior to Reconstruction plate in the management of bilateral parasymphysis fracture.
    Conclusion: Analytically Miniplates are superior to Reconstruction plate in the management of bilateral parasymphysis fracture. As the masticatory forces were reduced during fracture healing period, both fixations provide satisfactory healing. So both Miniplate and Reconstruction plate can be considered as fixation method for bilateral parasymphysis mandible fracture.

    Abstract Introduction: Third molars are common teeth to be impacted. The position of mandibular third molar is such that it is in close contact with inferior alveolar canal which may lead to nerve damage during its removal. So, this study was conducted to find out the prevalence of compression of inferior alveolar canal by mandibular third molars.
    Methods: This descriptive cross-sectional study was conducted on images collected from Dental Imaging Centers of Nepal from 25th June 2020 to 15th February 2021 after obtaining ethical clearance from Nepal Health Research Council (Reference number: 2100). A convenience sampling method was used to collect 433 cone-beam computed tomography images showing the relation between the third molars and inferior alveolar canal. Data were analyzed using the Statistical Package for the Social Sciences version 16. Point estimate at 95% confidence interval was calculated along with frequency and proportion for the binary data.
    Results: Out of 433 images, 135 (31.17%) (26.80-35.53 at 95% Confidence Interval) images showed compression of inferior alveolar nerve by mandibular third molar. The study result indicated that 16 (11.85%) buccally placed, 50 (37.03%) lingually placed and 69 (51.11%) inferiorly placed inferior alveolar canal were compressed by apices of mandibular third molars.
    Conclusions: The prevalence of compression of inferior alveolar canal by mandibular third molar was found to be similar to other studies done in similar settings. Compression of the canal was more evident when inferior alveolar canal is situated lingually.

    Abstract Introduction: Despite oral cancer being a common cancer in Nepal, little is known about oral potentially malignant disorders which have a high likelihood of malignant transformation. This study aims to find out the prevalence of oral potentially malignant disorders among patients attending the Department of Oral Medicine and Radiology of a tertiary care dental hospital.
    Methods: A descriptive cross-sectional study was carried out among 611 patients from 14th January, 2021 to 15th July, 2021 at the Department of Oral Medicine and Radiology of a tertiary care centre after receiving ethical clearance from the Institutional Review Committee (Reference number: 2306202002). Convenience sampling was done. Patients aged 18 years and above were enrolled into the study. Follow-up cases and patients requiring emergency treatment were, however, excluded. All demographic data, cancer and dietary history and clinical examinations were recorded. Descriptive analysis was done using Statistical Package for the Social Sciences version 22.0. Point estimate was done at a 95% Confidence Interval for frequency and percentages for binary data.
    Results: Among 611 total participants, the prevalence of oral potentially malignant disorders was found to be 26 (4.25%) (2.65-5.85 at a 95% Confidence Interval), where males were 19 (73.07%), females were 7 (26.92%), and 23 (88.46%) of them had history of consuming various forms of tobacco, areca nut, and alcohol.
    Conclusions: The prevalence of oral potentially malignant disorders in this hospital-based study was found to be lower than previous studies conducted in similar settings.

    Abstract The cutaneous metastasis is a late uncommon manifestation of cancer and indicates poor prognosis. The cutaneous metastasis can be a first presentation of underlying malignancy. The FNAC is an easy tool for rapid diagnosis of cutaneous metastasis. A 47-year-old female, known case of invasive ductal carcinoma presented to hospital with history of multiple nodular skin lesions over the body varying in size from 0.5-1.5 cm from past 2 months. The smears were cellular and comprises of loose clusters and singly scattered atypical epithelial cells, displaying moderate degree of pleomorphism with mild amount of cytoplasm, large irregular nucleus with fine to coarse chromatin, inconspicuous to conspicuous nucleoli and occasional atypical mitosis against a lipohemorrhagic background. The diagnosis of metastatic deposits of invasive ductal carcinoma was rendered. The clinical diagnosis of metastatic deposits can be challenges, especially in case of distant metastasis or when the cutaneous metastasis is the first presentation. The cutaneous metastasis can be confused clinically as adnexal lesion. The use of special stains and immunohistochemical markers can aid in diagnosis.

