Abstract This paper presents a detailed case study of a 48-year-old male who underwent ascending aortic aneurysm repair with a bioprosthetic valve five years prior and subsequently developed septic embolic encephalitis, an infrequent yet critical complication following cardiac valve replacement. The patient exhibited an array of initial symptoms, including generalized weakness, fatigue, fevers, chills, diarrhea, and altered mentation. Microbiological analysis of blood cultures revealed the presence of

Abstract The opioid epidemic is a significant public health crisis that has caused extensive harm and devastation in the United States. This literature review aimed to identify the contributing factors and negative consequences of the epidemic, as well as best practices for healthcare providers in managing the epidemic. Overprescribing opiates and opioids, lack of education and opportunity, and being unmarried or divorced were some of the identified contributing factors to dependence on opioids. The epidemic's negative consequences are substantial, leading to increased access to opioids for vulnerable populations, which consequently cause accidental death among men and the degradation of rural community health services. As part of the literature review, we also analyzed the best practices for healthcare providers, including implementing prescription drug monitoring programs (PDMPs). However, we found that while PDMPs resulted in a decrease in opioid overprescription and an increase in provider confidence when prescribing medication, the evidence for their effectiveness in improving rural community health services or reducing opioid overdoses and opioid-related deaths was inconclusive. Our review highlights that the greatest challenge to overcome is a lack of legal mandates and proper education for healthcare providers on best practices for addressing the epidemic. To regulate and control opioids effectively, tracking and standardizing prescription models by federal agencies and medical institutions is necessary but not enough. Legal action is vital for the successful containment of the opioid crisis.

Abstract Most inflammatory breast cancers are caused by invasive ductal cell carcinomas that arise from mammary epithelial cells lining the breast ducts. Typically, in these cancers, radiological signs are conspicuous, and a diagnosis is made after standard mammographic imaging or ultrasound. We report the case of a 54-year-old female who presented to a mammography clinic with right-sided breast pain and swelling. Upon physical examination, there was no palpable mass. Ultrasound and mammogram findings included mild skin thickening, normal-sized but irregularly shaped axillary lymph nodes, and no breast mass. Due to the presence of inflammatory changes (skin thickening) and abnormal lymph nodes but no obvious mass, an MRI was done to find the primary mass after a core needle biopsy of one lymph node showed metastatic ductal disease. In this patient, a 0.9 cm mass was found at the right 8:00 position on MRI. A second-look ultrasound was then performed and the mass was identified, followed by an ultrasound-guided core biopsy. The biopsy showed an invasive ductal cell carcinoma. In most cases, inflammatory breast cancer is associated with larger tumor sizes. However, a subset of patients with inflammatory breast cancer may present with a small primary breast tumor that causes inflammatory changes. Here, we present a rare case of inflammatory breast cancer associated with a small breast mass measuring less than 1 cm in size.

Abstract Background: Children with chronic kidney disease (CKD) and their families deal with challenging circumstances. While numerous studies have shown that both patients and parents in these families can experience a variety of challenges and concerns, the experience of siblings is less well understood. The focus of this scoping review was on research addressing the experiences and well-being of siblings of children with CKD.
Methods: Following scoping review methodology, five databases were searched for peer-reviewed research or graduate theses published in English that addressed the experience or well-being of siblings aged 25 years or younger (biological, step or foster) of children with CKD; studies from any year or location were included. Two independent coders identified relevant studies. Findings were summarized and synthesized.
Results: Of the 2990 studies identified, 19 were chosen for full text review and eight fit the inclusion criteria. Five of the selected studies were qualitative, two were quantitative and one used mixed-methods. Four broad themes across studies were identified including family functioning, significant relationships, psychological well-being, and coping strategies. While there was some convergence between qualitative and quantitative findings, these linkages were weak.
Conclusions: Several unmet needs of siblings were uncovered by this review. Sibling perceptions of differential parental treatment and desire for information about CKD emerged as priorities for practice. Using a strength-based approach in order to better understand sibling experiences and well-being was also recommended for future research. A higher resolution version of the Graphical abstract is available as Supplementary information.

Abstract This dissertation defines a new executive role of The Data Narrativist to address the emerging leadership needs of analytics organizations practicing data science. The Data Narrativist is designed to be an executive storyteller with expertise in the humanization of data driven communication. The Data Narrative Process provides an approach for managing exploratory analysis, analytics research, story creation and execution of data narratives. A new profile for this role is defined by a review of existing profiles from similar executive roles, a definition of emerging administrative roles, and an assessment of complementary methodologies. This thesis proceeds with an investigation into the structure and techniques of documentary film applied to the practice of creating narrative data visualization in video. Structural elements in documentary video are identified to engage a broad audience not formally trained in data visualization. An empirical analysis is performed using matched pairs of narrative visualization videos highlighting particular documentary techniques. With the accumulation of user preferences, voice over audio and the participatory mode are recommended to improve audience engagement of the analytical content. This work provides guidance for the construction of effective documentary narrative visualization videos as a form of enhanced data narrative. Documentary Data Narrative is incorporated into an evaluation of three unique types of enhanced data narrative to determine their effectiveness in knowledge transfer. Two examples of enhanced data narrative using data visualization are defined as Visual Data Narrative and Documentary Data Narrative. Another is defined as Computer Generated Text Data Narrative, constructed using Natural Language Generation (NLG) technology from the field of artificial intelligence. Together the three defined forms of enhanced data narrative are evaluated along with a traditional Human Generated Text Data Narrative. Each of the four types of data narrative is constructed from a common set of quantitative data measures regarding an earnings announcement for an established technology company. Based on the results of a set of knowledge retention questions, we perform a statistical analysis to determine the knowledge transfer performance for each form of data narrative. With this analysis we make observations about the effectiveness of the four discrete forms of data narrative.

Abstract Though there has been much written about dying and bereavement in recent years, the particular stress of terminal illness in childhood - as it affects both the families and the professionals - is only beginning to be better understood. In this book Dorothy Judd, a child psychotherapist who has worked with ill, disabled and dying children and adolescents for many years, places her clinical experience in the context of a full understanding of death, the moral and ethical issues raised by some of the treatments for life-threatening illness, and the current research into new developments in approa