    Abstract Background: Burkitt and Burkitt like lymphoma (BL/BLL) are highly proliferative germinal or post-germinal B cell tumors. Few studies have evaluated the impact of autologous stem cell transplantation (ASCT) on disease outcomes.
    Aim: We performed a systematic review to analyze the efficacy of ASCT as frontline consolidation and for treatment of relapsed/refractory cases in adult BL/BLL.
    Materials and methods: Eligible studies with clear outcome measures on the efficacy of ASCT in adult patients with BL/BLL were identified through systematic search. The overall survival (OS), progression-free survival (PFS), complete response (CR), partial response (PR), and progression/relapse were used to assess the efficacy.
    Results: For patients who underwent ASCT in first CR, 5-year PFS and OS ranged between 70-78% and 70-83% respectively. For relapsed/refractory disease, 5-year PFS and OS were 27% and 31%, respectively. Patients undergoing ASCT for chemoresistant disease fared poorly with 3-year OS of 7% vs 37% for chemosensitive disease (p ≤ 0.00001). The overall response rate to ASCT for patients transplanted in first CR ranged between 71% and 93% and was 37% for patients who were transplanted in disease status other than first CR. Disease progression/relapse was observed in 16-29% of the patients transplanted in first CR, and 55% to 60% in relapsed disease.
    Conclusion: We found insufficient evidence to support ASCT over chemotherapy alone in the first remission for adult BL/BLL. Evidence supports guidelines recommending ASCT for chemosensitive disease but suggests there is no benefit to ASCT for chemoresistant disease.

    Abstract Eustachian valve (EV) is usually a rudimentary structure in adults. It is an embryological remnant of sinus venosus that directs oxygenated blood from the inferior vena cava across the foramen ovale and into the left atrium. Intravenous drug use is most commonly associated with infective endocarditis of the right-sided heart structures. Other documented causes of such an occurrence are intracardiac devices like pacemakers and central venous catheters. Patients presenting with concerns of infection and embolic phenomenon should promptly undergo evaluation for infective endocarditis. Although an embryological remnant, the eustachian valve normally regresses after birth, except in a minority of the patients, it persists as a vestigial structure. Here we present an unusual case involving infective endocarditis of the eustachian valve and tricuspid valve both in a patient with recent automatic implantable cardioverter-defibrillator (AICD) placement and history of IV drug abuse and its systemic consequences in a patient with patent foramen ovale.

    Abstract The present study was performed to investigate the therapeutic potential of β- Carotene against STZ induced diabetes by using in vivo and in vitro models. MTT assay was performed to check the cytotoxic effect of β- Carotene in HepG2 liver cells which were treated with β- Carotene (10, 20 μM). The anti-diabetic activity was examined by estimating different enzymes in cell lines. Further, we validated activity by using in vitro models. Male Albino Wistar rats were divided into five groups each group contain six animals (n = 6). The diabetes was induced via intraperitoneal injection of STZ and the β- Carotene was treated with daily doses of 10 and 20 mg/kg for 14 days. After the last dose of β- Carotene, rats were sacrificed and the biochemical parameters were estimated in liver homogenate. The disease control group showed an elevation in the level of cytokine as well as ROS and β- Carotene-treated animals showed a reduction in the level of cytokine and normal content of anti-oxidant enzyme in liver tissue homogenate. We found β- Carotene had no toxic effect on HepG2 liver cells. In the case of the glucose utilization assay, it was found that glucose uptake level was significantly increased with the increasing concentrations of β-Carotene. In conclusion β- Carotene improves glucose metabolism along with oxidative status in STZ-induced diabetic rats.
